The Medication Optimization Value Proposition: Aligning Teams and Education to Improve Care.

Abstract

United States health care lags behind other countries in quality and cost. The present health care system is unsustainable, and there is now a quick movement toward value-based care. This article lays out essential care delivery elements, and makes the case for medication optimization to enable new value-based models.
