Dear Editor, A 51‐year‐old Italian policeman with diabetes and long‐standing, histopathologically confirmed psoriasis presented with green, velvety plaques on his sacrococcygeal and gluteal regions, surrounded by erythematous, scaly lesions (a). He reported excessive hyperhidrosis and use of a resistance elastic band to cover his psoriasis. Cultures of scrapings grew fluoroquinolone‐sensitive Pseudomonas aeruginosa.1 He started ciprofloxacin 250 mg twice daily for 7 days and secukinumab 300 mg subcutaneously. After 12 weeks of treatment with this anti‐interleukin‐17A inhibitor, the psoriasis totally cleared (b). Although some cutaneous infections or colonization with greenish discoloration due to P. aeruginosa are known, including green foot and green nail syndrome,2 this is the first report of green discoloration of psoriasis.
