Incidence and risk factors for intimate partner violence during the postpartum period.

OBJECTIVE To estimate the incidence and identify risk factors for intimate partner violence during postpartum. METHODS This prospective cohort study was conducted with women, aged between 18-49 years, enrolled in the Brazilian Family Health Strategy in Recife, Northeastern Brazil, between 2005 and 2006. Of the 1.057 women interviewed during pregnancy and postpartum, 539 women, who did not report violence before or during pregnancy, were evaluated. A theoretical-conceptual framework was built with three levels of factors hierarchically ordered: women's and partners' sociodemografic and behavioral characteristics, and relationship dynamics. Incidence and risk factors of intimate partner violence were estimated by Poisson Regression. RESULTS The incidence of violence during postpartum was 9.3% (95%CI 7.0;12.0). Isolated psychological violence was the most common (4.3%; 95%CI 2.8;6.4). The overlapping of psychological with physical violence occurred at 3.3% (95%CI 2.0;5.3) and with physical and/or sexual in almost 2.0% (95%CI 0.8;3.0) of cases. The risk of partner violence during postpartum was increased for women with a low level of education (RR = 2.6; 95%CI 1.3;5.4), without own income (RR = 1.7; 95%CI 1.0;2.9) and those who perpetrated physical violence against their partner without being assaulted first (RR = 2.0; 95%CI 1.2;3.4), had a very controlling partner (RR = 2.5; 95%CI 1.1;5.8), and had frequent fights with their partner (RR = 1.7; 95%CI 1.0;2.9). CONCLUSIONS The high incidence of intimate partner violence during postpartum and its association with aspects of the relationship's quality between the couple, demonstrated the need for public policies that promote conflict mediation and enable forms of empowerment for women to address the cycle of violence.

INTRODUCTION

Many studies on violence committed against women have identified the victim’s intimate partner as the most common aggressor. , , These studies have also estimated its magnitude and the factors associated with intimate partner violence (IPV) before and during pregnancy as well as evaluated its impact on the physical and mental health of women , and their children. However, postpartum IPV has been studied by few researchers. , - , , ,

Postpartum IPV prevalence varies from 8.3% in China to 24.2% in Sweden. The incidence of any type of postpartum IPV (psychological, physical and sexual) found in a population-based study by Guo et al (2004), in China, was around 2.5%. Whereas Hedin (2000) observed an incidence of 69.0% in a sample of 132 women attended, eight weeks after childbirth, in three prenatal clinics in Sweden. The incidence of physical violence was found in less than 1.0% of women in the United States in a population-based study, and at 11.0%, in a sample of 175 women receiving care at a prenatal clinic at a university hospital. These differences may have been influenced by the different methodologies, instruments, sample compositions and the postpartum period in which the interview was conducted, which compromised the comparability, especially with respect to the types of violence studied.

Intimate partner violence against women of reproductive age constitutes a public health problem and has attracted interest from scientific and political fields. Nevertheless, no articles on the incidence and risk factors for postpartum IPV in Brazil were found up to the conclusion of this study.

The risk factors for IPV during pregnancy and postpartum are similar to those found in other periods of life: younger women, , with no partner, lower education and precarious socioeconomic conditions, use of alcohol and illicit drugs by the woman and her partner, partner’s aggressive behavior outside the home, , length of the relationship, difficult communication with the partner, fights between the couple, partner suspicion of infidelity, physical violence by the woman against her partner without being assaulted first and controlling behavior by a partner. , Another IPV risk factor is the experience of woman or partner, in childhood or adolescence, with IPV against their mother or personal experience (direct) of violence perpetrated by a relative during childhood or adolescence, indicating another serious problem – the transgenerational transmission of violence. ,

The aim of this study was to evaluate the incidence of intimate partner violence during postpartum and identify its risk factors.

METHODS

This cohort study was performed at the Distrito S anitário II (one of the six health areas) in a poor region of the city of Recife, PE, in the Northeastern region of Brazil.

All pregnant women (n = 1,133), aged from 18 to 49 years old, who were enrolled in the Brazilian Family Health Strategy of the district were considered eligible. Twelve of these women did not complete the questionnaire: five were homeless, three moved away from the study area and four could not be found by the interviewers despite several visits. Once the losses were accounted for, 1,121 (98.9%) pregnant women were interviewed. 1,057 of these women were interviewed again during postpartum. Sixty-four women dropped out between pregnancy and postpartum, two of these for being unable to attend the second interview, three deaths, 37 who changed their address, four who ended up living on the street, 13 who moved to areas controlled by drug dealers and five who refused to participate. The 64 women who were not interviewed during postpartum had lower education (p = 0.001), but no statistically significant difference was observed regarding the frequency of violence during pregnancy, or with any other socioeconomic and demographic variables.

