[Study on autophagy in nucleated red blood cells in patients with myelodysplastic syndromes].

Abstract

Objective: To investigate the change of autophagy level of bone marrow nucleated red blood cell (RBC) in patients with myelodysplastic syndromes (MDS) .
Methods: Fifty-four MDS patients and thirty-three controls were enrolled in this study. The mitophagy were observed by transmission electron microscopy (TEM) . The level of autophagy-associated protein LC3B in GlycoA(+) nucleated RBC was measured by flow cytometry. The expressions of ULK1 and mTOR mRNA in GlycoA(+) nucleated RBC were measured by real-time PCR. The expression of the mitochondrial outer membrane protein TOM20 in GlycoA(+) nucleated RBC was detected by Western blot.
Results: Autophagosomes or autolysosomes were scarcely observed by TEM in MDS patients. The expression of LC3B in GlycoA(+) nucleated RBC in high-risk MDS patients (0.22±0.12) was significantly lower than that in normal controls (0.43±0.22, P<0.001) , and lower than that in low-risk MDS patients (0.40±0.16, P=0.001) . The expression of AMPK [0.26 (0.60) ] in GlycoA(+) nucleated RBC in high-risk MDS patients was significantly lower than that in controls [1.00 (2.07) , P<0.017) . The expression of ULK1 mRNA in GlycoA(+) nucleated RBC in high-risk MDS patients [0.27 (3.31) ] was significantly lower than that in controls [1.07 (4.41) , P<0.017]. The level of mTOR mRNA in GlycoA(+) nucleated RBC in high-risk MDS patients [1.82 (3.74) ] was significantly higher than that in controls [1.26 (1.38) , P<0.017]. The level of LC3B in GlycoA(+) nucleated RBC was negatively correlated with the HGB (r=0.529, P=0.009) in high-risk MDS patients. The expression of mitochondrial outer membrane protein TOM20 in high-risk MDS patients was 9.42±4.42.
Conclusion: Autophagy is impaired in nucleated RBC of MDS patients.

骨髓增生异常综合征(MDS)是一组异质性克隆性造血干细胞疾病,其特征为一系或多系髓系造血细胞发育异常及高风险向急性髓系白血病(AML)转化[1],贫血是MDS最常见的症状[2]。过去自噬一直被视为细胞死亡的一种方式,近年来随着对自噬研究的深入,发现自噬是一种重要的机体适应性反应,在机体处于应激环境时行使重要的促存活作用[3]。自噬通过自噬-溶酶体途径特异性地降解细胞内成分[4],包括受损的线粒体和错误折叠的蛋白质[5]。我们既往对MDS患者骨髓单个核细胞自噬方面的研究结果提示自噬可能对MDS起保护作用[6]。本研究中,我们分析了MDS患者骨髓中有核红细胞自噬在MDS贫血发病机制中的作用,现报道如下。

病例与方法

1.病例:以2015年7月至2016年7月我科收治的54例MDS患者为研究对象,其中男30例,女24例,中位年龄60.5(27~79)岁。按照2008 WHO诊断分型标准,难治性贫血(RA)2例,难治性中性粒细胞减少(RN)1例,难治性贫血伴环状铁粒幼红细胞(RARS)5例,难治性血细胞减少伴多系发育异常(RCMD)15例,难治性贫血伴有原始细胞增多1型(RAEB-1)7例,RAEB-2 23例,5q−综合征1例;54例患者均进行了染色体核型分析,其中18例核型异常,包括+8、−6、−2、−5、−7、5q−、7q−、20q−等。按国际预后积分系统(IPSS)危险度分层,低危3例、中危-1 25例、中危-2 17例、高危9例。将低危与中危-1定义为相对低危组,将中危-2与高危定义为相对高危组。对照组33例,原发免疫性血小板减少症24例,特发性白细胞减少症9例,其中男13例,女20例,中位年龄52(24~74)岁。研究方案已通过我院伦理委员会批准,所有研究对象均签署知情同意书。 2.透射电镜观察有核红细胞线粒体自噬过程:取MDS患者新鲜骨髓液5 ml,分离骨髓单个核细胞(BMMNC),参照文献[7]方法制备电镜标本,采用日立H-600透射电镜进行观察。 3.流式细胞术检测GlycoA+有核红细胞LC3B的表达:取骨髓液400 µl,肝素抗凝,PBS洗涤3次,按照抗体使用说明书将GlycoA及同型对照(美国BD公司产品)分别加入Falcon管,每管中分别加入上述洗涤过的全血50 µl,混匀,4 °C避光孵育15 min,加入破膜剂A 100 µl,4 °C避光孵育5 min,加入溶血素1 ml,室温避光10 min溶解红细胞,离心弃上清,加入破膜剂B 40 µl,随后加入LC3B-PE,4 °C避光孵育15 min,PBS洗涤2次,上流式细胞仪进行检测。 4.免疫磁珠法分选GlycoA+有核红细胞:取400幕过滤BMMNC悬液10 µl,PBS洗涤1次并计数,约1×107个细胞中加入20 µl GlycoA磁珠抗体,4 °C孵育15 min,加入1~2 ml缓冲液洗涤1次,500 µl缓冲液重悬细胞,磁性条件下过柱,脱离磁场洗脱并收集细胞。经验证细胞纯度可达到95%以上。 5.real-time PCR检测AMPKULK1mTOR基因表达水平:取GlycoA磁珠分选的有核红细胞,TRIzol法提取总RNA样本,总RNA产物检测浓度及纯度后即按SYBR® Premix Ex Taq逆转录试剂盒(日本TaKaRa公司产品)说明合成cDNA。引物序列由上海生工生物工程股份有限公司设计、苏州金唯智生物科技有限公司合成,以GAPDH为内参。引物序列见表1。反应条件:95 °C 30 s,95 °C 5 s,60 °C 34 s,共50个循环。应用7500 Real-time PCR仪(美国Applied Biosystems公司产品)和软件进行熔解及扩增曲线分析,读取扩增曲线上各组的Ct值及各组间相对定量的倍数,基因表达量以2−ΔΔCt值表示。

