Sir, Thank you for the comments on the article, especially from the pioneers who have contributed a lot in defining the disease.[12] It is not only the selection of drugs and anaesthesia techniques that are ill defined but also issues such as the need to do electrophysiologic studies and automatic cardioverter defibrillator insertion before surgery in asymptomatic patients with Brugada electrocardiogram pattern. With a paucity of large multicentric studies on the subject, we feel the decision should be individualised depending on the extent and duration of procedure. Another question to be addressed is the safety of central neuraxial and plexus blocks considering the propensity of local anaesthetics to block sodium channels. These are really important issues considering the fact that the syndrome is more common in the Asian population and many patients may be asymptomatic before surgery!
