Treatment of Bartholin gland cyst with CO2laser.

OBJECTIVE: To describe the results of treatment with CO2 laser for Bartholin gland cysts.
METHODS: Thirty-one women with Bartholin gland cysts were treated with CO2laser at an outpatient´s setting. Skin incision was performed with focused laser beam, the capsule was opened to drain mucoid content, followed by internal vaporization of impaired capsule.
RESULTS: There were no complications. Five patients had recurrence of the cyst and were submitted to a second and successful session.
CONCLUSION: CO2 laser surgery was effective to treat Bartholin gland cysts with minimal or no complications, and can be performed at an outpatient´s setting.

INTRODUCTION

Bartholin glands, or greater vestibular glands, are located one on each side of the labia minor, externally to the vaginal opening. During sexual stimulation, these glands release lubricant fluid.[1]

The first person to describe this gland was Kaspar Bartholin, in 1977. This Dane became known for describing it along with the accessory ducts of the salivary and sublingual glands.[1-3]

The Bartholin gland cyst consists of a tumor-like lesion of the vulva, which develops along the trajectory of the gland duct; despite its obstruction, the gland continues to secrete mucus. This situation accounts for 2% of gynecological appointments per year of women at child-bearing age.[2]

A study performed in Korea in 2009 reported that the incidence of cysts and abscesses of Bartholin gland occurred in women aged 15-50 years, with a greater incidence up to 40 years, decreasing from then on.[4]

In Brazil, in an article published in 2012, the mean age of 127 women with Bartholin gland cysts was 37.3 years, ranging from 18 to 61 years. Approximately 70% were multiparous, and the most frequent complaint was pain.[5]

Although benign, the lesion is associated with significant discomfort for patients. The most commonly described symptom is local pain, which may worsen when walking or sitting. If the cyst evolves to an abscess, the symptoms may be more severe and incapacitating, and even be accompanied by nausea, vomiting, and fever, similar to an infectious condition.[6]

The obstruction of the Bartholin gland originates retention of secretions with duct dilation and cyst formation. Oftentimes, women remain asymptomatic. However, if the cyst is infected and an abscess develops, the clinical picture becomes symptomatic. Diagnosis is made by means of the physical examination, observing a fluctuating mass in one of the small labia.[3]

Bartholin gland abscesses are common in single women and those of lower social and economic conditions.[1] Some studies showed that the secretion originated in these abscesses presented with agents such as Neisseria gonorrhoeae and Chlamydia trachomatis, but in most cases, the causative organisms are from the mixed vaginal flora, bacteroides, Escherichia coli, and Staphylococcus aureus.[1,3]

The differential diagnosis of the Bartholin gland cysts and abscesses is made with other vulvar masses, such as epidermal inclusion cysts, and cysts of Nuck and Skene duct. Carcinoma is rare. A study performed in Warsaw, Poland, from 1980 to 2009, recorded 1,296 patients with vulvar carcinoma treated at an oncology center. Nine of them had carcinoma of the Bartholin gland, including three patients with squamous cell carcinoma, three with cystic adenoid carcinoma, and three with sarcoma.[7]

Treatment used for cysts and abscesses of the Bartholin gland is still very controversial. There are many options, including antibiotics, drainage, marsupialization, gland excision, and destruction or cauterization of the cyst with silver nitrate and CO2 laser.[6]

Therapeutic options

The treatment of Bartholin cyst may be conservative or surgical, depending on the patient’s symptoms, size of the cyst, and whether it is infected or not. Asymptomatic patients require no treatment. If the gland becomes infected, treatment with broad-spectrum antibiotics and analgesics is necessary.[3,8]

The gold standard treatment is surgical removal of the entire cyst. However, this treatment is not performed very often since it affects the physiological vaginal lubrication and is associated with other complications. Less invasive therapeutic strategies have been proposed.[6,8]

Marsupialization

This was initially described by Jacobson, in 1950. It is performed with a small 1.5-to-3cm incision over the abscess to minimize scars and allow drainage of the gland’s secretion. After drainage, the capsule of the cyst is sutured with the borders fixed to the exterior, in order to avoid closing and formation a new cyst; over time, the process undergoes reepithelization. The cavity should be irrigated with saline solution and an antibiotic. This procedure is normally done under anesthesia - either general, local, or pudendal nerve block.[1,3]

