[Waist circumference, waist/height ratio, and neck circumference as parameters of central obesity assessment in children].

OBJECTIVE: To analyze studies that assessed the anthropometric parameters waist circumference (WC), waist-to-height ratio (WHR) and neck circumference (NC) as indicators of central obesity in children. DATA SOURCES: We searched PubMed and SciELO databases using the combined descriptors: "Waist circumference", "Waist-to-height ratio", "Neck circumference", "Children" and "Abdominal fat" in Portuguese, English and Spanish. Inclusion criteria were original articles with information about the WC, WHR and NC in the assessment of central obesity in children. We excluded review articles, short communications, letters and editorials. DATA SYNTHESIS: 1,525 abstracts were obtained in the search, and 68 articles were selected for analysis. Of these, 49 articles were included in the review. The WC was the parameter more used in studies, followed by the WHR. Regarding NC, there are few studies in children. The predictive ability of WC and WHR to indicate central adiposity in children was controversial. The cutoff points suggested for the parameters varied among studies, and some differences may be related to ethnicity and lack of standardization of anatomical site used for measurement.
CONCLUSIONS: More studies are needed to evaluate these parameters for determination of central obesity children. Scientific literature about NC is especially scarce, mainly in the pediatric population. There is a need to standardize site measures and establish comparable cutoff points between different populations.

Introduction

The prevalence of obesity in children has increased worldwide1 and is associated with risk factors for cardiovascular and metabolic disorders, which, due to their chronic and insidious nature, require careful monitoring in childhood, aimed at early detection and the establishment of interventions to prevent complications in adulthood.2 , 3

The body mass index (BMI) is the most commonly used parameter in all age groups to determine overweight and obesity. However, it does not provide accurate information on body fat distribution.2 Fat distribution is related to future health risks, and central obesity is more strongly associated to several risk factors for cardiovascular diseases than overall obesity.4

Body fat distribution can be verified through several anthropometric parameters. In recent years, new indicators have been proposed to evaluate central adiposity, such as waist circumference (WC), waist/height ratio (WHR), and neck circumference (NC). NC is a simple technique that can be used in screening children and adolescents, as well as in adults, with good performance as an indicator of central adiposity in both genders.5

WHR has also been proposed as a measure to evaluate central adiposity in childhood and adulthood in several populations.6 NC is a relatively new parameter for the evaluation of children and adolescents, of simple and fast measurement, and is an indicator of subcutaneous fat distribution in the upper body.7

However, despite these studies, there is no work in the literature aiming at a critical analysis of all three parameters as markers of adiposity in childhood.

In this context, this review aimed to analyze studies that evaluated WC, WHR, NC, and anthropometric parameters as indicators of central obesity in children.

Data source

The present study consisted of an integrative review performed after a search in the PubMed and Scientific Electronic Library Online (SciELO) databases. The bibliographic search was conducted in national and international journals in the databases through the portal of Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES). The keywords used to search for articles were: "waist circumference", "waist-to-height ratio", "neck circumference", "Children", "abdominal fat", and its correlates in Portuguese and Spanish. To search in PubMed, the following combination of descriptors in English was used: "Waist circumference", "Waist-to-height ratio", "Neck circumference", "Abdominal fat", and "Children", using as search criteria "All fields" for the first four and "Title/Abstract" for the latter. The search at SciElo, in turn, used the combination of descriptors in Portuguese and Spanish ("Perímetro da cintura/Circunferencia de la cintura", "Relação cintura/estatura/Relación cintura/talla", "Perímetro do pescoço/Circunferencia del cuello", "Gordura abdominal/ Grasa abdominal" e "Crianças/Niños"), to include articles published in national and international journals, in both languages, using as search criteria "All fields" for these descriptors.

Identified studies were selected by reading the abstracts, using, as an inclusion criterion, original articles that had information about the WC, NC, and WHR in the assessment of central obesity in children, in Portuguese, English, or Spanish. The exclusion criteria were: review articles, short communications, letters, and editorials. There was no limitation regarding the time of publication, considering that studies on such parameters are relatively recent in this age group.

Based on selected articles, a file was created for information extraction, including: names of authors, year of publication, purpose, location and type of study, sample size and characteristics, method of anthropometric measurements, statistical analyses performed, main results, and suggested cutoffs.

Data synthesis

The database search retrieved 1,525 studies on the topic. The number of identified articles categorized by database and descriptors used are described in Table 1. Initially, articles were analyzed based on the relevance of titles and abstracts, and 1,457 studies were excluded for not meeting the objectives of this review, which focused on the use of anthropometric parameters in the assessment of central obesity in children. Thus, 68 articles were selected for full reading.

However, after reading the articles in full, 19 were excluded for not meeting the inclusion criteria. Thus, 49 articles were part of this review, which were published in the period between 2000 and 2013. Among the selected studies, most (26 articles) referred to WC,3 , 5 , 8 - 31 12 articles assessed WHR,32 - 43 six studies assessed both parameters,44 - 49 and only five assessed NC.2 , 50 - 53 Fig. 1 presents the flowchart of the steps performed to select the studies for this review.

Waist Circumference (WC)

WC is the most widely used measure to assess abdominal obesity, and several studies have addressed its capacity to indicate central fat accumulation in children, as well as its positive correlation with BMI,8 - 10 , 44 total fat,11 and upper body fat percentage.12 Some authors suggest that this parameter is more consistent in terms of the balance between sensitivity and specificity to evaluate obese and non-obese children than BMI and WHR,45 and is a good indicator of central adiposity in children.13 Furthermore, it demonstrates a satisfactory performance in predicting total body fat content,11 , 14 as well as an indicator of upper body fat mass.5 , 15

However, some studies have not shown favorable results when using this parameter. Reilly et al,16 studying English children aged 9-10 years, compared the capacity of BMI and WC, in percentiles, to diagnose increased fat mass. The authors observed higher specificity of BMI percentile for both genders, using dual energy x-ray absorptiometry (DXA) as the reference method. In turn, in a study of Venezuelan children and adolescents aged 7-17 years using the subscapular/triceps skinfold ratio as the reference method in the analysis of ROC (receiver operating characteristic) curve, Perez et al 46 observed that WC was not effective to identify fat distribution, not showing satisfactory sensitivity and specificity.

Regarding reference values, studies conducted in different parts of the world have established WC cutoffs to determine central adiposity. The values were established using the LMS (L=lambda, asymmetry; M=Mi, median; and S=sigma, coefficient of variation) method, and were shown as percentile values and standard deviations,3 , 17 - 24 , 30 , 31 , 47 or were based on the ROC analysis, considering the BMI score as overweight/obesity according to the classification of the International Obesity Task Force (IOTF)25 , 26 , 48 or excess upper body fat measured by DXA5 , 49 as reference methods.

In most studies, the 90th percentile of the distribution of WC values was used as the critical value.17 - 20 , 22 - 24 , 26 , 47 Although some studies have shown significant differences when the WC measurement is performed at different anatomic sites in children,27 , 28 there was no agreement regarding the measurement site. Some studies performed the measure at midpoint between the last rib and the top of the iliac crest;3 , 17 - 24 , 47 , 48 others measured at "the minimum circumference between the iliac crest and the rib cage,5 , 26 slightly above the upper lateral border of the right ilium,29 or at the largest frontal extension of the abdomen between the bottom of the rib cage and the top of the iliac crest.25 One study performed the measurement in two places: at navel level and at midpoint between the anterosuperior iliac spine and the bottom of the rib cage.49 Table 2 presents a summary of these studies.

Waist-To-Height Ratio (WHR)

The rationale for the use of WHR is that, for a given height, there is an acceptable degree of fat stored in the upper portion of the body.32 Among the studies evaluating this parameter in children, controversial results were observed. In the study by Sant'Anna et al, 32 there was a strong correlation between the percentage of body fat and WHR of children aged 6-9 years of both genders, while Majcher et al 33 found no correlation between these parameters in children. A good correlation with BMI was observed in schoolchildren of both genders in the study performed by Ricardo et al 44 in southern Brazil, where it was suggested that WHR could be used as additional information to BMI/ age to determine total and central adiposity, respectively.

When comparing the diagnostic quality of BMI, WC, and WHR in screening for obesity in children, Hubert et al 45 concluded that WHR was not very effective to classify childhood obesity. In the study by Perez et al,46 with Venezuelan children and adolescents, the WHR also did not effectively identify fat distribution, as it did not provide adequate sensitivity and specificity.

Conversely, the study by Marrodán et al 34 demonstrated that WHR was an effective method to predict relative adiposity in children and adolescents aged 6-14 years. Brambilla et al 35 observed that, when compared to WC and BMI, WHR was the best predictor of adiposity in children and adolescents, suggesting that this parameter may be a useful substitute for measuring body fat when other measures are not available.

Considering the residual correlation between WHR (waist/height1 )) and height in children, studies have sought to investigate the dependence of this parameter on height and the influence of specific exponents on its predictive capacity to discriminate between children with different fat distribution.36 - 38 The value of 0.50 for the WHR has been established as the suitable cutoff for both adults and children.39 , 47 , 48 However, other values, most of them higher than 0.50, have been suggested to determine central obesity.

Regarding the methodological aspects, some studies used the classification of overweight/obesity by BMI according to the IOTF for the ROC curve analysis,34 , 40 , 41 while others considered the high percentage of body fat and upper body fat in relation to the study population, measured by bioelectrical impedance,32 DXA,49 and skinfold thickness42 as reference methods. There was also no agreement regarding the anatomical site for WC measurement, which leads to changes in WHR measurements. The cutoffs of these studies are presented in Table 3.

