Abstract
More than nine percent of the US population--or 29.1 million people--has diabetes. Of this group, more than eight million are undiagnosed, and one in three people who have been diagnosed also have kidney disease. Minority groups, including American Indians, Hispanics, and African Americans, continue to be the most at risk for diabetes and kidney disease. Community partnerships and physician education play major roles in awareness and education efforts. At DaVita Kidney Care, clinicians created the StepAhead program to address diabetes care management among its patients.Entities:
