Primary level of education included those who had fully completed primary education and who partly completed secondary education without direct access to post-secondary or tertiary education. In our analyses, we did not include those who (1) did not report medical problems in the last 12 months (11.0% for all questions) (2) declined to answer the questions (2.3%, 2.2%, 1.8%, and 2.2% for the four questions, respectively), or (3) did not remember (1.9%, 1.9%, 1.4%, and 2.2%)