Neck Circumference (NC)

Regarding NC, there have been few studies evaluating this parameter as an indicator of adiposity in children. The retrieved studies demonstrated that this anthropometric measurement had good performance in determining overweight and obesity in children and adolescents.2 , 50 - 52 Significant positive correlations could be observed between NC and BMI in both genders, as well as high correlations with other indices, such as those that assess central obesity, WC,2 , 50 - 52 and arm circumference.50 However, a low correlation with the percentage of body fat was observed.50 In the study by Nafiu et al,51 NC appears to correlate better with BMI and WC in males than in females, and a stronger correlation was found between NC and other anthropometric indices in older children than in younger ones.

When evaluating a total of 4,581 Turkish children and adolescents from elementary and high schools in the city of Kayseri, Central Anatolia, Mazicioglu et al 50 established mean, median, and percentile values of NC that can be used as preliminary data for future studies on body fat distribution. The NC cutoffs to identify overweight and obesity suggested by several studies are shown in Table 4. In all studies, NC was measured at the level of the thyroid cartilage; in contrast, the stratification of overweight/obesity by BMI used in the ROC analysis differed between the studies, which followed the classifications of the IOTF,50 the Centers for Disease Control and Prevention,51 and the Chinese Obesity Task Force,52 whereas other researchers2 used a local reference, explaining that references in stature can differ significantly between populations.

Discussion

According to what has been suggested by most studies, WC is an anthropometric measure that provides relevant information about body fat distribution, reflecting the degree of central adiposity in children. Some controversy could be explained by the study methodology, such as the work of Perez et al,46 which used the subscapular/triceps skinfold ratio as the reference method for ROC curve analysis. Although skinfold thickness is often used to estimate total body fat, there is considerable variability between individuals regarding subcutaneous thickness, tissue compressibility in a given location, and the ratio of several deposits of adipose tissue.54

It is worth mentioning that computed tomography (CT) and magnetic resonance imaging (MRI) are considered the gold standard methods to assess body fat distribution, providing information about the morphological and anatomical location of different deposits (subcutaneous, visceral, and intermuscular adipose tissue). DXA, in turn, measures total fat with high accuracy and relatively low radiation, but does not differentiate between intra-abdominal and subcutaneous fat.54

Considering the significant differences observed among several studies in which the WC values were compared,18 , 19 , 22 - 24 , 29 it is important to establish reference values for WC in children, stratified by age and gender, that are specific for the population of several countries.

Currently, there is no consensus regarding the anatomical site where the WC should be measured. However, in a study of children aged 6-9 years-old, it was observed that the WC measured at midpoint between the last rib and the iliac crest presented the best correlation with body fat percentage.27 Bosy-Westphal et al 28 observed that, in children, WC values differed significantly according to the anatomical site of measurement. The smallest value was found when the measurement was performed below the last rib, and the highest value, above the iliac crest, whereas an intermediate value was found at midpoint between these sites.

In the study by Sant'Anna et al,27 the measurement performed on the umbilicus was statistically higher among females. Thus, the interpretation of differences in WC between studies of different populations should be performed with caution, considering that the measure may have been performed in different anatomical sites.

WHR is a simpler measure of health risk than other anthropometric indices in children, such as BMI/age, as it requires no adjustment for age and gender,43 having emerged as a central adiposity parameter and significant predictor of risk factors for cardiovascular disease in children and adolescents. Ashwell and Hsieh6 proposed the use of WHR as a simple screening tool, considering its fast and effective measurement. They suggest that WHR is more sensitive, less expensive, and easier to measure and calculate than BMI.

Moreover, a value of 0.5 would indicate an increased risk for males and females of different ethnic groups, being applicable to adults and children. However, in a study of 5,725 Norwegian children and adolescents,48 the recommended cutoff of 0.5 showed high sensitivity and specificity to detect obesity in individuals aged 6-18 years; however, in younger children, this cutoff was not appropriate due to low specificity. The authors also suggest that, for overweight, the cutoffs should be different for children and adolescents aged 6-12 years and 12-18, and should not be defined for the younger age group.48

Due to the residual correlation between WHR and stature in children, the division of WC by height elevated to the power of 1 (waist/height1) may be insufficient to properly adjust the height during growth.36 , 37 Tybor et al 36 evaluated a representative sample of children and adolescents aged 2-18 years of the National Health and Nutrition Examination Survey (NHANES), from 1999-2004, stratified by age and gender, and found a residual correlation between height and WHR between 0.29 and 0.36.

Conversely, the study by Taylor et al 37 demonstrated that the simple division of the WC by height correctly discriminates, at least 90% of the time, children and adolescents with high and low levels of total and central fat.