6.Western blot法检测TOM20蛋白表达:在GlycoA磁珠分选的有核红细胞中加入150 µl细胞裂解液,置于冰上裂解30 min。离心提取蛋白。调整浓度至3 µg/µl,样品按4∶1加入5×上样缓冲液,进行SDS-PAGE。电泳后转膜,用含50 g/L脱脂奶粉的TBST缓冲液封闭1 h,加入5%牛奶稀释的一抗(TOM20:兔抗人IgG 1∶1 000;内参β-actin:兔抗人IgG 1∶500),4 °C孵育过夜。TBST洗涤3次,每次10 min。加入辣根过氧化物酶标记的二抗(鼠抗兔1∶1 000)反应1 h。TBST洗涤3次,每次10 min。化学发光法显影,ChemiDoc TM XRS+蛋白成像仪成像。 3.统计学处理:应用SPSS 21.0统计软件进行分析,符合正态分布的计量资料用均数±标准差表示,组间比较采用单因素方差分析,两两比较方差齐的采用LSD检验,方差不齐采用Tamhane's T2检验;不符合正态分布的计量资料用中位数(四分位间距)表示,组间比较采用Kruskal-Wallis H检验,组内两两比较调整显著性水准为0.017。相关性分析应用Pearson相关检验。

结果

1.透射电镜观察MDS患者有核红细胞自噬情况:我们对5例MDS患者及5例对照组患者GlycoA+有核红细胞进行了超微结构的分析,MDS患者不易见到自噬体及自噬溶酶体结构(图1),提示自噬体和自噬溶酶体形成障碍。由于病例数少,并未进行半定量分析。

A:部分细胞质与细胞器被包裹进一种特异性的双层膜或者多层膜结构的自噬体中;B:自噬体与溶酶体融合形成自噬溶酶体;C:细胞质和细胞器成分在自噬溶酶体里降解成为核苷酸、脂肪酸、氨基酸及其他小分子物质 2.MDS患者有核红细胞LC3B表达:相对高危组(23例)、相对低危组(21例)MDS患者及对照组(22例)GlycoA+有核红细胞LC3B表达水平分别为0.22±0.12、0.40±0.16及0.43±0.22,组间比较差异有统计学意义(F=9.882,P<0.001),进一步采用LSD检验进行两两比较,相对高危组LC3B水平显著低于对照组(P<0.001)及相对低危组(P=0.001),相对低危组LC3B水平与对照组比较差异无统计学意义(P=0.577)。提示相对高危组MDS患者GlycoA+有核红细胞LC3B表达水平降低。 3.MDS患者有核红细胞AMPKULK1mTOR mRNA表达:Kruskal-Wallis H检验结果见表2,对照组、相对低危组、相对高危组患者有核红细胞AMPKULK1mTOR mRNA组间比较差异均有统计学意义(AMPK:χ2=9.710,P=0.008;ULK1:χ2=14.303,P=0.001;mTOR:χ2=6.019,P=0.049),进一步两两比较显示,相对高危组患者AMPKULK1 mRNA明显低于对照组(P值均<0.017),mTOR mRNA明显高于对照组(P<0.017)。

