The Authors Reply We greatly appreciate the valuable comment by Kobayashi et al. As mentioned, the bone marrow analysis is essential for the diagnosis of essential thrombocythemia (ET). Our short report did not intend to imply that a bone marrow biopsy is not essential for the diagnosis of ET (1). The patient was clinically diagnosed with ET and was treated with anagrelide by a hematologist before the onset of stroke. Given the limited space afforded by “PICTURES IN CLINICAL MEDICINE,” we expressed this as follows, “The examinations suggested not secondary but essential thrombocythemia, although bone marrow aspiration was not performed.”
