Abstract
BACKGROUND An elongated styloid process occurs with Eagle syndrome and causes a variety of symptoms. Fracture of the elongated styloid process is a rare condition. With spontaneous fractures, appropriate treatment cannot be performed in many cases due to unexplained symptoms and indefinite complaints that are not thoroughly examined. CASE REPORT We encountered a case of fractured elongate styloid process reaching the hyoid bone. Based on the findings from cervical 3-dimensional computed tomography (3D-CT), we chose surgery using the transcervical approach instead of the intraoral approach. Symptoms resolved following surgery. CONCLUSIONS A fractured styloid process is a rare event, but it needs to be recognized. If drug therapy does not work, surgical treatment is needed and has been shown to be effective. Surgical therapies include an intraoral approach and a transcervical approach. In the case presented here, 3D-CT was very useful for diagnosis and surgery selection.Entities:

Types of symptoms.
|
Sore throat Pharynx discomfort Tongue root pain Tongue root discomfort |
|
Pain with swallowing Discomfort when swallowing |
|
Pain and discomfort of the face and neck, which is the innervation domain Ear pain, tinnitus (because the glossopharyngeal nerve is distributed in the eustachian tube and tympanic plexus) |
|
Dull pain along the length of the carotid artery due to compression of the cervical sympathetic nerve Symptoms such as shoulder stiffness dissipated to the shoulder and arm through afferent fibers |
Treatment method and characteristics.
|
NSAIDs Carbamazepine Pregabalin Trigeminal nerve block | |
| • Less invasive | • Surgical entry is difficult |
| • No scarring on the neck | • Scarring on the neck |
| • Depending on the anatomical position, it can reach the base of the styloid process at short distances | • Due to the distance from the base of the styloid process, the surgical wound is large in the case of enucleation of the base part |
| • Risk of vascular injury due to the narrow field of operation | • Risk of secondary damage before reaching the styloid process |
|
Depending on the case, local anesthesia can be used Possibility of deep cervical infection from oral surgical wounds |
Resectable even if the styloid process is long and thick |
