**Body mass index (BMI) category was defined based on the WHO guidelines as underweight (<18.5 kg/m2), normal weight (18.5–24.9 kg/m2), overweight (≥25–29.9 kg/m2) and obese (≥30 kg/m2)—underweight and normal were collapsed together for this analysis.
