Abstract
OBJECTIVE: Basic science studies demonstrated a general intramyocardial angiogenetic response potentially responsible for the creation of a microvascular neocapillaries network assisting myocardial function. We hypothesized that the benefit provided by the reperfusion of left anterior descending (LAD) territories and the biological angiogenetic drive triggered by the revascularization could translate in a global improvement in ventricular contractility, not restricted to the grafted area.Entities:

Inclusion and exclusion criteria.
| Coronary artery disease with LAD, LCX and RCA disease |
| Absence of severe valvular heart disease (e.g. aortic stenosis) |
| Coronary anatomy unfavourable to percutaneous coronary intervention |
| No previous percutaneous coronary interventions or cardiac surgery |
| EuroSCORE II > 6% |
| Left ventricular ejection fraction < 40% |
| Preoperative wall motion abnormalities in non-LAD territories |
| Exclusion criteria |
| Off-pump surgery performed for technical reasons (porcelain aorta, redo surgery) |
| Off-pump surgery performed for anatomical reasons (epicardial arteries < 1 mm, absence of conduits to perform aorto-coronaric grafts) |