The first contact with the pregnant women was made during their prenatal consultation. The pregnant women were identified from the prenatal care records of 42 Family Health Strategy teams. Records from community health agents were also evaluated so as to include women not receiving antenatal care at the family health strategy units. The preferred option was for the prenatal interview to take place at the health unit, but some women were interviewed at home, when requested.

The women, during their postpartum period, were contacted during childcare consultations or at home, following the same pattern that was established for interviews during pregnancy. Most of the interviews were held at the interviewees’ homes between May and December 2006.

The 518 women who reported IPV before and during pregnancy were excluded to estimate the incidence, leaving 539 women who were reinterviewed up to 12 months after childbirth.

Two questionnaires were applied: one during pregnancy and the other during the postpartum period. Data were collected between July 2005 and December 2006 by female interviewers.

Confidentiality and privacy for those interviewed were guaranteed. Regardless of whether or not the women had experienced IPV, all received specific information on social services, health, legal and law enforcement, which were made available in the study area. Services that provide assistance to female victims of violence were solicited to provide help to any women interviewed who was suffering from domestic violence.

The IPV-related questions were prepared referring to the questionnaire of the World Health Organization Multi-country Study on Women’s Health and Domestic violence against women, which has already been validated in Brazil.

The intimate partner was defined as a partner or former partner whom the woman is living or used to live with, regardless of any formal union and whom the woman was having or had had sexual relations with.

Questions to identify physical violence were characterized as physical aggression or use of weapons or objects to cause injury; psychological violence as threatening behavior, humiliation or insult; and sexual violence as sexual relations imposed by physical force or threats and any imposed acts that were considered humiliating. Women who answered “Yes” to at least one of the questions that referred to each type of violence were considered a positive case. The time of every report of violence was explored about its occurrence before and during pregnancy and postpartum. Additional information about the study methods are reported in other publications. ,

The analysis was guided by a theoretical and conceptual model (Figure 1), which describes the hierarchical relationships between postpartum IPV risk factors, divided into three levels of determinants (proximal, intermediate and distal).

Level 1 (distal) included sociodemographic characteristics of the woman and her partner: age (< 20 years; ≥ 20 years), race/skin color (white; non-white), years in education (< 9 years; ≥ 9 years) and employment status (inactive; active). The following variables were also taken into account for the woman: currently living with partner (yes; no), housing status (owner; not owner) and own income (yes; no).

Level 2 (intermediate) included behavioral variables, with answers (yes; no) about the women: tobacco use, alcohol abuse and illicit drug use; and answers (yes; no) about the partner: alcohol abuse, illicit drug use and aggressive behavior outside the home.

Level 3 (proximal) included the dynamics of the relationship: relationship duration (≤ 1 year; > 1 year), fights between the couple (≥ 1 time/month; < 1 time/month), physical violence committed by the women against her partner without being assaulted first (yes; no), communication with the partner (good; difficult), and controlling behavior by the partner (none; moderate; high). More detailed descriptions of these last two variables can be found in published literature. Analysis was performed using Stata, version 10.1 software. Poisson regression was used to estimate crude and adjusted relative risk (RR), and the 95% confidence interval of the association between postpartum IPV and the explanatory variables.

The analysis strategy for the hierarchical model of determination, following the distal-proximal order, which made it possible to evaluate whether the effect of risk factors on postpartum IPV, was direct or mediated by other factors. , Model 1 included variables from levels 1, 2 and 3, which were adjusted by the variables at the same level. In model 2, variables from level 2 were adjusted for the variables from the same level and for those that remained in model 1. In model 3, variables from level 3 were adjusted for those that remained in models 1 and 2.

The statistically significant variables (p ≤ 0.05) in model 1 were kept in the subsequent models. The previously tested block, which lost its significance when included in a new block, remained in the model.

When the three hierarchical levels are considered, the model presents: a) estimates for the effects of sociodemographic characteristics of the woman and her partner on postpartum IPV, not mediated by variables from the intermediate and proximal levels (dashed and dotted arrows in Figure 1); b) estimates for the effects of sociodemographic characteristics of the woman and her partner on the relationship dynamics, not mediated by the intermediate level (dashed arrow); c) estimates for the effects of the behavior of the woman and her partner on postpartum IPV, adjusted for variables from the distal level and not mediated by variables from the proximal level (dotted arrow); and d) estimates for the effects of the variables of the relationship dynamics on the postpartum IPV, adjusted for variables from the distal and intermediate levels.

This research was approved by the Ethics Committee at the Federal Universidade Federal de Pernambuco (Protocol 303/2004). All participants signed an informed consent term.

RESULTS

The incidence of some kind of postpartum IPV (Figure 2) was 9.3% (95%CI 7.0;12.0), with isolated psychological violence being more common (4.3%; 95%CI 2.8;6.4) than overlapping psychological and physical violence (3.3%; 95%CI 2.0;5.3) and psychological and/or physical and/or sexual violence (1.7%; 95%CI 0.8;3.0). Sexual violence was the least frequent and all the cases occurred in the first 40 days of postpartum.

The risk of postpartum IPV was most common among women without own income, those who had less than nine years of education, who were not living with their intimate partner when the interview was held or whose partners abused alcohol and used illicit drugs (Table 1). Postpartum IPV was also more common in women, who had been in their relationship for less than one year, those with a very controlling partner, those who reported having frequent fights with their partner or who had committed physical assaults against their partner without being assaulted first (Table 1).

Table 2 shows the results of the hierarchical analysis. The risk of postpartum IPV was higher for women with low education (RR = 2.6; 95%CI 1.3;5.4) with no personal income (RR = 1.7; 95%CI 1.0;2.9), which physically attacked the partner without being assaulted first (RR = 2.0; 95%CI 1.2;3.4), had a very controlling partner (RR = 2.5; 95%CI 1.1;5.8) and had frequent fights with their partner (RR = 1.7; 95%CI 1.0;2.9).

The association between alcohol abuse by the partner and postpartum IPV lost statistical significance when adjusted for factors related to sociodemographic conditions.

DISCUSSION

This is the first known Brazilian cohort study to estimate the incidence and risk factors for postpartum IPV. The data show a high incidence of postpartum IPV associated with gender inequality in the relationship.

The incidence of postpartum IPV was higher than that found by Guo et al (2004) and by Martin et al (2001), who evaluated postpartum physical violence. However, it was lower than the incidence of physical violence found by Gielen et al (1994) and the three types of violence cited by Hedin (2000).

As in other studies the overlapping of different types of violence was frequent. The highest incidence of psychological violence alone or overlapping with other types had been reported in some studies, , but differs from a study performed in China, that found psychological violence to be less common than sexual or physical violence. The low percentage of psychological violence reported in some studies may reflect cultural influences and structured relationships in gender inequality, which make it difficult for women to recognize situations that are considered psychological abuse. According to Charles and Perreira (2007), the high percentage of psychological violence during postpartum can be due to high levels of stress and discord, which are associated with the significant life changes that occur for a woman or a couple when a new child is born.

In this study, all episodes of sexual violence reported occurred in the first 40 days after childbirth. Macy et al (2007) found a higher percentage of sexual violence in the first month of postpartum. For Jasinski (2004), this may result of less interest of women in sexual activity in the immediate postpartum, which is possibly a result of their special hormonal state following childbirth. In addition, Ansara et al (2005) showed that women who have recently given birth may experience physical problems, ranging from fatigue to pain in various parts of the body, especially for those who are victims of IPV. Women also face problems of psychosocial adjusting to motherhood and difficulties regarding lack of support and understanding of the dynamics of puerperium, particularly from their partner.

A varied group of potential risk factors for postpartum IPV emerged during the bivariate analysis. However, during the hierarchical analysis, remained associated with the incidence of postpartum IPV women with low education, without own income and those who physically abused their partner without being assaulted first; frequent fights between the couple and controlling behavior by the partner.

The risk of postpartum IPV found in this study was higher for women who had less than nine years of schooling, which suggest the importance of education for empowerment, self-confidence and an active stance in society. Women’s higher levels of schooling can also be one of the factors that make them more vulnerable to IPV, highlighting the gender inequality that permeates violent relationships and the importance of promoting equal access to education for men and women.

The lack of women’s financial autonomy demonstrated by not having own income was associated with incidence of postpartum IPV, probably for making the woman more dependent upon the man, for her own and her children’s survival. This vulnerability hinders the possibilities of negotiating changes in the relationship, of separating from the partner or of seeking help when assaulted. In addition, women living in poverty are exposed to more severe and frequent IPV. Financially independent women can become more empowered in certain contexts, but not in those where women’s economic activity can be understood as a challenge to male identity of power and control. ,

Couples who quarreled frequently showed increased risk of postpartum IPV. For Jasinski (2004), couples with a newborn child often experience sleepless nights, and the changes in family dynamics may cause arguments between the couple, especially regarding sexual activity, which can lead to increased sexual violence. Other factors, suggested by Stewart (1994), are increasing financial responsibility, women’s physical changes and couples adjusting to their new roles of father and mother and other family relationships. This context, which includes interpersonal conflict, dissatisfaction with the relationship, poor communication and difficult for problem solving strengthens the role of the relationship dynamics between the couple regarding the incidence of IPV.

The association of controlling behavior with postpartum IPV, found during this study, was also shown in other studies on IPV. , , Controlling behavior may reflect the increased vulnerability for IPV of women living in social contexts of gender inequality.

Physical aggression perpetrated by the woman against her partner was also reported by Swan and Snow (2006) as a strong risk factor for violence against women. These authors consider contextual factors to understand the reasons that lead a woman assaulting her partner. They argue that this action can be an expression of the woman’s desire to defend herself and their children from physical harm; but it can also go beyond self-defense and include anger, revenge and the desire to control the partner.

