Fig. 1Ultrasonographic finding of the patient's right wrist. Extensor pollicis longus tendon rupture, wrist level, 4.31 cm gap. Hypoechoic fluid in the expected location of the tendon (arrow).
Fig. 2T2-weighted magnetic resonance imaging of the right hand. Complete tear of extensor pollicis longus tendon (A) Coronal view: no tendon in the gap (between 2 arrows) (B) Transverse view: no tendon in the expected course (arrow).
Fig. 3Thumb and wrist joints burden of the patient's tasks. Extension of the thumb and ulnar deviation of the wrist are thought to be burden on the extensor pollicis longus tendon. (A, B) Extension of the thumb: applying pressure while installing door trim and trunk garnish (C) Ulnar deviation of the wrist: holding the hand-arm vibration tool while injecting antifreeze liquid (D) Repetitive motion of the wrist: tightening a screw with the wrench while installing air cleaner, battery cable, and duct.

Score: 1–2, normal; 3–5, points cautious work; 5–6, points risk work; 7, points high-risk work.
Score criteria are shown as follows. *1: Wrist flexion/extension 20–45°, twist less than 30°, finger gripping; 2: Wrist flexion/extension more than 45°, twist more than 30°. †2: More than 10 times per minute or static posture for more than 10 seconds; 3: More than 20 times per minute or static posture for more than 30 seconds. ‡2: When using parts or tools requiring significant force on fingers and wrists; 5: Hard work that requires a lot of force. §1: Hammering with a hand more than 10 times per hour (A); 1: Using vibration tool more than 2 hours (B).

Numbers in bold are total scores and detailed JSI scores are expressed in parentheses intensity of exertion, duration of exertion, efforts per minute, hand/wrist posture, speed of work, duration of task per day (hours) in order.
JSI: job strain index.
*Three points of JSI refer to probably safe; †5 points associated with distal upper extremity disorders; ‡7 points probably hazardous.
