Abstract
RATIONALE: Radium-223-Dichloride (Ra-223) is an alpha-emitter, used to treat bone metastases. Patients with high metastatic burden and/or with increased trabecular bone uptake could present a higher incidence of hematologic toxicity. We hypothesized that these two factors are predictors of bone marrow failure.Entities:

Figure 1Segmentation analysis of SPECT/CT images. The program detects the compact bone border, on a slice-by-slice basis, by identifying the abrupt change of Hounsfield value (segmented edge). The mean density value of this volume is used to tell apart the trabecular bone (BVol) from the metastases volume (MVol) on the voxels lying inside the detected surface. The program then generates two masks, corresponding to BVol and to TVol, which are exported to the coregistered SPECT images, from which the functional information is extracted.

Figure 3Correlation between percent metastatic invasion and mean uptake of unaffected trabecular bone. The entity of metastatic invasion positively and tightly correlates with bone uptake within trabecular bone. Said uptake can present significant variations among patients.
Figure 4Correlation between appendicular BVol mean counts and blood values drop, after three (a, c) and six (b, d) cycles of Ra-223-Dichloride. PLT = platelets; WBC = leukocytes.