In a randomized prospective study, 83 women submitted to marsupialization were followed up, 24.1% presented with recurrence, and the most frequent postoperative symptoms were discharge at the surgical site and labial edema.[9]Studies on marsupialization reported a global recurrence rate varying from 2 to 25%.[10]

It is important to point out that if the cyst develops in a postmenopausal woman, a biopsy should be done to evaluate possible malignity.[11]

Fistulization – Word catheter and Jacobi ring

Placement of the Word catheter is a technique first described by Buford Word, in 1964, and has the advantages of being a simple method, it is possible to perform the procedure at an outpatient setting, and patients recover promptly. The disadvantage is no availability of this catheter at hospitals. The success of this method is based on the ancient principle that a foreign body in a wound hinders its natural closure, resulting in the formation of a fistula with wound reepithelization.[3,12]

Word catheters are placed by means of a 5mm incision inside the small labia, in the region of the Bartholin gland. At the extremity of the catheter, there is a sac that is inflated with up to 3mL of sterile saline solution, and the catheter is left in place for 4 to 6 weeks.[6]

A study published in 2008 developed a technique similar to that of the Word catheter. It proposed the placement of a device similar to this catheter that allowed drainage and reepithelization, and was removed after three weeks with total recovery of the patient.[12]

The Jacobi ring is a rubber catheter that can be inserted through the cyst or abscess and secured by suture. This device also allows drainage and reepithelization.[13]

In a randomized study comparing the Word catheter and the Jacobi ring, there were no differences in recurrence rates in both techniques, but the authors showed greater patient satisfaction with the use of the ring.[14]

Recurrence of Bartholin cyst was observed in 4 to 17% of 111 women after use of the catheter during a six-month follow-up. Premature loss of the catheter was the most common adverse event.[14]

Ablation with silver nitrate

Application of silver nitrate to treat Bartholin gland cyst or abscess is considered a simple and effective technique, which can be performed in an outpatient setting. After local cleaning and anesthetic infiltration, a 0.5-to-1.0cm long incision is made on the mucosa over the cyst, the content is drained and silver nitrate is placed on the cavity, with no sutures. The solution is removed 48 hours later.[3]

In a study done with a group of 76 patients submitted to this treatment, the recurrence rate was 26.3%. The most frequent postoperative complaints were a burning sensation at the surgical site, hematoma, and dyspareunia.[9]

Sclerotherapy with alcohol

This is a procedure carried out in a short amount of time and with a rapid rate of cure. After incision of the skin and drainage of the cyst emptying the cavity, the site is irrigated with 70% alcohol for approximately 5 minutes. Recurrence was observed in 8 to 10% of cases, and the most common postoperative sign was transient hyperemia.[15]

Bartholinectomy

Exeresis of Bartholin gland is a surgical treatment that requires greater operative time. It is an elective procedure, performed in the absence of infection, and it is normally used after failure of other techniques.[1,6]

CO2 laser

CO2 laser can be use both to vaporize and to remove the Bartholin gland. This surgical procedure is simple and quick, but expensive. It may be performed at an outpatient setting, with minimal discomfort for the patient in the intra- and postoperative periods.[8]

A study reported a series of 19 patients with Bartholin cysts treated with the minimally invasive approach of CO2 laser, and demonstrated the surgical procedure as being extremely simple, and performed in a very short time, on average, seven minutes. Patients’ satisfaction was high in the short- and long-run.[16]

The objective of the study done by Panici et al. was to describe the conservative surgical technique with CO2 laser, evaluate its viability, complication rate, and the results obtained. It demonstrated that laser treatment can be safely used at the outpatient setting, patients complained very little of intraoperative pain, and there are high rates of cure in the long-term.[16]

The analysis of 200 patients with Bartholin cysts submitted to CO2 laser treatment showed a mean age of 32 years, with one delivery, and 87% of patients received some antibiotic therapy. As to the follow-up data after the procedure, the rate of cure with a single laser application was 95.7%, and repeating laser application was effective in cases of recurrence during follow-up.[17]

The study published by Speck et al. evaluated the use of laser in 22 women. The patients were oriented to return two and four weeks after the procedure for follow-up. At the first visit, all presented with mucoid discharge, and complete recovery occurred within a period of 3 to 4 weeks. Only two patients presented with recurrences, requiring a new application.[18]

This form of treatment seems to be a good alternative, less invasive, quick, and safe for cases of Bartholin cysts. The recurrence rates, on average, were less than 10% and cure after a new laser procedure is complete.[16,18]

OBJECTIVE

To describe the results of treatment with CO2 laser for Bartholin gland cyst.

