Sir, I appreciate your consideration to the article, “Bilateral lateral rectus myotoxicity after retrobulbar anesthesia.”[1] It is hard to confirm the myotoxicity to lateral rectus muscle in this case because the myotoxicity problem should be ruled out the other causes including thyroid orbitopathy and other infiltrative myopathies. However, we presumed that this case might be due to the myotoxicity based on the time of onset, medical history, and histologic findings. Unfortunately, we could not find any information about the restriction between two cataract surgeries because the patient had visited the clinic at 1 month after the second surgery. As you comment, the inhibitors of macrophage might be one of the treatments for myotoxicity in the future. Sincerely yours,
