The primary outcome was patients’ understanding of key ACP concepts. Secondary outcomes were ACP documentation completed within 48 hours of hospital admission, concordance between patients’ expressed and chart-documented wishes for care, patients’ satisfaction with decision-making and patients’ decisional confidence.