The validity of WHR by the formula waist/height1 was evaluated by Nambiar et al 38 in a cohort of 3,597 Australian children aged 5 to 17 years. The authors observed that WHR could be used in the study population, and was more appropriate than BMI due to its capacity to explain body fat distribution and the associated cardiovascular health risks. These findings indicate the need for further researches to investigate the degree of dependence of WHR with height and how this influences the association between central adiposity and risk factors for cardiovascular disease in this age group. Furthermore, it is necessary to evaluate whether the use of an exponent different from 1 can reduce bias and improve measurement accuracy.

Regarding the NC, despite the scarcity of studies in the literature that adopted this measurement, the results those that used it as a parameter to assess central adiposity in children indicate that such measurement may be a useful screening tool to identify overweight or obesity. It may also be useful to diagnose children at risk for high adiposity, an important predictor of cardiovascular health problems, especially when references adjusted for age and gender are available.2 , 51 The high correlations between NC and BMI may indicate that NC is a reliable index to determine obesity. Additionally, significant correlations between this parameter and other indicators of central obesity reflect the similarity between them.

The low correlation between the NC and the percentage of body fat, in turn, may mean that NC is a measure of disproportionate accumulation of fat instead of a general measure of obesity.50 A strong point in favor of the use of NC is that it has a good intra- and inter-rater reliability,53 and it is not necessary to perform multiple measurements to attain accuracy and reliability. Additionally, when compared to other indicators of upper body fat, NC measurement is simpler.

NC is a new parameter that has shown good results in the evaluation of children and can be used both in clinical practice and in epidemiological studies as a marker for central obesity. Special attention should be given to this parameter in children, as findings in researches conducted with infants have shown an association with cardiometabolic risk factors.7 , 55 However, further studies to evaluate the usefulness of NC as an indicator of adiposity are needed, and it is necessary to establish reference values for children at a younger age range.

It can be concluded that WC was the most often used parameter in studies, and has shown good performance in the assessment of central obesity, although the results of some studies are controversial. WHR has been proposed as a useful parameter to assess fat distribution in children, but some issues are worth investigating, such as a residual correlation with height during growth. NC, although a more recent measure and little studied so far, has proved to be satisfactory as a parameter to assess central adiposity in children.

The differentiated cutoffs for the different studied parameters may be due to ethnic differences, as well as the lack of standardization of the anatomical point used in the assessment of measures such as WC. Thus, new studies are necessary in order to further investigate the usefulness of these parameters in determining central obesity in childhood, including the standardization of the place where measures are to be taken and the determination of cutoffs that are comparable between different populations.

Introdução

A prevalência de obesidade em crianças tem aumentado em todo o mundo1 e está associada a fatores de risco para distúrbios cardiovasculares e metabólicos, que, por sua natureza crônica e insidiosa, requerem um acompanhamento cuidadoso na infância, visando à detecção precoce e ao estabelecimento de intervenções para prevenir complicações na vida adulta.2 , 3

O índice de massa de corporal (IMC) é o parâmetro mais comumente utilizado entre todas as faixas etárias para determinar sobrepeso e obesidade. Entretanto, não fornece informação precisa sobre a distribuição da gordura corporal.2 A distribuição de gordura relaciona-se ao prognóstico de risco para a saúde, sendo que a obesidade central se associa mais fortemente a diversos fatores de risco para doenças cardiovasculares, quando comparada à obesidade corporal total.4

A distribuição da gordura corporal pode ser verificada por meio de diversos parâmetros antropométricos e, nos últimos anos, novos indicadores têm sido propostos para avaliar a adiposidade central. Merecem destaque o perímetro da cintura (PC), a relação cintura/estatura (RCE) e o perímetro do pescoço (PP). A medida do PC é uma técnica simples que, assim como em adultos, poderia ser utilizada na triagem de crianças e adolescentes, com bom desempenho como indicador de adiposidade central em ambos os sexos.5 A RCE também tem sido proposta como uma medida a ser utilizada para avaliar a adiposidade central na infância e na vida adulta em diversas populações.6 O PP é um parâmetro relativamente novo na avaliação de crianças e adolescentes, de mensuração simples e rápida, sendo um indicador da distribuição de gordura subcutânea na parte superior do corpo.7

No entanto, apesar desses estudos, não há trabalho na literatura visando a uma análise crítica dos referidos parâmetros, de forma conjunta, como marcadores de adiposidade na infância. Nesse contexto, o presente artigo objetiva analisar estudos que avaliaram os parâmetros antropométricos PC, RCE e PP como indicadores da obesidade central em crianças.

