Creating and validating an instrument to identify the workload at an oncology and hematology outpatient service.

OBJECTIVE: Construct and to validate an instrument for measuring the time spent by nursing staff in the interventions/activities in Outpatient Oncology and Hematology, interventions based on Nursing Interventions Classification (NIC), for key areas of Pediatric Oncology and Oncology Nursing.
METHODS: Cross-sectional study divided into two steps: (1) construction of an instrument to measure the interventions/Nursing activities and (2) validation of this instrument.
RESULTS: We selected 32 essential interventions from NIC for Pediatric Oncology and Oncology Nursing areas. The judges agreed with removing 13 and including 6 interventions in the instrument, beyond personal activity.
CONCLUSION: The choice of essential interventions from NIC is justified by the gain time on research.

INTRODUCTION

Identifying the workload is the key for determining the number of Nursing staff, according to the Canadian Nurses Association (CNA).( At outpatient services, emergency rooms and primary care units, for instance, the workload can be calculated as the product of time spent in interventions and Nursing activities and the number of patients who received those interventions/care.( At an outpatient unit, nursing staff workload is influenced by several factors, such as: patient characteristics (patient classification systems), characteristics of the role of nursing staff (competencies, skills and attitudes) and number of patients who require care. Due to frequent changes in schedules and patient flow, nursing staff workload at outpatient services is often less predictable.(

In order to determine the workload at inpatients units we need to classify patients according to their level of dependence on nursing staff. A Patient Classification System should be adopted (.

Over several decades nursing managers have developed a wide range of tools to measure workloads at inpatients units. However such tools are not easily adaptable to outpatient units. This shortage has been leading institutions to use some metrics, such as the number of physicians, clinical profile, number of scheduled patients and number of procedures to determine the appropriate number of staff. Nevertheless, those metrics do not reflect the number of nursing staff that is required to support nursing care practice.(

Brazilian researchers( have been using the taxonomy proposed by the Center for Nursing Classification and Clinical Effectiveness (CNC), entitled Nursing Interventions Classification (NIC) as a reference to identify nursing care interventions, with the purpose of determining nursing workload.(

The NIC organized essential interventions for 45 specialties, that is, those that refer to a limited set of core interventions and that define the nature of a specialty. Those are the most frequent, predominant or essential interventions for the role of a specialist nurse.(

Determining the appropriate number of nursing staff according to the patients’ needs in outpatient care is vital in order to provide safe care, for an optimal benefit-cost ratio and for the quality of the environment. Continuous assessment of the work environment and the implementation of consistent, reliable and valid measures is critical in order to anticipate and justify personnel needs.(

The workload is always the most important indicator when determining the number of nursing staff. It is regarded as a powerful management tool because it demonstrates how important it is to balance the number of staff both in quantitative and qualitative terms when providing care to health service users. The workload as an indicator supports administrative and policy decisions made by nurses. It also contributes effectively to the negotiations surrounding the number of nursing staff conducted with healthcare organization managers.( A previously validated instrument is required to identify this workload.

Our study began with the following question: which activities interfere with the workload of a nursing team in an outpatient unit? Based on this question, we developed the following hypothesis: essential nursing interventions in the field of Oncology listed in the NIC could provide a reference for creating an instrument to determine the workload within an Oncology and Hematology outpatient environment.

OBJECTIVE

To create and validate an instrument to measure time spent by the nursing team on interventions/activities at an Oncology and Hematology Outpatient Service, based on the interventions listed in the Nursing Interventions Classification (NIC) for the essential areas of Pediatric Oncology and Oncology Nursing.

METHODS

This cross-sectional study was conducted at the Escola de Enfermagem da Universidade de São Paulo, upon approval by the Research Ethics Committee under number 170,263/12. Research data were organized and processed in two stages.

First stage: creating the instrument

First we selected the interventions proposed by the NIC for essential Nursing areas, namely Pediatric Oncology and Oncology Nursing, which are considered representative of outpatient nursing practice in Oncology and Hematology.

The interventions we selected were listed in ascending order, according to domains, with their respective NIC code. This distribution aimed at optimizing the identification of interventions during the following stage, resulting in a workshop.

We created an instrument consisting of 32 interventions distributed across 6 domains out of 7 listed in the NIC: Physiological: Complex, Behavioral, Physiological: Basic, Health System, Safety and Family. There were no interventions within the Community domain.

In the instrument, researchers asked respondents to assess each activity in terms of the representativeness of care assistance and management practices followed by Nursing professionals at an Oncology and Hematology Outpatient Service. Furthermore, we tested the relevance of interventions, according to the NIC’s definition. We also investigated whether other interventions should be included or excluded. At the end of the instrument, we provided eight lines so that respondents could include any other intervention or activity.

