BACKGROUND: Potentially harmful unplanned extubation (UE) may occur in patients on mechanical ventilation (MV) in an intensive care unit (ICU) setting. *Two patients were not reintubated at the request of the families. Accordingly, we have excluded these patients from comparisons of mortality. Nine unplanned extubation patients died in this study.
