Except for age-specific mortality rates, overall and subgroup mortality rates were all age adjusted using the population of 2010 as the reference.

†Percentage change in rate was calculated as (mortality in 2013–mortality in 2006)/mortality in 2006×100%. 95% CI was estimated using negative binomial regression.
