Purple urine bag syndrome: case report.

A 65-year-old male with a history of urinary tract trauma requiring cystotomy and chronic bladder catheterization, presenting with chronic and uninvestigated changes in the color of the urine bag system, with no urine color change, and positive urine culture for Proteus mirabilis . These characteristics refer to the purple urine bag syndrome, a not weel-known condition, with a benign course in most cases, and associated with urinary tract infection in patients with chronic bladder catheterization. Although it is characterized by marked changes, it is underdiagnosed by healthcare professionals.

INTRODUCTION

The purple urine bag syndrome (PUBS) is a fairly unknown condition.( It is associated to urinary tract infection by bacteria that metabolize tryptophan into indigo and indirubin.( These pigments precipitate inside the urine bag system, giving it a purple hue.( The known risk factors include older patients, female, patients in long-term care institutions, constipation and alkaline urine.( Recognition and treatment are simple and important so that patients and family members can be educated to avoid excess management.( In this article we report the case of a patient with this condition.

CASE REPORT

A 65-year-old male obese patient, independent for activities of daily living, smoker and alcohol drinker as from 11 years of age (four cigarettes a day and 3 liters of sugarcane spirit/ cachaça per month). Patient reports falling from a tree (10 meter-high) 7 years ago, which resulted in pelvic trauma and hip fracture, and having an indwelling bladder catheter placed for an undetermined duration at the time. After removing the catheter, the patient had difficulty urinating; an urethrocystogram was performed in December 2012, showing an irregular urethra, with diffuse narrowing of the penile urethra. In October 2013, the patient evolved with urinary retention, hematuria and pyuria, in addition to testicular and anal injury, progressing to Fournier`s gangrene, requiring a suprapubic cystostomy. Since then the patient has been monitored in an outpatient setting, coming in to change the indwelling bladder catheter and the urinary bag every 30 days. In 2016, approximately 10 days after changes, the bag system started to show a bluish hue and, after 30 days, it turned purple, however, with no urine color change. During this interval, the patient was informed by healthcare professionals that the color changes were due to the ink used in the material of which the urine bag was made.

When evaluated for another change of the indwelling bladder catheter, the patient presented with a purple urine bag system ( Figure 1 ), yellow urine with a strong, foul smell, and pain in the hypogastrium. The patient`s diet was based on animal protein and fat, and he denied any comorbidities, use of medications, intestinal constipation, prior treatment for urinary tract infection, recent hospitalizations, and systemic symptoms. The patient had not taken proper care of the urine bag system, e.g . keeping it at the level of the shoulders, which led to backflow of contaminated urine into the bladder.

During the change, a sterile urine sample was collected and submitted for analysis and culture. He initiated treatment with trimethoprim-sulfamethoxazole, and a return visit was scheduled for 15 days later. Two weeks later, the urine bag had a bluish hue ( Figure 2 ) and laboratory tests showed abnormal elements and turbid sediments, bright yellow, density 1.005, pH 6.5, several pyocytes, red blood cells and negative nitrite. Also, the urine culture was positive for nitrofurantoin-resistant Proteus mirabilis .

Thirty days after collection, the urine still had a “bluish” hue, however less intense than before. Thus, treatment with ciprofloxacin was started, with 30-day control, and in the end, the patient had clear urine and the color of the urine bag system returned to normal ( Figure 3 ).

DISCUSSION

The purple urine bag syndrome was first reported in 1978.( It is a rare manifestation of urinary tract infection by bacteria that metabolize byproducts of tryptophan into red and blue pigments, of which the most common are Providencia spp ., Escherichia coli , Proteus spp ., Pseudomonas spp ., Klebsiella pneumoniae , Morganella spp . and Enterococcus spp . In the case reported, we detected the growth of P. mirabils .

Pigments are first produced in the gastrointestinal tract, where microbiome bacteria metabolize tryptophan into indole, which is transported in the bloodstream to the liver, where it is conjugated into indoxyl sulphate. It is excreted in urine, where bacterial sulfatases and phosphatases convert it into indoxyl which, in the presence of alkaline urine, is oxidated into indigo (blue) and indirubin (red).( The literature shows that nearly 10% of long-term care patients with an indwelling bladder catheter are at risk of developing PUBS.( The course of the syndrome is usually benign, although associated with higher morbidity and mortality, when compared to urinary tract infection alone.( With this in mind, it is important to properly take the patient`s clinical history, since an erroneous diagnosis could lead to improper management of patients with PUBS. In the case reported, although the patient did not have many of the associated risk factors (constipation, older age, long-term care or alkaline urine), there was a clinical history clearly consistent with PUBS. However, since this condition is fairly unknown by healthcare professionals, the patient was erroneously informed that the purple color was from the ink of the bag system, which put the patient at risk due to lack of proper management.

CONCLUSION

The purple urine bag syndrome is a fairly unknown condition, which leads to marked changes in the urine bag system due to bacterial metabolism in patients with indwelling bladder catheterization. Despite these changes, it is underdiagnosed by healthcare professionals unaware of the condition. Although the syndrome most often has a benign course, this is a risky situation since it is associated with urinary tract infection and higher morbidity and mortality among these patients.

