Abstract
One innovative community-based setting to promote health is the faith community, where care is provided by a faith community nurse (FCN). This descriptive study describes the practice of FCNs, FCN functions and standards, identifies Healthy People 220 Leading Health Indicators being addressed by FCNs, and explores how the FCN model of community-based practice can support implementation of Healthy People 2020.Entities:
