Abstract
OBJECTIVES: Extracranial carotid artery true aneurysm is extremely uncommon, and definite treatment has traditionally involved open surgical repair. Although successful management of the internal carotid artery aneurysms using endovascular stenting and embolization has recently been reported, only a limited number of cases have been reported.Entities:

Figure 1.(a) Computerized tomography angiogram (CTA) reveals a large left carotid artery aneurysm with ulcerated intramural thrombosis (arrows), and (b) CTA with 3D reconstruction shows the aneurysm with irregular thrombus involving the left distal common and proximal internal carotid arteries.
Figure 2.(a) Intraoperative angiogram confirms the left carotid aneurysm, and (b) coil embolization of the left external carotid artery (arrow) is performed using Interlock coils (Boston Scientific Corp., Marlborough, MA) to prevent type II endoleak.
Figure 3.(a) Postoperative computerized tomography angiogram (CTA) shows that the Viabahn (W. L. Gore & Associates Inc., Newark, DE) covered stent is widely patent, with complete thrombosis of the aneurysmal sac, and (b) CTA with 3D reconstruction demonstrates that the flexible covered stent is smoothly following the natural course of the carotid artery without kink or stenosis.