METHODS

During the period from January 2007 to December 2014, 31 patients diagnosed with Bartholin gland cysts were seen at the Unit for Nucleus for Prevention of Gynecological Disease - Department of Gynecology, Escola Paulista de Medicina. This study was approved under CAAE: 45073715.3.0000.5505. A retrospective evaluation was made of the medical records of 31 patients, and the data for description of the technique, along with the results, were grouped.

The mean age of the group was 34.9 years, ranging from 22 to 48 years.

Four patients had already been treated; three of them used antibiotic treatment and one of them underwent bartholinectomy.

Surgical procedure

The procedure consisted of antisepsis with a solution of topical povidone-iodine and local anesthesia with 2% xylocaine with a vasoconstrictor. With the CO2 laser at the potency of 10 to 25W, used continuously coupled to the colposcope, a longitudinal incision was made with the laser beam using a focused beam to open the capsule of the cyst. The lateral borders of the incision were maintained under tension with grippers for exposure of the cavity, with subsequent drainage of the content and cleaning of the interior with sterile saline solution. Destruction of the capsule tissue was performed with vaporization, using the unfocused laser beam (Figure 1). Patients were instructed to do sitz-baths with a solution of povidone-iodine diluted in water, three times a day, and to abstain from sexual activity for 2 to 3 weeks. Antibiotics and analgesics were prescribed for patients presenting any sign of infection.

RESULTS

The procedure was performed with local anesthesia at an outpatient clinic. All patients reported minimal discomfort, inherent to outpatient’s procedures, with pain with the anesthetic infiltration and sensation of local heat with the application of the laser beam.

The patients were evaluated 15 and 30 days after the laser treatment. In the first two weeks, there was continuous drainage of mucus, and at the end of the first month, there was complete healing of the surgical wound.

Five patients (16%) experienced Bartholin cysts recurrence until 6 months after the first laser session and were submitted to a second session. After the second phase of treatment, there were no recurrences.

In the 31 cases analyzed, after the healing period, there were no reports of pain at the incision site, and there were no scars and retractions.

DISCUSSION

In Italy, this technique has been used by several authors, due to the ease of access to laser technology.[8,16,17] In Brazil, we note very little experience of laser use for marsupialization of the Bartholin cyst due to cost of the equipment.[5] Our organization, focused primarily on teaching in the medical field, is considered a reference in care of lower genital tract diseases. Experience with the said technique was due to the fact of having the laser equipment, and the great number of patients with the said disease. In a prior analysis of our cases, in 2007, we had already noted positive responses to this surgical method.[18]

The recurrence rate in our survey was similar to that of literature, according to the various techniques initially described. Treating the Bartholin cyst with CO2 laser proved effective for resolution of the disease and of post–treatment comorbidities, such as pain and scars in the affected area.[18] It is a treatment that associates marsupialization with the destruction of the diseased capsule by means of the laser bean, providing good results, albeit no different from those that use only surgery or cauterization.[1,3,6,16]

The procedure was performed at an outpatient clinic, with a minimal amount of bleeding during the act, besides good and rapid healing. Surgical methods with a cold scalpel are not exempt from causing greater bleeding, and those destructive with cauterization by means of alcohol or silver nitrate can be more painful in the postoperative phase. These are factors that have influenced our preference for the technique presented.[1,3,16]

The CO2 laser proved to be an effective method with low morbidity. We noted, with this technique, that healing was rapid, with minimum fibrosis and very little pain, a complication seen with the conventional techniques described by other authors.[3,4,5]

These women are generally young, sexually active and we should be concerned with the frequent risk of sequelae, such as dyspareunia due to fibrosis of the gland, especially with the destructive cauterization approaches.

