Inclusion and exclusion criteria

Early unplanned hospital readmission (≤72 hours of acute care discharge). Aged ≥18 years. Readmitted to acute medical, acute surgical or short stay unit. Clinically stable. Able to consent to participate (English speaking, cognitively intact). | Clinically too unwell to participate. Unable to consent to participate (non-English speaking or cognitive impairment). |
