Likelihood ratio tests of comparison with adjusted model in Table 2 (i.e. test for effect modification). Two further variables tested for effect modification, but with P > 0.01 (adjusted models): benzodiazepine use P = 0.03; age P = 0.87. All interactions were fitted individually in adjusted model. As a sensitivity check, all interactions were included in a single, adjusted model to test their independence: the results were very similar to those reported here.