A síndrome do saco coletor de urina roxo (SSCUR) é uma entidade pouco conhecida.( Ela está associada à infecção urinária por bactérias, que metabolizam o triptofano em indigo e indirubina.( Esses corantes precipitam no sistema coletor de urina, deixando-o com uma coloração arroxeada.( Fatores de risco atribuídos são idosos, sexo feminino, pacientes institucionalizados, constipação e urina alcalina.( O reconhecimento e o tratamento são simples e importantes para que o paciente e os familiares sejam orientados, e o manejo excessivo seja evitado.( Neste trabalho, iremos relatar o caso de um paciente com essa condição.

RELATO DE CASO

Homem de 65 anos, obeso, independente para atividades da vida diária, tabagista e etilista desde os 11 anos de idade (quatro cigarros por dia e 3L de cachaça por mês). O paciente refere queda de árvore (10 metros) há 7 anos, que provocou trauma pélvico com fratura de bacia, permanecendo com sonda vesical de demora por tempo indeterminado na época. Após retirar a sonda, apresentou dificuldade para urinar, sendo submetido à uretrocistografia em dezembro de 2012, evidenciando uretra irregular e com estreitamento difuso da uretra peniana. Em outubro de 2013, houve evolução para retenção urinária, hematúria e piúria, além de lesão em testículo e ânus, que evoluíram para síndrome de Fournier, necessitando de realização de cistotomia suprapúbica. Desde então, o paciente realiza acompanhamento ambulatorial, com troca de sonda vesical de demora e saco coletor a cada 30 dias. Em 2016, notou-se que aproximadamente 10 dias após as trocas, o sistema coletor passava a mudar sua coloração para um tom azulado, que, ao final de 30 dias, tornava-se arroxeado, porém, sem alteração da coloração da urina. Nesse intervalo, era informado por profissionais de saúde que as mudanças de tom seriam causadas pela tinta do material do saco coletor de urina.

Durante a avaliação para troca de sonda vesical de demora, o paciente apresentava sistema coletor roxo ( Figura 1 ), urina amarela e com odor fétido, e dor em hipogastro. Além disso, tinha alimentação baseada em proteína animal e gordura, e negava comorbidades, uso de medicamentos, constipação intestinal, tratamento anterior para infecção do trato urinário, internações recentes e sintomas sistêmicos. O paciente não realizava cuidados adequados com o sistema coletor de urina, por exemplo, acomodando-o nos ombros, o que permite o refluxo de urina contaminada para a bexiga.

Durante a troca, foi colhida urina de forma estéril, que foi enviada para análise e cultura. A partir disso, foi iniciado o tratamento com sulfametoxazol e trimetoprima, com retorno programado para 15 dias depois. Após 2 semanas, o saco coletor apresentava tom azulado ( Figura 2 ) e os exames complementares apresentavam elementos anormais e sedimentos de aspecto turvo, amarelo claro, densidade de 1,005, pH de 6,5, numerosos piócitos, hemácias e nitrito negativo. Além disso, os exames apontaram urocultura positiva para Proteus mirabilis resistente à nitrofurantoína.

Trinta dias após a coleta, a urina permaneceu com tom azulado, porém em menor intensidade que antes. Assim, foi iniciado o tratamento com ciprofloxacino, com controle de 30 dias, apresentando, ao final, urina clara e saco coletor sem alteração na coloração ( Figura 3 ).

DISCUSSÃO

O primeiro relato médico da SSCUR foi em 1978.( Trata-se de uma manifestação rara de infecção do trato urinário por bactérias que metabolizam produtos do triptofano em pigmentos vermelhos e azuis, sendo as mais comuns, Providencia spp ., Escherichia coli , Proteus spp ., Pseudomonas spp ., Klebsiella pneumoniae , Morganella spp . e Enterococcus spp . No caso relatado, evidenciamos o crescimento da bactéria P. mirabils .

A produção de corantes começa no trato gastrintestinal, em que bactérias da flora metabolizam o triptofano em indol, que é transportado pela circulação até o fígado, onde é conjugado em indoxil sulfato. Este é excretado na urina, onde sulfatases e fosfatases bacterianas o convertem em indoxil, que, na presença de urina alcalina, é oxidado em indigo (azul) e indirubina (vermelho).( A literatura aponta que quase 10% dos pacientes institucionalizados e em uso crônico de cateter vesical de demora podem desenvolver a SSCUR.( O curso dessa síndrome é geralmente benigno, embora esteja associado à maior morbimortalidade, se comparada à infecção do trato urinário isolada.( Sabendo disso, é importante colher uma boa história clínica, visto que o diagnóstico equivocado pode levar à condução inadequada de pacientes com SSCUR. No caso relatado, apesar de não apresentar muitos fatores de risco associados (constipação, idade avançada, institucionalização ou urina alcalina), havia história clínica claramente compatível com SSCUR. Mas por se tratar de uma condição pouco conhecida pelos profissionais de saúde, o paciente era informado equivocadamente que a coloração estava associada à tinta do sistema coletor, o que ocasionou riscos ao paciente pela falta de tratamento adequado.

CONCLUSÃO

A síndrome do saco coletor de urina roxo é uma entidade pouco conhecida, geradora de alterações marcantes no sistema coletor de urina, em decorrência de metabolização bacteriana em pacientes com cateterismo vesical de demora. Apesar dessas alterações, é subdiagnosticada por profissionais de saúde que desconhecem esta condição. Tal fato torna-se perigoso, pois, embora a síndrome tenha curso benigno na maioria das vezes, está associada a infecção urinária e maior morbimortalidade nesses pacientes.
