Team-Based Care for External Telemonitoring in Patients with Heart Failure.

Josie Dimengo1, Gerrye Stegall2.

Abstract

Multiple studies have been conducted assessing strategies for managing heart failure with the goal of decreasing admissions and readmissions, decreasing mortality, increasing self-management, and improving quality of life. Telemonitoring has been explored as an intervention to support and monitor adherence to all aspects of the heart failure management plan, including medication and appointment adherence, diet, exercise, and self-care. This article addresses the findings of studies and provides recommendations for future consideration. The need to apply the right interventions, to the right patient, at the right time, based on self-efficacy and readiness to change is also addressed. Published by Elsevier Inc.
