Abstract
BACKGROUND: We aimed to investigate whether various immune-related plasma proteins, alone or in combination with conventional clinical risk factors, can predict spontaneous preterm delivery (SPTD) and intra-amniotic infection in women with premature cervical dilation or a short cervix (≤ 25 mm).Entities:

aData for the histologic evaluation of the placenta were only available in 51 of the 80 women because in 10 cases, delivery took place at another institution and in 19 cases, histologic evaluation of the placenta was not performed because of our institutional policy that only the placentas in cases of preterm delivery are to be sent for histopathologic examination.
Regression coefficients, ORs, and 95% CIs of the final non-invasive predictive modela for predicting SPTD at < 32 weeks of gestation
| Predictors | Beta-coefficient | SE | OR (95% CI) | |
|---|---|---|---|---|
| Plasma IL-6, pg/mL | 0.312 | 0.112 | 1.367 (1.097–1.703) | 0.005 |
| Cervical dilatation, cm | 1.023 | 0.279 | 2.783 (1.611–4.806) | < 0.001 |

| Constant | −3.826 | 0.958 | 0.022 | < 0.001 |
The formula generated to predict SPTD at < 32 weeks was as follows: Y = loge (Z) = −3.826 + 0.312 × (plasma IL-6 [in pg/mL]) + 1.023 × (cervical dilatation [in cm]) + 0.072 × (plasma C3a [in ng/mL]). Z = eY and risk (%) = (Z/[1+Z]) × 100.
OR = odds ratio, CI = confidence interval, SPTD = spontaneous preterm delivery, SE = standard error, IL-6 = interleukin-6.
aFinal model resulting from a forward regression analysis including the following predictive parameters: cervical dilatation, C3a, C5a, and IL-6 in the plasma, and disease entity.
Fig. 1Receiver-operating characteristic curves in predicting spontaneous preterm delivery at less than 32 weeks by plasma IL-6 and C3a, cervical dilatation, and combined prediction model (including plasma IL-6 and C3a, and cervical dilatation). The area under the curve for plasma IL-6 and C3a, cervical dilatation, and combined prediction model was 0.750, 0.688, 0.793, and 0.901, respectively (P = 0.005 between plasma IL-6 and the combined prediction model, P < 0.001 between plasma C3a and the combined prediction model, P = 0.006 between cervical dilatation and the combined prediction model).
IL-6 = interleukin-6.
