Abstract
OBJECTIVE: Glycated haemoglobin (HbA1c) is recommended as an additional tool to glucose-based measures (fasting plasma glucose [FPG] and 2-hour plasma glucose [2PG] during oral glucose tolerance test [OGTT]) for the diagnosis of diabetes; however, its use in sub-Saharan African populations is not established. We assessed prevalence estimates and the diagnosis and detection of diabetes based on OGTT, FPG, and HbA1c in an urban black South African population. RESEARCH DESIGN AND METHODS: We conducted a population-based cross-sectional survey using multistage cluster sampling of adults aged ≥18 years in Durban (eThekwini municipality), KwaZulu-Natal. All participants had a 75-g OGTT and HbA1c measurements. Receiver operating characteristic (ROC) analysis was used to assess the overall diagnostic accuracy of HbA1c, using OGTT as the reference, and to determine optimal HbA1c cut-offs.Entities:

Data are percentage (95% CI). OGTT = oral glucose tolerance test. FPG = fasting plasma glucose. 2h PG = 2 hour post-load plasma glucose. IFG = impaired fasting glucose. IGT = impaired glucose tolerance. OGTT and FPG categories of glycaemia were defined according to the World Health Organization (1998) definitions, HbA1c according to the World Health Organization (2011) definition. Proportions include those classified with IFG, IGT or diabetes by OGTT, FPG, 2h PG-alone or HbA1c and those with a previous history of diabetes. Age-standardised = direct standardisation (WHO world standard population).

RR = Risk Ratio. 95% CI = 95% Confidence Interval. OGTT = oral glucose tolerance test. FPG = fasting plasma glucose. BMI = body mass index. WC = waist circumference.
aCrude = univariable poisson regression between risk factor and diabetes.
bPartially Adjusted = multivariable poisson regression adjusted for age, sex and clustering at the planning unit cluster and household level.
cFully adjusted = multivariable poisson regression adjusted for age, sex, waist circumference or BMI, blood pressure, haemoglobin, lipids, HIV infection, family history of diabetes, smoking status, alcohol use, physical activity and clustering at the planning unit cluster and household level.
Fig 1Receiver Operating Curve (ROC) curves for HbA1c for the detection of diabetes with OGTT (a) and FPG (b) as the reference.
Area under the ROC curve: 0.94 (95% CI 0.88–0.99) with oral glucose tolerance test (OGTT) as the reference (a) and 0.95 (95% CI 0.88–1.00) with fasting plasma glucose (FPG) as the reference (b). Optimal HbA1c cut off: 42 mmol/mol (6.0%) with OGTT as the reference (sensitivity 89.2%, specificity 92.0%) (a), and 42 mmol/mol (6.0%) with FPG as the reference (sensitivity 96.3%, specificity 91.4%) (b).