In a study by Guo et al (2004), 44.6% of women responded all suffered aggressions, using active strategies to maximize their security, whereas a study by Silva et al (2012) shows a higher percentage (85.0%). Some studies that evaluated the bidirectionality of violence between partners found that the frequency of IPV perpetrated by women was higher, but resulting serious injury was more common when the man was the perpetrator.

Alcohol abuse by the partner was associated with postpartum IPV, but lost statistical significance when adjusted to the socioeconomic conditions. The association between alcohol abuse by the partner and IPV is still controversial. This association depends on the different ways of measuring alcohol use, types of samples and violence, individual partner characteristics, situational variables and relationship factors. , ,

This study has several advantages: it is a population-based cohort study on gender violence and complements previous results from prospective studies on postpartum IPV; , , , , , it evaluates three kinds of violence; the questionnaire used is internationally recognized, it is validated in Brazil and gauges violence based on specific acts, which increases the reliability of the results; and it has a theoretical-conceptual hierarchical model that aims to integrate contextual dimensions to understand why violence in intimate relationships exists.

Some limitations should be considered. Violence perpetrated by an intimate partner is a complex, delicate and intimate topic. Thus, women’s psychological resources used to deal with the trauma suffered and their difficulties and obstacles that result from this experience can interfere with their willingness to talk about it. Women may not always recognize their experience of IPV when questioned in some situations, or they can minimize the importance of IPV by considering it to be natural, which may contribute to the seriousness of these actions being underestimated. In addition to these factors, which are intrinsic to woman, others may cause violence to be underestimated, such as: the non-empathetic interviewer-interviewee relationship, the interview location, the woman’s insecurity about interview confidentiality, the current relationship with the partner, the sense of fear regarding the partner-aggressor and the protection that the woman gives the partner based on her desire to maintain the relationship, especially if he is the father of the child; and among many other factors, the stigma and the shame of being assaulted. With the objective of minimizing those limitations, interviewers selected had experience in dealing with “violence against women” and were properly trained and monitored during the field work.

In Brazil, the social and institutional responses to demands from women’s movements have resulted in greater IPV awareness. However, cases reported by women and cases identified and reported by institutions and health professionals still continue to be underestimated. The high observed incidence of VPI during postpartum may still be underestimated and shows that postpartum is a not period of security for either the woman or her child. Women reporting violence is dependent on, as well as various other factors, the socioeconomic conditions of women and their partners. It is also dependent on personal concepts and sociocultural contexts, in which hierarchy is more or less legitimized, which contributes to the increase or decrease in reports of violence. , , ,

This study showed a high incidence of IPV, which involved a complex network of risk factors, with emphasis on the quality of the affective-sexual relationship. In this sense, to reduce the incidence of IPV, in addition to the public policy of protection and assistance to women, preventive measures that foster conflict mediation and seek social and gender equity would be needed. In this context, another important aspect is that, in addition to women, children are also exposed to IPV during their vulnerable and early stage of development. Therefore, childcare consultations are opportunities for the health professional to build a trusting relationship with the mother, which can facilitate strategies for disclosing violence and enable to find ways of empowerment for women to face the cycle of violence.

INTRODUÇÃO

Muitos estudos sobre violência cometida contra mulheres têm indicado o parceiro íntimo como principal perpetrador, , , estimado sua magnitude e os fatores associados à violência por parceiro íntimo (VPI) antes e durante a gestação e avaliado seu impacto sobre a saúde física e mental das mulheres , e dos seus filhos. No entanto, a violência por parceiro íntimo no pós-parto (VPIPP) tem sido estudada por poucos pesquisadores. , - , , ,

A prevalência de VPIPP varia de 8,3% na China 0 a 24,2% na Suécia. A incidência de algum tipo de VPIPP (psicológica, física e sexual) encontrada no estudo de base populacional de Guo et al (2004), na China, foi de cerca de 2,5%. Já Hedin (2000) observou incidência de 69,0% numa amostra de 132 mulheres, atendidas oito semanas depois do parto, em três clínicas de pré-natal na Suécia. Nos Estados Unidos, a incidência de violência física foi encontrada em menos de 1,0% das mulheres, em estudo de base populacional, e em 11,0%, em amostra de 175 mulheres que recebiam cuidados em uma clínica de pré-natal de um hospital universitário. Essas diferenças podem ser influenciadas pelas diferentes metodologias, instrumentos, composição das amostras e período do pós-parto no qual a entrevista foi realizada, prejudicando a comparabilidade, principalmente com relação aos tipos de violência estudados.

A violência por parceiro íntimo contra as mulheres em idade reprodutiva constitui um problema de saúde pública e têm instigado interesse das áreas científica e política. Apesar disso, até a conclusão deste estudo, não foram encontrados artigos sobre incidência e fatores de risco para VPIPP no Brasil.

Os fatores de risco para VPI durante a gravidez e pós-parto são similares àqueles encontrados durante a vida: mulheres mais jovens, , sem companheiro, com baixa escolaridade e precárias condições socioeconômicas, uso de álcool e drogas pela mulher e pelo parceiro, comportamento agressivo do parceiro fora de casa, , duração do relacionamento, comunicação obstaculizada com o parceiro, brigas do casal, parceiro suspeita de infidelidade, violência física da mulher contra o parceiro sem ser agredida e o comportamento controlador do parceiro. , Outro fator de risco para VPI é a experiência da mulher ou do parceiro, na infância ou adolescência, com VPI contra a mãe ou experiência pessoal (direta) de violência perpetrada por algum familiar, indicando outro sério problema – a transmissão transgeracional da violência. ,

O objetivo deste estudo foi avaliar a incidência e identificar fatores de risco para violência por parceiro íntimo no pós-parto.

MÉTODOS

Foi realizado estudo de coorte no Distrito Sanitário II (uma das seis áreas de saúde) em região pobre da cidade do Recife, PE, na região Nordeste do Brasil.

Todas as gestantes (n = 1.133) com idade de 18 a 49 anos cadastradas na Estratégia Saúde da Família do referido distrito foram consideradas elegíveis. Dessas, 12 não responderam ao questionário: cinco eram moradoras de rua, três mudaram-se da área do estudo e quatro não foram localizadas pelas entrevistadoras mesmo após a realização de várias visitas. Uma vez contabilizadas as perdas, 1.121 (98,9%) mulheres foram entrevistadas na gravidez. Destas, 1.057 foram novamente entrevistadas no pós-parto. Houve 64 perdas entre gestação e pós-parto, sendo duas mulheres incapacitadas para a segunda entrevista, três óbitos, 37 não localizadas por mudança de endereço, quatro que passaram a viver na rua, 13 que mudaram para áreas sob controle do tráfico de drogas e cinco que se recusaram a participar. Essas 64 mulheres que não foram entrevistadas no pós-parto tinham menor escolaridade (p = 0,001), mas não mostraram diferença estatisticamente significante quanto à frequência de violência na gravidez e às outras variáveis socioeconômicas e demográficas.

O primeiro contato com as gestantes foi feito durante consulta de pré-natal. As gestantes foram identificadas a partir dos registros de cuidados no período pré-natal de 42 equipes da Estratégia Saúde da Família. Para incluir mulheres que não recebiam cuidados pré-natais em unidades da Estratégia Saúde da Família, foram avaliados também os registros de Agentes Comunitários de Saúde. A entrevista do pré-natal ocorreu preferencialmente na unidade de saúde, mas algumas mulheres foram entrevistadas em casa, quando solicitavam.

No período pós-parto, as mulheres foram contatadas nas consultas de puericultura ou no domicílio, seguindo o mesmo padrão estipulado para as entrevistas na gravidez. A maioria das entrevistas foi realizada nas casas das entrevistadas, entre maio e dezembro de 2006.

Para estimar a incidência, foram excluídas 518 mulheres que relataram VPI antes e durante a gravidez, permanecendo 539 mulheres que foram reentrevistadas até 12 meses após o parto.

Foram aplicados dois questionários: o da gestante e o da puérpera. Os dados foram obtidos entre julho de 2005 e dezembro de 2006 por entrevistadores do sexo feminino.

Confidencialidade e privacidade para as entrevistadas foram garantidas. Independentemente das mulheres terem experimentado VPI, todas receberam informações específicas sobre os serviços sociais, de saúde, jurídicos e policiais, disponíveis na área do estudo. Serviços que prestam assistência a mulheres vítimas de violência foram contatados para atender as mulheres entrevistadas que apresentavam situações de violência.

As questões relativas à VPI foram elaboradas tendo como referência o questionário do Estudo Multipaíses sobre a Saúde da Mulher e Violência Doméstica, da Organização Mundial da Saúde, já validado no Brasil.

Parceiro íntimo foi definido como um parceiro ou ex-parceiro com o qual a mulher vivia ou costumava viver, independentemente de uma união formal e com quem a mulher estava tendo ou tinha tido relações sexuais.

As perguntas para identificar a violência física foram caracterizadas como agressão física ou uso de objetos ou armas para produzir lesões; violência psicológica, como comportamentos ameaçadores, humilhações e insultos; e violência sexual, como relações sexuais impostas por meio de força física ou ameaças e imposição de atos que foram considerados humilhantes. Mulheres que responderam “sim” a pelo menos uma das questões que compõem cada tipo de violência foram consideradas como caso positivo. Cada relato de violência foi explorado com relação à ocorrência antes e durante a gravidez e no pós-parto. Informações adicionais sobre os métodos do estudo estão relatadas em outras publicações. ,

A análise foi guiada por um modelo teórico-conceitual (Figura 1), que descreve as relações hierárquicas entre fatores de risco de VPIPP, dividido em três níveis de determinantes (distal, intermediário e proximal).

O nível 1 (distal) incluiu características sociodemográficas da mulher e do seu parceiro: idade (< de 20 anos; ≥ 20 anos), raça/cor da pele (branca; não-branca), anos de escolaridade (< 9 anos; ≥ 9 anos) e inserção produtiva (inativa; ativa). Para a mulher, foram também consideradas as seguintes variáveis: viver atualmente com parceiro (sim; não), moradia (proprietária; não proprietária) e renda própria (sim; não).

O nível 2 (intermediário) incluiu variáveis de comportamento, respostas (sim; não) da mulher: tabagismo, uso abusivo de álcool e uso de drogas ilícitas; e respostas (sim; não) do parceiro: uso abusivo de álcool, uso de drogas ilícitas e comportamento agressivo fora de casa.

O nível 3 (proximal) incluiu a dinâmica da relação: duração da relação (≤ 1 ano; > 1 ano), brigas entre o casal (≥ 1 vez/mês; < 1 vez/mês), violência física cometida pela mulher contra o parceiro sem estar sendo agredida (sim; não), comunicação com o parceiro (boa; obstaculizada), e comportamento controlador do parceiro (ausente; moderado; muito). Descrição mais detalhada dessas duas últimas variáveis encontra-se publicada. A análise foi realizada usando o software Stata, versão 10.1. A regressão de Poisson foi usada para estimar risco relativo (RR), bruto e ajustado, e intervalo de confiança de 95% da associação entre VPIPP e as variáveis explanatórias.

A estratégia de análise pelo modelo hierarquizado de determinação, seguindo a direção distal-proximal, permitiu avaliar se o efeito dos fatores de risco sobre a VPIPP foi direto ou mediado por outros fatores. , O modelo 1 incluiu variáveis dos blocos 1, 2 e 3, as quais foram ajustadas pelas variáveis do mesmo nível. No modelo 2, as variáveis do bloco 2 foram ajustadas pelas variáveis do mesmo nível e por aquelas que permaneceram no modelo 1. No modelo 3, as variáveis do bloco 3 foram ajustadas pelas que permaneceram nos modelos 1 e 2.

As variáveis estatisticamente significantes (p ≤ 0,05), no modelo 1, foram mantidas nos modelos subsequentes. As de bloco já testado, que perderam significância ao ser incluído um novo bloco, permaneceram no modelo.

Considerando-se os três níveis hierárquicos, o modelo apresenta: a) estimativas dos efeitos das características sociodemográficas da mulher e do seu parceiro sobre a VPIPP, não mediados por variáveis dos níveis intermediário e proximal (seta tracejada e pontilhada da Figura 1); b) estimativas dos efeitos das características sociodemográficas da mulher e do seu parceiro sobre a dinâmica da relação, não mediados pelo nível intermediário (seta tracejada); c) estimativas dos efeitos do comportamento da mulher e do parceiro sobre a VPIPP, ajustadas por variáveis do nível distal e não mediadas por variáveis do nível proximal (seta pontilhada); e d) estimativas dos efeitos das variáveis da dinâmica da relação sobre a VPIPP, ajustadas por variáveis dos níveis distal e intermediário.

A pesquisa foi aprovada pelo Comitê de Ética da Universidade Federal de Pernambuco (Protocolo 303/2004). Todas as participantes assinaram termo de consentimento livre e esclarecido.

RESULTADOS

A incidência de algum tipo de VPIPP (Figura 2) foi de 9,3% (IC95% 7,0;12,0), sendo a violência psicológica isolada mais frequente (4,3%; IC95% 2,8;6,4) que a sobreposição de violência psicológica com física (3,3%; IC95% 2,0;5,3) e violência psicológica e/ou física e/ou sexual (1,7%; IC95% 0,8;3,0). A violência sexual foi a de menor frequência e todos os casos ocorreram nos primeiros 40 dias do pós-parto.

O risco de VPIPP foi mais comum entre as mulheres sem renda própria, com menos de nove anos de estudo, que não estavam vivendo com parceiro íntimo no momento da entrevista ou cujos parceiros faziam uso abusivo de álcool e uso de drogas ilícitas (Tabela 1). Também, foi mais frequente para as mulheres que tinham menos de um ano de relacionamento, parceiro muito controlador, ou relatos de brigas frequentes com o parceiro ou de agressões físicas contra o parceiro sem ser agredidas (Tabela 1).

A Tabela 2 apresenta os resultados da análise hierárquica. O risco de VPIPP foi maior para mulheres com baixa escolaridade (RR = 2,6; IC95% 1,3;5,4), sem renda própria (RR = 1,7; IC95% 1,0;2,9), que agrediam fisicamente o parceiro sem estarem sendo agredidas (RR = 2,0; IC95% 1,2;3,4), tinham um parceiro muito controlador (RR = 2,5; IC95% 1,1;5,8) e tinham brigas frequentes com o seu parceiro (RR = 1,7; IC95% 1,0;2,9).

A associação entre o uso abusivo de álcool pelo parceiro e IPVPP perdeu significância estatística quando ajustado para os fatores relacionados às condições sociodemográficas.

DISCUSSÃO

O presente estudo é a primeira coorte brasileira, conhecida até o momento, que estima incidência e fatores de risco para VPIPP. Os dados mostram incidência elevada de VPIPP associada à desigualdade de gênero na relação.

A incidência de VPIPP foi maior que a encontrada por Guo et al (2004) e por Martin et al (2001), avaliando apenas violência física no pós-parto. Contudo, foi menor que a incidência de violência física encontrada por Gielen et al (1994) e os três tipos de violência citados por Hedin (2000).

Assim como em outros estudos, os tipos de violência ocorrem muito frequentemente com sobreposições. A maior incidência de violência psicológica isolada ou em sobreposição com outros tipos é consistente com alguns estudos, , mas discordante com estudo conduzido na China, que encontrou violência psicológica menos comum que violência sexual e física. Os baixos percentuais de violência psicológica relatados em alguns estudos podem estar sob influências culturais e relações estruturadas na desigualdade de gênero, que dificultam o reconhecimento, pelas mulheres, de situações que são consideradas agressões psicológicas. Para Charles e Perreira (2007), o alto percentual de violência psicológica no pós-parto pode ser resultado dos elevados níveis de estresse e discórdia, associados às mudanças significativas na vida da mulher e do casal com o nascimento de um filho.

Todos os relatos de violência sexual neste estudo ocorreram nos primeiros 40 dias do puerpério. Macy et al (2007) encontraram percentual mais elevado de violência sexual no primeiro mês do pós-parto. Para Jasinski (2004), tal fato se deve ao menor interesse da mulher na atividade sexual no pós-parto imediato, possivelmente pelo estado hormonal especial do puerpério. Além disso, Ansara et al (2005) mostraram que a puérpera pode apresentar problemas físicos, que vão desde fadiga até dores em diversas partes do corpo, principalmente quando é vítima de VPI. As mulheres enfrentam, ainda, problemas de adaptação psicossocial com a maternidade e dificuldades com a falta de apoio e de entendimento da dinâmica do puerpério, sobretudo por parte do parceiro.

Um grupo variado de potenciais fatores de risco para VPIPP surgiram na análise bivariada. Contudo, na análise hierárquica, permaneceram associadas com a incidência de VPIPP mulheres com baixa escolaridade, sem renda própria e que agridem fisicamente o parceiro sem estarem sendo agredidas; brigas frequentes do casal e o comportamento controlador do parceiro.

O risco de VPIPP encontrado neste estudo, maior para mulheres com menos de nove anos de escolaridade, reafirma a importância da educação para o empoderamento, a autoconfiança e uma postura ativa na sociedade. Alguns estudos observaram que maior escolaridade das mulheres também pode ser um dos fatores que as tornam mais vulneráveis à VPI, evidenciando a desigualdade de gênero que perpassa as relações violentas e a importância de promover o acesso igualitário à educação para homens e mulheres.

A falta de autonomia financeira da mulher, evidenciada por não ter renda própria, mostrou-se associada à incidência de VPIPP, provavelmente por tornar a mulher mais dependente do homem, tanto pela sua sobrevivência como pela do filho. Essa vulnerabilidade dificulta as possibilidades de negociar mudanças na relação, de separar-se do parceiro agressor ou procurar ajuda quando agredida. Além disso, as mulheres que vivem na pobreza estão expostas à VPI de forma mais severa e frequente. Com independência financeira, as mulheres podem se tornar mais fortalecidas em determinados contextos, mas não naqueles onde a atividade econômica da mulher pode ser entendida como um desafio à identidade masculina de poder e de domínio. ,

Casais que brigavam frequentemente apresentaram risco maior de VPIPP. Para Jasinski (2004), no pós-parto, com um filho recém-nascido, as noites mal dormidas e as mudanças na dinâmica familiar podem provocar conflitos entre o casal, especialmente discussões sobre a atividade sexual, o que pode levar ao aumento da violência sexual. Outros fatores, sugeridos por Stewart (1994), são o aumento da responsabilidade financeira, as mudanças físicas da mulher e os ajustes do casal nos novos papéis de pai e mãe e nas outras relações familiares. Esse contexto, que inclui conflito interpessoal, insatisfação com a relação, comunicação precária e dificuldade para resolver problemas, reforça o papel da dinâmica da relação entre o casal na ocorrência de VPI.

