Abstract
INTRODUCTION: Rectal faecal impaction (RFI) from functional constipation is a common problem in children. Maintenance therapy should start after successful disimpaction. However, there is uncertainty with regard to the optimal disimpaction regimen. AIM: We systematically evaluated the effect of polyethylene glycol (PEG) compared to enema for treating RFI.Entities:

Characteristics of included studies
| Study | Randomisation | Allocation | Blinding | Follow up (end of intervention) | Intention to treat analysis | Participants | Rectal faecal impaction (definition) | Intervention (oral PEG) | Comparison (enema) | Treatment success/failure (definition) |
|---|---|---|---|---|---|---|---|---|---|---|
| Bekkali 2009 | Unclear | Unclear | No | 80/90 (85%) | No (available case analysis) | Age 4–16 years ( | Large amount of hard stool in the rectum | 3350 plus electrolytes (1.5 g/kg/day) for 6 days | Dioctyl sulfosuccinate sodium (60 ml < 6 years, 120 ml ≥ 6 years), daily for 6 days | Success: absence of faecaloma on rectal examination or at the abdominal radiography |
| Miller 2012 | Yes | Yes | No | 62/80 (77.5%) | No (available case analysis) | Age 1–17 years ( | Lower quadrant mass or dilated rectum with hard stool; functional faecal retention; excessive stool in colon on abdominal radiograph | PEG 3350 (1.5 g/kg/day, max. dose 100 g/day) for 3 days | Milk and molasses mixed 1: 1 (10 ml/kg max. 500 ml), single dose | Failure: a participant who received an enema at home, returned to the emergency department for evaluation, or was later admitted to an inpatient service for treatment of faecal impaction |
