Abstract
A 67-year-old woman presented with metamorphopsia in the right eye. Leopard mottling was seen temporal to the fovea oculus dexter with corresponding hyper- and hypo-autofluorescent lesions on fundus autofluorescence. Spectral domain-optical coherence tomography revealed hyperreflective dots in the retinal pigment epithelium and choroid with subretinal fluid (SRF). Intravitreal bevacizumab was administered with which SRF resolved, albeit with increase in the areas of mottling. The patient was diagnosed to have metastatic ductal carcinoma of the right breast. It is important to bear in mind that the well-known entity of bilateral diffuse uveal melanocytic proliferation can rarely present unilaterally.Entities:

Figure 1(Top) Reddish brown lesions seen temporal to the macula in the right eye (leopard mottling). (Bottom left) - Typical giraffe pattern seen in fundus autofluorescence. (Bottom right) - Normal autofluorescence of the left eye
Figure 2(Top) - Alternate areas of hyper- and hypo-autofluorescence - Giraffe pattern of autofluorescence corresponding clinically to the area of leopard mottling. (Bottom) - Increase in area of giraffe pattern, extending beyond the arcades after intravitreal bevacizumab
Figure 3(Top left) - Fundus fluorescein angiography mid-arteriovenous phase showing reversal of pattern seen in fundus autofluorescence [Figure 2 top]. (Top right) - Pinpoint hyperfluorescence seen above the superotemporal arcade (yellow arrows). (Bottom) - Indocyanine green angiography showing alternating hyper and hypocyancescence corresponding to giraffe pattern and multiple hypercyanescent spots throughout the fundus (yellow arrows)
Figure 4(Top) Areas of subretinal fluid, hyperreflective spots in the outer retina (white arrows), retinal pigment epithelium hyperplasia (yellow arrows) and atrophy. Hyperreflective spots in the choroid. (Blue arrows) - Elevation of the choroid seen in the subfoveal area and area temporal to leopard mottling. (Bottom) - Resolution of subretinal fluid and flattening of the contour of the choroid
