•An epidemiological model of causal pathways in the syndemic of diabetes and depression is presented.•Depression and diabetes are clustering in the adult Brazilian population partly due to shared social and behavioural causes.•There is an interaction between depression and diabetes in their association with disability.•There is a differential susceptibility to the effect of obesity on diabetes across levels of education and depression.Entities:
