B: bronchial hyperresponsiveness
C: peak expiratory flow (PEF) < 250 L/min/kg
D: marginal pulmonary function (1.0 L < forced expiratory volume in one second [FEV1] < 1.2 L and 40% < FEV1% < 60%).
