aThe review was conducted during the week of February 9, 2015. To calculate the time to extraction and publication, we reviewed the annual reports to determine the following dates: the period of data described (including end date, if available), the earliest listed data extraction date following the period of data described as an indicator of when work was initiated on report preparation, and the date of report release. Unless otherwise specified, we used December 31 as the exact date to represent the end of the period described for calculations. For the data extraction and report release dates, if only the month and year were available, we used the 15th of the month in calculations.