Fontes dos dados

O presente estudo consistiu numa revisão integrativa realizada por meio de busca eletrônica nas bases de dados PubMed e Scientific Electronic Library Online (SciELO). O levantamento bibliográfico se deu em periódicos nacionais e internacionais nas bases de dados pelo portal da Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES). Os descritores utilizados para a busca dos artigos foram: "Perímetro da cintura", "Relação cintura/estatura", "Perímetro do pescoço", "Crianças", "Gordura Abdominal" e seus correlatos em inglês e espanhol. Para a pesquisa no PubMed, utilizou-se combinação dos descritores em inglês "Waist circumference", "Waistto-height ratio", "Neck circumference", "Abdominal fat" e "Children", utilizando como critério de busca "All fields" para os quatro primeiros e "Title/abstract" para o último. A pesquisa no SciElo, por sua vez, utilizou a combinação dos descritores em português e espanhol ("Perímetro da cintura/Circunferencia de la cintura", "Relação cintura/ estatura/Relación cintura/talla", "Perímetro do pescoço/ Circunferencia del cuello", "Gordura abdominal/Grasa abdominal" e "Crianças/Niños"), de forma a abranger artigos publicados em periódicos nacionais e internacionais, em ambos os idiomas, utilizando como critérios de busca "Todos os índices" para os referidos descritores.

Os estudos identificados foram selecionados pela leitura dos resumos, adotando-se como critério de inclusão os artigos originais que apresentassem informações relativas a PC, RCE e PP na avaliação da obesidade central em crianças em português, inglês ou espanhol. Como critérios de exclusão, adotaram-se: artigos de revisão, comunicação breve, cartas e editoriais. Não se delimitou o período das publicações, considerando que os estudos de tais parâmetros são relativamente recentes nessa faixa etária.

Com base nos artigos selecionados, elaborou-se uma ficha para extração de informações, incluindo-se: nomes dos autores e ano de publicação, objetivo, local e tipo do estudo, tamanho e características da amostra, forma de aferição das medidas antropométricas, análises estatísticas realizadas, principais resultados, pontos de corte sugeridos.

Síntese dos dados

A pesquisa nas bases de dados resultou em 1.525 estudos sobre o tema. O número de artigos identificados categorizados por base de dados e descritores utilizados está descrito na tabela 1. Inicialmente, os artigos foram analisados com base na relevância de títulos e resumos, sendo então excluídos 1.457 estudos que não atendiam aos objetivos desta revisão, cujo enfoque é a utilização dos parâmetros antropométricos na avaliação da obesidade central em crianças. Assim, 68 artigos foram selecionados para leitura na íntegra. Entretanto, após a leitura dos artigos completos, 19 foram excluídos por não atenderem aos critérios de inclusão. Dessa forma, 49 artigos fizeram parte desta revisão, os quais foram publicados no período de 2000 a 2013. Entre os estudos selecionados, a maior parte (26 artigos) era referente ao PC,3 , 5 , 8 - 31 12 artigos abordaram a RCE,32 - 43 6 estudos contemplaram ambos os parâmetros,44 - 49 e apenas 5 trabalhos eram referentes ao PP.2 , 50 - 53 A figura 1 apresenta o fluxograma demonstrando as etapas realizadas até a seleção dos estudos para compor esta revisão.

Perímetro da cintura

O PC é a medida mais utilizada para avaliar a obesidade abdominal, e diversos estudos abordaram sua capacidade para indicar o acúmulo de gordura central em crianças, bem como sua correlação positiva com o IMC8 - 10 , 44 e com o percentual de gordura total11 e do tronco.12 Alguns autores sugerem que esse parâmetro é mais consistente, em termos do balanço entre sensibilidade e especificidade, para avaliar crianças obesas e não obesas, do que o IMC e a RCE,45 sendo bom indicador de adiposidade central em crianças.13 Além disso, demontra-se desempenho satisfatório na predição do conteúdo de gordura corporal total,11 , 14 além de ser indicador de massa de gordura do tronco.5 , 15

Entretanto, alguns estudos não apresentam resultados favoráveis à utilização desse parâmetro. Reilly et al,16 estudando crianças inglesas de 9 a 10 anos, compararam a capacidade do IMC e do PC, em percentis, para diagnosticar a massa de gordura aumentada. Os autores observaram maior especificidade do percentil de IMC para ambos os sexos, utilizando Dual Energy X-ray Absorptiometry (DEXA) como método de referência. Pérez et al,46 por sua vez, numa pesquisa realizada com crianças e adolescentes venezuelanos na faixa etária de 7 a 17 anos por meio da razão prega cutânea subescapular/tricipital como método de referência na análise da curva ROC (Receiver-Operating characteristic), encontraram que o PC não foi eficaz para identificar a distribuição de gordura, não apresentando valores satisfatórios de sensibilidade e especificidade.

No tocante aos valores de referência, estudos realizados em diversas partes do mundo estabeleceram pontos de corte do PC para determinar a adiposidade central. Os valores foram estabelecidos utilizando o método LMS (L=lambda: assimetria; M=mi: mediana; e S=sigma: coeficiente de variação) e apresentadas como valores de percentis e desvios-padrão3 , 17 - 24 , 30 , 31 , 47 ou a partir da análise ROC, sendo considerado o IMC sobrepeso/obesidade de acordo com a classificação do International Obesity Task Force (IOTF)25 , 26 , 48 ou excesso de gordura do tronco medido pela DEXA5 , 49 como métodos de referência.

Na maioria dos estudos, o percentil 90 da distribuição dos valores de PC foi adotado como valor crítico.17 - 20 , 22 - 24 , 26 , 47 Apesar de trabalhos terem apontado diferenças significativas quando a medida do PC é realizada em diferentes pontos anatômicos em crianças,27 , 28 não houve concordância quanto ao local de aferição da medida entre os estudos. Alguns trabalhos realizaram a medida no ponto médio entre a última costela e a parte superior da crista ilíaca;3 , 17 - 24 , 47 , 48 outros, na "circunferência mínima entre a crista ilíaca e a caixa torácica",5 , 26 "um pouco acima da borda superior lateral do ílio direito",29 na "maior extensão frontal do abdome entre a parte inferior da caixa torácica e o topo da crista ilíaca",25 e um estudo realizou a medida em dois locais: no "nível do umbigo" e "na distância média entre a espinha ilíaca anterossuperior e a parte inferior da caixa torácica".49 A tabela 2 apresenta um resumo desses estudos.

Relação cintura/estatura

A base lógica da RCE é que, para determinada estatura, há um grau aceitável de gordura armazenada na porção superior do corpo.32 Entre os estudos que avaliaram esse parâmetro na população infantil, foram encontrados resultados controversos. No estudo de Sant'Anna et al,32 houve forte correlação entre o percentual de gordura corporal e a RCE de crianças de 6 a 9 anos de ambos os sexos, enquanto Majcher et al 33 não observaram nenhuma correlação entre

o percentual de gordura corporal e a RCE em crianças. Boa correlação com o IMC foi verificada em escolares de ambos os sexos na pesquisa realizada por Ricardo, Gabriel e Corso,44 no Sul do Brasil, na qual foi sugerido que a RCE poderia ser utilizada como informação complementar ao IMC/idade para determinar a adiposidade corporal central e total, respectivamente.

Ao comparar a qualidade diagnóstica de IMC, PC e RCE na triagem de obesidade em crianças, Hubert et al 45 concluíram que a RCE foi pouco eficaz na classificação do estado de obesidade infantil. No trabalho de Pérez et al,46 com crianças e adolescentes venezuelanos, a RCE também não identificou de forma eficaz a distribuição de gordura, uma vez que não apresentou valores adequados de sensibilidade e especificidade.

Por outro lado, o estudo de Marrodán et al 34 mostrou que a RCE foi um método eficaz para prever a adiposidade relativa em crianças e adolescentes de 6 a 14 anos. Brambilla et al 35 observaram em seu trabalho que, comparada ao PC e ao IMC, a RCE foi o melhor preditor da adiposidade em crianças e adolescentes, sugerindo que esse parâmetro pode ser um substituto útil para medida de adiposidade corporal quando outras medidas não estão disponíveis.

Considerando a correlação residual entre a RCE (cintura/ estatura1) e a altura em crianças, estudos buscaram investigar a dependência desse parâmetro da altura e a influência de expoentes específicos sobre sua capacidade preditiva na discriminação entre crianças com diferentes distribuições de gordura.36 - 38 O valor de 0,50 para a RCE vem sendo estabelecido como ponto de corte apropriado tanto para adultos quanto para crianças.39 , 47 , 48 Entretanto, outros valores, em sua maioria inferiores a 0,50, têm sido sugeridos para determinar a obesidade central.

Quanto aos aspectos metodológicos, para análise da curva ROC alguns estudos utilizaram a classificação de sobrepeso/obesidade pelo IMC de acordo com o IOTF,34 , 40 , 41 enquanto outros consideraram o elevado percentual de gordura corporal e do tronco em relação à própria população do estudo, mensurados por bioimpedância elétrica,32 DEXA49 e pregas cutâneas42 como métodos de referência. Também não houve concordância quanto ao ponto anatômico da medida do PC, o que leva a alterações nas medidas de RCE. Os pontos de cortes referentes a esses estudos são apresentados na tabela 3.

Perímetro do pescoço

Em relação ao PP, existem poucos estudos avaliando esse parâmetro como indicador de adiposidade em crianças. Os estudos encontrados mostraram que essa medida antropométrica apresenta bom desempenho na determinação do sobrepeso e da obesidade em crianças e adolescentes.2 , 50 - 52 Encontraram-se correlações positivas significantes entre PP e IMC em ambos os sexos, bem como elevadas correlações com outros índices, como os que avaliam a obesidade central, o PC 2 , 50 - 52 e o perímetro braquial.50 Por outro lado, observou-se baixa correlação com o percentual de gordura corporal.50 No estudo de Nafiu et al, 51 o PP parece se correlacionar melhor com o IMC e o PC em meninos do que em meninas, e, além disso, uma correlação mais forte foi verificada entre PP e outros índices antropométricos em crianças mais velhas do que em mais jovens.

Avaliando um total de 4.581 crianças e adolescentes turcos de escolas primárias, secundárias e de ensino médio na cidade de Kayseri da Anatólia Central, Mazicioglu et al 50 geraram valores de médias, medianas e percentis de PP que podem servir como dados preliminares para estudos futuros na distribuição da gordura corporal. Os pontos de corte de PP para identificar crianças com sobrepeso e obesidade sugeridos pelas diversas pesquisas estão dispostos na tabela 4. Em todos os estudos, a medida do PP foi aferida no nível da cartilagem da tireoide; por outro lado, a classificação de sobrepeso/obesidade pelo IMC utilizada na análise ROC diferiu entre os trabalhos, sendo aplicadas classificações de acordo com o IOTF,50 o Center Control Disease,51 a Chinese Obesity Task Force,52 e outros pesquisadores2 utilizaram uma referência local, justificando que referências de estatura podem diferir significativamente entre diferentes populações.

Discussão

De acordo com o que tem sido sugerido pela maioria dos estudos, o PC é uma medida antropométrica que fornece informação relevante sobre a distribuição de gordura corporal, refletindo o grau de adiposidade central em crianças. Algumas controvérsias poderiam ser explicadas pela metodologia do estudo, a exemplo do trabalho de Pérez et al,46 em que se utilizou a razão prega cutânea subescapular/tricipital como método de referência na análise da curva ROC. Embora as espessuras das pregas cutâneas sejam frequentemente aplicadas para estimar a gordura corporal total, existe considerável variabilidade entre indivíduos na espessura subcutânea e compressibilidade do tecido em dado local e na razão de vários depósitos de tecido adiposo.54

É importante destacar que a tomografia computadorizada (TC) e as imagens de ressonância magnética (IRM) são considerados métodos padrão-ouro para avaliar a distribuição de gordura corporal, fornecendo informações sobre a localização anatômica e morfológica de diferentes depósitos (tecido adiposo subcutâneo, visceral e intermuscular). A DEXA, por sua vez, mede a gordura total com alta precisão e radiação relativamente baixa, mas não distingue entre a gordura intra-abdominal e subcutânea.54

O estabelecimento de valores de referência para o PC em crianças de acordo com idade e sexo específicos para a população em diversos países é de grande relevância, levando-se em conta as diferenças significativas verificadas nos diversos estudos em que os valores de PC foram comparados entre si.18 , 19 , 22 - 24 , 29

Atualmente não existe consenso em relação ao ponto anatômico onde o PC deve ser medido. Entretanto, numa análise de crianças de 6 a 9 anos, observou-se que o PC medido no ponto médio entre a crista ilíaca e última costela foi o que apresentou melhor correlação com o percentual de gordura corporal.27 Bosy-Westphal et al 28 observaram que, em crianças, os valores de PC diferiram significativamente de acordo com o local de medição, sendo encontrado perímetro menor quando a medida foi tomada abaixo da última costela, valor maior no ponto acima da crista ilíaca e, para o ponto médio entre esses locais, foi encontrado um valor intermediário. No estudo de Sant'Anna et al, 27 a medida realizada sobre a cicatriz umbilical foi estatisticamente maior entre meninas. Assim, a interpretação de diferenças no PC entre os estudos de diferentes populações deve ser realizada com cautela, considerando que a medida pode ter sido realizada em diferentes pontos anatômicos.

A RCE é uma medida de risco para a saúde mais simples do que outros índices antropométricos em crianças, como o IMC/idade, uma vez que não requer nenhum ajuste para idade ou sexo,43 tendo surgido como parâmetro de adiposidade central e preditor significativo de fatores de risco para doenças cardiovasculares em crianças e adolescentes. Ashwell e Hsieh6 propõem a adoção da RCE como uma ferramenta de triagem simples, sendo sua medida rápida e eficaz. Os autores sugerem que a RCE é mais sensível, mais barata e mais fácil de medir e calcular que o IMC. Além disso, o valor igual a 0,5 indicaria um risco aumentado para homens e mulheres de diferentes grupos étnicos, sendo aplicável a adultos e crianças. Todavia, em estudo realizado com 5.725 crianças e adolescentes noruegueses,48 o ponto de corte recomendado de 0,5 apresentou elevada sensibilidade e especificidade para detectar obesidade em indivíduos na faixa etária de 6 a 18 anos; porém, em crian ças mais jovens, esse ponto de corte não foi apropriado devido à baixa especificidade. Os autores sugerem ainda que, para o sobrepeso, os pontos de corte deveriam ser diferentes para crianças e adolescentes de 6 a 12 anos e de 12 a 18, e não deveriam ser definidos para o grupo etário mais jovem.48