5.MDS患者有核红细胞TOM20蛋白表达:我们检测了3例相对高危组、3例相对低危组MDS患者及3例对照组患者有核红细胞TOM20蛋白的表达水平,各组分别为9.42±4.42、3.04±1.54及1.36±0.40,提示相对高危组患者有核红细胞TOM20蛋白表达升高。

讨论

虽然最近的研究显示MDS患者中存在多种突变[8],但是单独这些突变不能概括MDS的表型,提示MDS患者中还存在其他通路的调节异常。贫血是MDS患者最常见的临床症状,我们研究了MDS患者骨髓有核红细胞的自噬水平,旨在分析其在MDS贫血发生机制中的作用。 首先,我们采用透射电镜证实了MDS患者有核红细胞中存在自噬过程。红细胞在终末分化过程中去除细胞核和细胞器,其中,去除线粒体是红细胞形成的重要过程。选择性线粒体自噬特异地将线粒体定位到自噬体,然后在溶酶体降解,已知自噬是清除完整的、受损的、功能紊乱的线粒体的唯一方式[9]。有研究已经证实,缺乏Atg1相关丝氨酸苏氨酸激酶ULK1,红细胞清除线粒体、核糖体延迟,也发生了贫血。 哺乳动物雷帕霉素靶点复合体1(mTORC1)在调节自噬通路中起重要的作用。它将PI3K/Akt通路、细胞营养状况或能量水平的信号通路整合起来。活化的mTORC1通过磷酸化自噬蛋白复合体(ULK1/2)抑制自噬。在能量水平偏低的情况下,AMPK通过丝氨酸苏氨酸激酶LKB1活化,然后活化TSC2抑制mTORC1活性,最终增加自噬活性。有研究证实红细胞成熟和稳态需要PRKAA1(AMPK1)依赖的自噬介导的线粒体清除[10]。ULK1促进传统的自噬而且促进红细胞成熟终末阶段线粒体的清除[11]。我们的结果显示相对高危组MDS患者AMPKULK1表达均显著低于对照组,mTOR表达显著高于对照组。提示相对高危组患者可能存在自噬缺陷。红细胞中mTOR的异常活化导致蛋白酶体介导的ULK1的降解,ULK1缺乏红细胞呈大细胞性,清除线粒体缺陷,红细胞寿命缩短。相对高危组MDS患者mTOR蛋白活化,可能抑制自噬水平,促进异常线粒体的积累,导致贫血。 条件性敲除小鼠造血干细胞必要的自噬基因Atg7Atg7−/−红细胞积累了大量导致细胞死亡的膜电位改变的线粒体[12],小鼠发展成贫血,表现型类似MDS。自噬不仅清除去极化的线粒体,而且还清除mtDNA突变的线粒体。Li-Harms等[13]利用年老的mtDNA突变小鼠mtDNA突变的积累研究了自噬与体内红细胞mtDNA突变相关的线粒体功能紊乱的关系。结果表明分化的红细胞中mTOR的异常活化通过抑制早期红系祖细胞线粒体自噬和成熟红细胞中线粒体的清除导致mtDNA突变小鼠发生贫血。我们的结果显示相对高危组MDS患者有核红细胞LC3B表达较相对低危组及对照组明显减低,提示相对高危组患者自噬水平明显减低,与PCR结果相一致。荷兰的一项研究提示自噬可能是清除缺陷的铁过载线粒体的保护机制,自噬能抑制MDSAML转化[14]。MDS较常发生于老年人可能与随着年龄的增长自噬水平降低有关。 自噬能够通过线粒体蛋白的降解联合自噬标志物如LC3Ⅱ的出现及P62蛋白的降解来检测。通过检测线粒体蛋白表达量的变化可反映末期线粒体总量的改变,从而间接反映自噬的活性。TOM20蛋白是线粒体的外膜蛋白,我们分析了MDS及对照组的有核红细胞TOM20的表达。结果显示相对高危组高于对照组及相对低危组。 我们的结果显示相对高危MDS患者贫血严重度与与LC3B水平呈负相关,提示有核红细胞自噬可能参与高危MDS贫血发病机制。一项临床试验显示雷帕霉素对部分老年MDS患者有效,通过抑制线粒体功能紊乱与mTOR活化的恶性循环,活化红系祖细胞自噬水平,减轻了MDS患者的症状[15]。另外,目前治疗高危MDS患者的药物阿扎胞苷,除了减少甲基化外,也可以增加细胞的自噬与凋亡[16],均提示诱导自噬可能治疗MDS 总之,MDS患者有核红细胞自噬水平随危险度的升高而降低,自噬水平降低可能与MDSAML转化有关。高危MDS自噬水平与贫血严重度呈负相关,诱导自噬的药物(mTOR抑制剂雷帕霉素等)可能缓解MDS患者的贫血症状,抑制MDSAML的转化。
