Abstract
PURPOSE: Esophagectomy with extended lymph node (LN) dissection is a standard treatment for resectable esophageal cancer to prevent recurrence, but severe, potentially life-threatening postoperative complications are still important issues. Accurate diagnosis of LN metastases would enable the decision to dissect or leave the LNs in regions with high risk of complications. Advancements in intraoperative gamma probe and radioactivity detectors have made intraoperative navigation surgery possible using a radiotracer as a marker. F-FDG is one such candidate markers, and the diagnostic power of FDG through counting the radioactivity close to each LN should be elucidated.Entities:

Clinicopathological Characteristics of 20 Patients
FIGURE 1Representative true-positive FDG PET/CT findings for patient 11. The primary esophageal lesion with the SUVmax of 16.5 (A), PET-positive finding in the right supraclavicular LN station with SUVmax of 4.2 (B), in the left recurrent nerve LN station with SUVmax of 3.5 (C), and in the gastric cardiac LN station with SUVmax of 3.6 (D), which are visually identified; the LN stations are pathologically confirmed to include metastatic LN.
FIGURE 3Representative false-positive FDG PET/CT finding for patient 8. A, The primary esophageal lesion has the SUVmax of 17.7. B, A PET-positive finding is identified in the left tracheobronchial LN station with SUVmax of 3.3, but pathologically, there is no metastatic LN.
