Abstract
Background: The optimal treatment for a distal radius fracture (DRF) remains an ongoing discussion. This study observed whether early activity postinjury can lead to the prevention of type 1 complex regional pain syndrome (CRPS-1). Method: Patients who underwent nonoperative treatment for a DRF were invited to participate in this study. Patients followed an exercise program with progressive loading exercises at home immediately after cast removal. After a minimum of 3 months, patients were interviewed by telephone to determine the presence of disproportionate pain. If present, the patients were seen during a clinical consultation to determine whether they had CRPS-1, using the Budapest Diagnostic Criteria.Entities:

Figure 1.Example of a page from the instruction leaflet (English version). In this leaflet, the home exercise program is explained. Patients need to perform the exercises every day, and the exercises are intensified over the 3 months.
Figure 2.The subjective items of the Budapest Criteria are shown. These items were used to score the patients during the telephone interview. The items are translated to Dutch.
Budapest Diagnostic Criteria for CRPS-1.
| Continuing pain, which is disproportionate to any inciting event | Yes/no | |
|---|---|---|
| Must report at least 1 symptom in | Yes/no | |
| Must display at least 1 sign
| Yes/no | |
| There is no other diagnosis that better explains the signs and symptoms | Yes/no | |
| Diagnosis of CRPS-1 | Yes/no |
Note. Positive score indicating CRPS: if 1 sign in 2 or more of the above categories is checked as yes. CRPS-1 = type 1 complex regional pain syndrome.
A sign is counted only if it is observed at the time of diagnosis.