Additionally, this technique allows new sessions in case of a recurrence.

Since this is an outpatient method, the patient does not need to be away from her activities for very long, there is no need to be admitted to hospital, and there is a reduction in hospital costs. On the other hand, the equipment is very expensive and requires a qualified professional to handle it.

Since there is no hospitalization, the patient is discharged home after the procedure, and soon returns to her daily activities.

CONCLUSION

The Bartholin cyst approach with CO2 laser, according to the technique described of opening of the cyst, drainage of the content, and vaporization of the capsule, can be used as an effective conservative treatment with low rates of recurrence, complications, and great patient satisfaction. It does not require hospital admission, which means saving money and time. The technique to be used depends on the skills of the physician and availability of equipment.

INTRODUÇÃO

As glândulas de Bartholin, ou glândulas vestibulares maiores, estão localizadas uma de cada lado interno dos lábios menores, externamente à abertura vaginal. Durante a estimulação sexual, essas glândulas liberam fluido lubrificante.[1]

O primeiro a descrever a glândula foi Kaspar Bartholin, em 1977. Esse dinamarquês ficou conhecido por descrevê-las, bem como os ductos acessórios das glândulas salivares e sublinguais.[1-3]

O cisto de glândula de Bartholin consiste em tumoração da vulva, que se desenvolve no trajeto do ducto glandular por sua obstrução, e a mesma continua a secretar muco. Essa situação é responsável por 2% das consultas ginecológicas ao ano em mulheres na idade reprodutiva.[2]

Estudo realizado na Coreia em 2009 relatou que a incidência de cistos e abcessos da glândula de Bartholin ocorria em mulheres entre 15 e 50 anos, com maior incidência até os 40 anos e decréscimo a partir dessa idade.[4]

No Brasil, em publicação de 2012, a média etária descrita das portadoras de cisto de Bartholin foi de 37,3 anos, variando de 18 a 61 anos, no total de 127 mulheres. Aproximadamente 70% eram multíparas, e a queixa mais frequente foi a dor.[5]

Embora benigna, a lesão está associada a desconforto significante das pacientes. O sintoma mais descrito é dor local, que pode piorar ao deambular ou sentar. Se o cisto evoluir para abcesso, os sintomas podem ser mais severos e incapacitantes, e estarem acompanhados de náuseas, vômitos e febre, semelhantemente a um quadro infeccioso.[6]

A obstrução da glândula de Bartholin dá origem à retenção de secreções com dilatação do ducto e formação do cisto. A mulher, por muitas vezes, pode ficar assintomática com sua presença. Porém, se o cisto for infectado e desenvolver abscesso, o quadro torna-se bastante sintomático. O diagnóstico é realizado por meio do exame físico, observando-se massa flutuante em um dos pequenos lábios.[3]

Abcessos da glândula de Bartholin são mais comuns em mulheres solteiras e de origem socioeconômica mais baixa.[1] Alguns estudos mostraram que a secreção originada desses abscessos apresentava agentes como a Neisseria gonorrhoeae e Chlamydia trachomatis, porém, na maioria dos casos, os organismos causadores são da flora vaginal mista, bacteroides, Escherichia coli e Staphylococcus aureus.[1,3]

O diagnóstico diferencial dos cistos e abscessos da glândula de Bartholin é feito com outras massas vulvares, como cistos de inclusão epidérmica, cistos de canal de Nuck e Skene. O carcinoma é raro. Estudo realizado em Varsóvia, na Polónia, nos anos de 1980 a 2009, registrou 1.296 pacientes com carcinoma vulvar que foram tratados em centro de oncologia. Nove delas tiveram carcinoma da glândula de Bartholin, incluindo três pacientes com carcinoma espinocelular, três com carcinoma adenoide cístico e três com sarcoma.[7]

O tratamento usado para cistos e abscessos da glândula de Bartholin ainda é muito controverso. Há muitas opções para tratamento, incluindo antibióticos, drenagem, marsupialização, excisão da glândula, e destruição ou cauterização do cisto com nitrato de prata e laser de CO2.[6]