A associação do comportamento controlador com a VPIPP, encontrada neste estudo, foi também mostrada em outros estudos sobre VPI ao longo da vida. , , O comportamento controlador pode refletir o aumento da vulnerabilidade para VPI de mulheres que vivem em contextos sociais de desigualdade de gênero.

Agressão física perpetrada pela mulher contra seu parceiro foi também relatada por Swan e Snow (2006) como um forte fator de risco para violência contra a mulher. Esses autores consideraram fatores contextuais para entender os motivos que levam uma mulher a agredir o seu parceiro e argumentam que esse fato pode expressar o desejo para defender a si própria e a seus filhos de dano físico; mas também pode ir além da legítima defesa e abranger a raiva, a vingança e o desejo de controlar o parceiro.

No estudo de Guo et al (2004), 44,6% das mulheres revidaram todas as agressões sofridas, usando estratégias ativas para maximizar sua segurança, enquanto o estudo de Silva et al (2012) mostra percentual maior, de 85,0%. Alguns estudos que avaliaram a bidirecionalidade da violência entre os parceiros encontraram que a frequência de VPI perpetrada por mulheres foi maior, mas a ocorrência de lesões graves foi mais frequente quando o homem foi o perpetrador.

O uso abusivo de álcool pelo parceiro foi associado à VPIPP, mas perdeu significância estatística quando ajustado às condições socioeconômicas. A associação entre abuso de álcool pelo parceiro e ocorrência de VPI ainda é controversa. Essa associação depende das diferentes formas de medir o uso do álcool, tipos de amostras e de violência, características individuais dos parceiros, variáveis situacionais e fatores da relação. , ,

Este estudo tem diversas vantagens: é uma coorte de base populacional sobre violência de gênero e complementa os resultados anteriores de estudos prospectivos sobre VPIPP; , , , , , avalia os três tipos de violência; o questionário aplicado é reconhecido internacionalmente, validado no Brasil e afere a violência a partir de atos concretos, o que aumenta a fidedignidade dos resultados; e apresenta um modelo hierárquico teórico-conceitual que procura integrar dimensões contextuais para entender porque a violência ocorre nas relações íntimas.

Algumas limitações devem ser consideradas. Violência por parceiro íntimo é um tema complexo, delicado e íntimo. Então, os recursos psíquicos da mulher para enfrentar o trauma sofrido e suas dificuldades e bloqueios decorrentes dessa experiência podem interferir na sua disponibilidade para falar sobre esse assunto. Em algumas situações, as mulheres podem não reconhecer a experiência de VPI quando questionadas ou podem minimizar a importância da VPI por considerá-la como natural, o que pode contribuir para a subestimação da sua seriedade. Além desses fatores, que são intrínsecos da mulher, outros podem causar a subestimação da violência, como: a relação não-empática entre entrevistadora-entrevistada, o local da entrevista, a insegurança da mulher sobre a confidencialidade do seu relato, a relação atual com o parceiro agressor, o sentimento de medo em relação ao parceiro-agressor e a proteção que a mulher dá ao parceiro pelo desejo de manter a relação, especialmente se ele é o pai da criança; e entre tantos outros fatores, o estigma e a vergonha de ser agredida. Para minimizar essas limitações, as entrevistadoras selecionadas tinham experiência em lidar com o tema “violência contra a mulher” e foram devidamente treinadas e acompanhadas durante o trabalho de campo.

No Brasil, as respostas sociais e institucionais às demandas dos movimentos de mulheres têm propiciado maior visibilidade à VPI. No entanto, a subestimação de casos revelados pelas mulheres e de casos identificados e notificados pelas instituições e pelos profissionais de saúde ainda persiste. A alta incidência de VPI no pós-parto encontrada pode ainda estar subestimada e mostra que o pós-parto não é um período de segurança nem para a mulher nem para a criança. A revelação da violência pelas mulheres depende, além de diversos outros fatores, das condições socioeconômicas das mulheres e de seus parceiros e dos conceitos pessoais e contextos socioculturais, nos quais a hierarquia de gênero é mais ou menos legitimada, o que contribui para o aumento ou diminuição dos relatos de violência. , , ,

O presente estudo mostrou elevada incidência de VPI, envolvida numa complexa rede de fatores de risco, com destaque para a qualidade da relação afetiva-sexual. Nesse sentido, para reduzir a incidência de VPI seriam necessárias, além das políticas públicas de proteção e assistência, medidas de prevenção que promovam a mediação de conflitos e busquem equidade social e de gênero. Outro aspecto importante é que nesse contexto, além das mulheres, as crianças também estão expostas à VPI numa fase precoce e vulnerável do desenvolvimento. Portanto, as visitas para as consultas de puericultura são oportunidades para o profissional de saúde construir com a mulher um vínculo de confiança, que pode facilitar estratégias para revelação da situação de violência e possibilitar a busca de formas de empoderamento das mulheres para enfrentar o ciclo da violência.