Devido à correlação residual entre RCE e estatura em crianças, a divisão do PC pela estatura elevada à potência de 1 (cintura/estatura1) pode ser insuficiente para ajustar corretamente a estatura durante o crescimento.36 , 37 Tybor et al,36 avaliando uma amostra representativa de crianças e adolescentes de 2 a 18 anos do National Health and Nutrition Examination Survey (NHANES), de 1999-2004, estratificada por idade e sexo, encontraram correlação residual entre a estatura e a RCE entre 0,29 e 0,36. Por outro lado, o estudo de Taylor et al 37 mostrou que a simples divisão do PC pela estatura discrimina corretamente, em pelo menos 90% das vezes, crianças e adolescentes com altos e baixos níveis de gordura total e central. A validade da RCE pela fórmula cintura/estatura1 foi avaliada por Nambiar et al 38 em uma coorte de 3.597 crianças australianas com idades entre 5 e 17 anos. Os autores verificaram que a RCE pode ser usada na população do estudo, sendo mais apropriada que o IMC devido à sua capacidade para explicar a distribuição de gordura corporal e os riscos de saúde cardiovasculares associados. Esses achados indicam a necessidade de mais pesquisas para investigar o grau de dependência da RCE com a estatura e de que maneira isso influencia as relações entre adiposidade central e os fatores de risco para doenças cardiovasculares na faixa etária em questão. Além disso, é preciso avaliar se o uso de um expoente diferente de 1 poderia reduzir o viés e melhorar a precisão da medida.

Em relação ao PP, apesar da escassez de trabalhos na literatura utilizando essa medida, os resultados dos estudos referentes à sua utilização como parâmetro de avaliação da adiposidade central em crianças evidenciam que tal medida pode ser uma ferramenta de triagem útil para identificar o sobrepeso ou a obesidade. Pode ser útil também para diagnosticar crianças de risco para elevada adiposidade, um importante preditor de problemas de saúde cardiovascular, especialmente quando se dispõem de referências ajustadas para idade e sexo.2 , 51 As elevadas correlações encontradas entre o PP e o IMC podem indicar que o PP é um índice confiável para determinar a obesidade. Além disso, as correlações significativas entre esse parâmetro e outros indicadores de obesidade central refletem a semelhança entre eles. A baixa correlação entre o PP e o percentual de gordura corporal, por sua vez, pode significar que o PP é uma medida de acúmulo de gordura desproporcional, em vez de uma medida geral de obesidade.50 Um ponto forte para a utilização do PP é que ele apresenta uma boa confiabilidade intra e interexaminadores,53 não sendo necessário realizar múltiplas medidas para precisão e confiabilidade. Adicionalmente, comparado a outros indicadores de gordura corporal superior, a medida do PP é mais simples de realizar, facilitando a logística de sua obtenção.

O PP é um parâmetro novo que tem mostrado bons resultados na avaliação de crianças, podendo ser utilizado tanto na prática clínica quanto em estudos epidemiológicos como um marcador para a obesidade central. Atenção especial deve ser dada a esse parâmetro na população infantil, uma vez que resultados de pesquisas realizadas com crianças já têm demonstrado associação com fatores de risco cardiometábolicos.7 , 55 Entretanto, são necessários mais estudos para avaliar a utilidade do PP como um indicador de adiposidade, bem como é necessário estabelecer valores de referência para crianças numa faixa etária mais jovem.

Conclui-se que o PC foi o parâmetro mais utilizado nos estudos e tem demonstrado bom desempenho na avaliação da obesidade central, apesar de os resultados de alguns trabalhos serem controversos. A RCE tem sido proposta como um parâmetro útil para avaliar a distribuição de gordura em crianças, mas algumas questões ainda merecem ser investigadas, como sua correlação residual com a estatura durante o crescimento. O PP, apesar de ser uma medida mais recente e ainda pouco estudada, tem se mostrado satisfatório como parâmetro de avaliação da adiposidade central na população infantil. Os pontos de corte diversificados para os parâmetros estudados podem ser devidos a diferenças étnicas, bem como à falta de padronização do ponto anatômico utilizado na aferição de medidas como o PC. Assim, tornam-se necessários novos estudos que aprofundem a utilidade desses parâmetros na determinação da obesidade central na infância, incluindo-se a padronização do local de aferição das medidas para o estabelecimento de pontos de cortes que sejam comparáveis entre as diversas populações.
