Integers reported as nationally representative count. Age reported as mean years and sd. LOS reported as mean days and sd followed by range (minimum to maximum). Percentages for bolded items are for totals within year, all other percentages are within procedure class. P-values reported using the Mantel-Haenszel test of trend for procedure categories, Pearson chi-squared or regression where appropriate.
