Clinical features, previous treatments, and outcomes of patients treated with systemic tacrolimus
| Patient no./sex/age decade | Duration of disease before initiation of tacrolimus, years | Previous treatments, (maximum dosage and duration) | Tacrolimus dosage | Outcome |
|---|---|---|---|---|
| 1 | 5 | Prednisone (30-40 mg/d for 5 years) Hydroxychloroquine (400 mg/d for 4 months) Chloroquine (250 mg/d for 6 months) Quinacrine (100 mg/d for 3 months) Dapsone (100 mg/d for 2 months) Acitretin (20 mg/d for 3 months) Pentoxifylline (400 mg/3xd for 3 months) Leflunomide (20 mg/d for 5 months) Mycophenolate mofetil (2 g/d for 3 months) Methotrexate (7.5 mg/week) | 1 mg/2xd | Improved oral intake and pain resolved No gingival erythema and resolution of lesions in oral cavity Patient maintained on 1 mg daily monotherapy with no adverse effects |
| 2 | >20 | Prednisone (60 mg/d for >15 years) Dapsone (75 mg/d for unknown duration) Azathioprine (125 mg/d for 7 months) Methotrexate (17.5 mg/d for 4 months) Mycophenolate mofetil (2 g/d for 5 months) | 1 mg/2xd | Reduced gingival erythema Older lesions resolved Fewer new lesions but pain (burning) in mouth still present Patient flared upon cessation of tacrolimus |
| 3 | 3.75 | Prednisone (40 mg/d for 10 months) Hydroxychloroquine (400 mg/d for 3 months) Chloroquine (250 mg/d for 2 months) Dapsone (150 mg/d for 5 months) Methotrexate (20 mg/week for 5 months) Mycophenolate mofetil (2 g/d for 7 months) | 0.5 mg/2xd | Decreased pain and improved oral intake Decreased gingival erythema, previous erosions resolved Discontinued by primary admitting team during prolonged hospitalization to minimize risk of infection |
For patient 1, all medications taken before tacrolimus were discontinued because of a lack of response, with the exception of mycophenolate mofetil and methotrexate. Mycophenolate mofetil was discontinued after patient developed severe leukopenia at 2 g/d after failing to respond to a dosage <2 g/d. Patient developed severe leukopenia after 1 dose of methotrexate.
For patients 2 and 3, all medications given before tacrolimus were discontinued after no response was observed at optimal dosages over the specified duration of treatments.