Opções terapêuticas

O tratamento do cisto de Bartholin pode ser conservador ou cirúrgico, dependendo dos sintomas da paciente, do tamanho do cisto, e se está infectado ou não. Pacientes assintomáticas não necessitam de qualquer tratamento. Se a glândula torna-se infectada, o tratamento com antibióticos de amplo espectro e analgésicos faz-se necessário.[3,8]

O padrão-ouro de tratamento é a remoção cirúrgica de todo o cisto. No entanto, este tratamento muitas vezes não é realizado porque afeta a lubrificação vaginal fisiológica e está associado a outras complicações. Estratégias terapêuticas menos invasivas têm sido propostas.[6,8]

Marsupialização

Foi inicialmente descrita por Jacobson, no ano de 1950. Realizada com uma pequena incisão de 1,5 a 3cm sobre o abscesso, para minimizar cicatrizes e permitir a drenagem de secreções da glândula. Após a drenagem, faz-se sutura da cápsula do cisto com as bordas fixadas em seu exterior, para evitar o fechamento e a formação de novo cisto; com o passar do tempo, o processo sofre reepitelização. A cavidade deve ser irrigada com solução salina e antibiótico. Esse procedimento normalmente é realizado sob anestesia, que pode ser geral, local ou com bloqueio do nervo pudendo.[1,3]

Em estudo prospectivo randomizado, foram acompanhadas 83 mulheres submetidas à marsupialização, 24,1% apresentaram recidiva, e os sintomas pós-operatórios mais frequentes foram saída de secreção no sítio cirúrgico e edema labial.[9]

Estudos sobre marsupialização relataram taxa de recidiva global variando de 2 a 25%.[10]

É importante salientar que, se o cisto se desenvolver em mulher na pós-menopausa, a biópsia deve ser realizada, para ser avaliada possível malignidade.[11]

Fistulização - cateter de Word e anel de Jacobi

A colocação de cateter de Word é técnica descrita pela primeira vez em 1964 por Buford Word e tem como vantagens a simplicidade do método, a possibilidade de colocação em meio ambulatorial e a fácil recuperação da paciente. A desvantagem é a falta desse cateter nas instituições hospitalares. O sucesso deste dispositivo é baseado no princípio antigo de que corpo estranho em uma ferida impede seu fechamento natural, resultando na formação de fístula com reepitelização.[3,12]

Os cateteres de Word são colocados por meio de incisão de 5mm no interior dos pequenos lábios, na região da glândula de Bartholin. Na extremidade do cateter, há uma bolsa que é insuflada com até 3mL de solução salina estéril, e o cateter é deixado no lugar por 4 a 6 semanas.[6]

Estudo publicado em 2008 desenvolveu técnica semelhante ao cateter de Word. Nele, foi proposta a colocação de dispositivo semelhante ao cateter, que permitia a drenagem e a reepitelização, sendo retirado após 3 semanas com total recuperação da paciente.[12]

O anel de Jacobi é um cateter de borracha que pode ser inserido por meio do cisto ou abscesso e fixado por sutura. Esse dispositivo também permite drenagem e reepitelização.[13]

Em estudo randomizado comparando o cateter de Word e o anel de Jacobi, não houve diferença nas taxas de recidiva nas duas técnicas, mas os autores mostraram maior satisfação das pacientes com o uso do anel.[14]

A recidiva do cisto de Bartholin foi observada em estudo com 111 mulheres, em 4 a 17% das pacientes após o uso do cateter no acompanhamento de 6 meses. A perda prematura do cateter foi o evento adverso mais comum.[14]

Ablação com nitrato de prata

A aplicação de nitrato de prata para tratamento de cisto ou abscesso da glândula de Bartholin é considerada uma técnica simples, eficaz e que pode ser realizada em ambiente ambulatorial. Após a limpeza local e a infiltração de anestésico, é realizada incisão na mucosa sobre o cisto de 0,5 a 1,0cm de comprimento, drenagem do conteúdo e colocação de nitrato de prata na cavidade sem realização de suturas. A remoção da solução ocorre após 48 horas.[3]

Em estudo com um grupo de 76 pacientes submetidas a este tratamento, a taxa de recidiva foi de 26,3%. As queixas mais frequentes no pós-operatório foram queimação no sítio cirúrgico, hematoma e dispaurenia.[9]

