Abstract
A therapeutic education programme comprising individual sessions in the patient's home has been tried out with patients with diabetes. It enables them to gain a better understanding of the disease and become players in their own health care. This initiative strengthens the doctor-nurse partnership within a coordinated care pathway.Entities:
