Abstract
BACKGROUND: Accurate record-keeping is important for continuity and quality of care. Completing a child's Road-to-Health Booklet (RTHB), or the older, less detailed, Road-to-Health Card/Chart (RTHC), immediate interpretation thereof and appropriate action facilitates comprehensive care, which could contribute to a decline in child morbidity and mortality.Entities:

Note: Documentation of birth weight, birth length, head circumference and APGARs were similar in the majority (90%) of RTHBs and RTHCs. However, documentation of mother’s blood group, antenatal and intrapartum history and neonatal feeding was better documented in the RTHB than in the RTHC (p < 0.000). Despite this, only 26% RTHBs had documented maternal antenatal history. There was no difference in the proportion of RTHBs and RTHCs that documented four or more neonatal characteristics, but maternal sociodemographic information was significantly better in RTHBs than in RTHCs, p = 0.0006 (Table 3).