Escleroterapia com álcool

Trata-se de procedimento realizado em pouco tempo e com cura rápida. Após incisão de pele e drenagem do cisto com esvaziamento da cavidade, o local é irrigado com álcool 70% por aproximadamente 5 minutos. A recidiva foi observada em 8 a 10%, e o sinal mais comum no pós-operatório foi hiperemia transitória.[15]

Bartolinectomia

A exerése da glândula de Bartholin é um tratamento cirúrgico que requer maior tempo operatório. Consiste em procedimento eletivo, realizado na ausência de infeção, normalmente utilizado após falha das outras técnicas.[1,6]

Laser de CO2

O laser de CO2 pode ser utilizado tanto para vaporizar como para extirpar o cisto da glândula de Bartholin. Esse procedimento cirúrgico é simples e rápido, porém dispendioso. Pode ser realizado em ambiente ambulatorial, com mínimo desconforto da paciente no intra e pós-operatório.[8]

Estudo que relatou série de 19 pacientes com cisto de Bartholin tratadas com abordagem minimamente invasiva de laser CO2 demonstrou o procedimento cirúrgico como extremamente simples, sendo realizado em curtíssimo período de tempo, com média de 7 minutos. A satisfação das pacientes foi alta, a curto e longo prazo.[16]

O objetivo do estudo de Panici et al. foi descrever técnica cirúrgica alternativa conservadora com laser de CO2, avaliar a viabilidade, a taxa de complicação e os resultados obtidos. Demonstraram que tratamento com laser pode ser realizado com segurança em ambiente ambulatorial, pouca dor intraoperatória e altas taxas de cura a longo prazo.[16]

A análise de 200 pacientes com cisto de Bartholin submetidas ao tratamento com laser de CO2 mostrou que o perfil delas era média de idade de 32 anos, com um parto e 87% das pacientes receberam algum tipo de tratamento com antibióticos. Quanto aos dados de seguimento após o procedimento, a cura com uma única aplicação de laser foi de 95,7% e a repetição do laser foi efetiva nas recidiva durante o seguimento.[17]

O estudo publicado por Speck et al. avaliou o emprego do laser em 22 mulheres. As pacientes foram orientadas a retornar na segunda e quarta semana após o procedimento para seguimento. No primeiro controle, todas apresentavam drenagem de secreção mucoide, e a recuperação total ocorreu no período de 3 a 4 semanas. Apenas duas pacientes apresentaram recidiva, necessitando de nova abordagem.[18]

Essa forma de tratamento parece boa alternativa, menos invasiva, rápida e segura para cisto da glândula de Bartholin. As taxas de recidiva em média, não ultrapassam 10% e a cura após novo procedimento de laser é total.[16,18]

OBJETIVO

Descrever os resultados do tratamento com laser de CO2 para o cisto da glândula de Bartholin

MÉTODOS

No período de janeiro de 2007 a dezembro de 2014 foram atendidas 31 pacientes com diagnóstico de cisto da glândula de Bartholin no Núcleo de Prevenção em Doenças Ginecológicas do Departamento de Ginecologia da Escola Paulista de Medicina. Este estudo foi aprovado sob número do CAAE: 45073715.3.0000.5505. Foi realizada avaliação retrospectiva dos prontuários das 31 pacientes, os dados para descrição da técnica, com os seus resultados, foram reunidos.

A média etária do grupo foi de 34,9 anos, variando de 22 a 48 anos.

Quatro pacientes já tinham sido tratadas, sendo que três utilizaram antibioticoterapia e uma, bartolinectomia.

Procedimento cirúrgico

O procedimento consistiu na antissepsia com solução de iodopovidona tópica e em anestesia local com xilocaína a 2% com vasoconstritor. Com o laser de CO2 na potência de 10 a 25W, utilizado de modo contínuo acoplado ao colposcópio, realizou-se incisão longitudinal com o feixe de laser com raio focalizado, de modo a abrir a cápsula do cisto. Os bordos laterais da incisão foram mantidos em tensão com pinças, para exposição da cavidade, com subsequente drenagem do conteúdo e lavagem do interior com solução salina estéril. Realizou-se a destruição do tecido capsular com vaporização, utilizando o feixe de laser desfocalizado (Figura 1). As pacientes foram orientadas a realizar banhos de assento com solução de iodopovidina diluída em água três vezes por dia, e abstinência sexual de 2 a 3 semanas. Antibióticos e analgésicos foram prescritos para pacientes que apresentavam sinais de infecção.

RESULTADOS

O procedimento foi realizado com anestesia local em ambulatório. Todas as pacientes referiram desconforto mínimo, inerentes a procedimentos ambulatoriais, com dor à infiltração do anestésico e sensação de calor local com a aplicação do feixe laser.

As pacientes foram avaliadas 15 e 30 dias após o tratamento com laser. Nas 2 primeiras semanas, houve drenagem contínua de muco e, ao final do primeiro mês, cicatrização completa da ferida operatória.

Cinco pacientes (16%) apresentaram recidiva do cisto de Bartholin até 6 meses após primeira sessão de laser e foram submetidas a uma segunda sessão. Após a segunda fase de tratamento, não houve recidivas.

Nos 31 casos analisados, após período de cicatrização, não foi relatada dor no local da incisão e nem foram observadas cicatrizes e retrações.

DISCUSSÃO

Na Itália, esta técnica tem sido utilizada por vários autores, devido à facilidade de acesso à tecnologia a laser.[8,16,17] No nosso meio, observa-se pouca experiência de uso do laser para marsupialização do cisto de Bartholin, pelo custo do equipamento.[5] Nosso serviço, voltado principalmente ao ensino da área médica, é considerado referência no atendimento de doenças do trato genital inferior. A experiência em tal técnica foi devido ao fato de termos o equipamento a laser, e à grande demanda de pacientes com a afecção em questão. Em análise anterior dos nossos casos, em 2007, já observávamos respostas positivas com esse método cirúrgico.[18]

A taxa de recidiva do nosso levantamento foi semelhante aos dados da literatura de acordo com as várias técnicas inicialmente descritas. O tratamento com laser de CO2 para cisto de Bartholin se mostra eficaz para resolução da doença e das comorbidades pós-tratamento, como dor e cicatrizes na região abordada.[18] É um tratamento que associa a marsupialização com a destruição da cápsula doente pelo feixe de laser, oferecendo bons resultados, porém não diferindo daqueles que utilizam só a cirurgia ou a cauterização.[1,3,6,16]

O procedimento foi realizado ambulatorialmente, com o mínimo de sangramento durante o ato, além de boa e rápida cicatrização. Métodos cirúrgicos a bisturi frio não são isentos de apresentar sangramentos maiores, e os destrutivos com cauterização por meio de álcool ou nitrato de prata podem ser mais dolorosos no pós-operatório. Estes são fatores que têm influenciado nossa preferência pela técnica apresentada.[1,3,16]

O laser de CO2 mostrou ser método efetivo e com pouca morbidade. Observamos, com esta técnica, que a cicatrização foi rápida, com mínima fibrose e pouca dor remanescente, complicação foi observada pelas técnicas convencionais descrita por outros autores.[3,4,5]

Estas mulheres, em geral jovens, são sexualmente ativas e devemos nos preocupar com o frequente risco de sequela, como a dispareunia, consequente à fibrose da glândula, principalmente nas abordagens destrutivas de cauterização.

Além do mais, esta técnica permite novas sessões em caso de recidiva.

Por se tratar de método ambulatorial, a paciente não necessita se ausentar por muito tempo de suas atividades, não requerendo internação, e existe redução dos custos hospitalares. Por outro lado, o equipamento é de alto custo e requer profissional habilitado para manipulá-lo.

Por não existir período de internação hospitalar, a paciente é liberada para casa logo após o procedimento e em pouco tempo retorna às suas atividades diárias.

CONCLUSÃO

A abordagem do cisto de Bartholin com o laser de CO2, de acordo com a técnica descrita de abertura do cisto, drenagem do conteúdo e vaporização da cápsula, pode ser usada como tratamento conservador eficaz com baixas taxas de recidiva, complicações e grande satisfação das pacientes. Não requer internação hospitalar que representa uma forma de economia financeira e temporal. A técnica a ser empregada depende da habilidade do médico e da disponibildade do equipamento.