Research details were shown to judges using PowerPoint slides ten minutes before the instrument’s assessment. At that point, we explained the goals of the workshop: to analyze and validate nursing interventions selected according to the NIC, in addition to answering any further questions.

There were two stages during the workshop. First participants were given a chance to reread the instrument and give their opinion on it. The second stage consisted of a consensus on answers.

Five days before the workshop, the instrument was sent (Chart 1) to the judges by email, together with a letter of instructions.

Second stage: validating the instrument

Validation assessment means identifying to what extent the instrument is appropriate to measure what it intends to measure.( In this research we used content validation, which involves judgment from experts with extensive professional experience regarding the elements contained in the instrument and who are critical of the representativeness of that which we intend to measure.(

We employed the Delphi method to analyze data. This consists of requesting, collecting, arranging and analyzing data regarding a particular phenomenon. Such data result from the opinions of experts in the proposed field of study. This method involves an interactive questionnaire that is submitted several times to the group. They answer the questionnaire, and the aim is to search for similar opinions on the result.(

The first stage in the Delphi method was selection of judges. We used the following criteria: judges had to be nurses or nursing technicians with at least five years’ experience at an Oncology Outpatient Service and/or with at least 5 years’ experience using the NIC; they also had to agree to take part in the workshop. All of them agreed to take part in this stage.

We proposed the best dates, times and locations for the group of judges. After the judges were chosen, we sent them information about the workshop meeting, which was expected to last 4 hours.

Interventions were deemed validated when judges reached an agreement rate ≥70%.(

Data were stored in a database created for this research using Microsoft Excel 2013.

Validated interventions were grouped into a single instrument containing interventions that contributed to the workload of the Nursing team in an Oncology and Hematology Outpatient Service.

RESULTS

We selected essential NIC interventions for the following specialties: Pediatric Oncology and Oncology Nursing. This selection resulted in 32 interventions, with their respective codes and definitions, as shown in chart 2.

Interventions are distributed across six out of seven NIC domains (Figure 1).

The Workshop lasted four hours and was held at the Escola de Enfermagem da USP. During the workshop, each intervention was assessed in terms of the representativeness of interventions made at the Oncology and Hematology Outpatient Service, and of the relevance of interventions, according to the NIC’s definition. Furthermore, we investigated whether there were any interventions that should be included or excluded.

The workshop was led by the researcher. Six judges participated and are characterized in table 1.

NIC: Nursing Interventions Classification.

With respect to all 32 Nursing interventions we selected, 100% of the judges agreed to exclude 13 of them and to include another 6. They also agreed on grouping and maintaining the other interventions, as well as including personal activity. This resulted in 25 interventions and 1 personal activity.

From all 25 validated interventions, 84% were part of essential interventions for specialties areas in Pediatric Oncology and Oncology Nursing.

Based on the results of validated interventions and activities, we developed an instrument to track the time spent by the Nursing team of an Oncology and Hematology Outpatient Service performing their nursing activities/interventions. In order to optimize data collection, interventions were listed alphabetically, with their respective NIC codes, and numbered in ascending order. Work days were divided into columns of 10-minute intervals in order to record the intervention/activity performed (Figure 2).

DISCUSSION

Our selection based on essential NIC interventions for Nursing in Pediatric Oncology and Oncology Nursing, which we used as a starting point for creating the instrument, resulted in 84% of interventions being validated at the workshop. This confirms the hypothesis that essential interventions for specialties provide reference for research. In addition, they facilitate the identification of interventions that have an impact on Nursing teams’ workload and on reducing time spent by researchers when selecting interventions.

The workshop method was also used in two other instances:( to validate nurse activities at a chemotherapy center and to map those activities in Nursing interventions, according to the NIC.

The use of NIC interventions as a reference in Oncology Outpatient Service research papers has been adopted by several authors( and in studies conducted in other areas in order to measure the workload of Nursing teams.(

In an outpatient setting, time is crucial when providing safe care. The study of interventions is beneficial because it reflects all activities. Therefore, the choice for essential interventions for Nursing specialties in Pediatric Oncology and Oncology Nursing, under the Nursing Interventions Classification (NIC), and not for activities, is justified by the time saved on research.

De Souza et al.( initiated their research based on the activities conducted by nurses at a Chemotherapy Outpatient Service in a hospital specializing in Oncology. They used semi-structured interviews, document analysis and a questionnaire. After that they mapped activities into interventions, according to the NIC.

Interventions mapped in this study resulted in 32 interventions distributed across 6 domains: Physiological: Complex, Physiological: Basic, Behavioral, Health System, Family and Safety. They did not use personal activities because they believed such activities could hinder comparisons with other studies, since activities may vary between different institutions. However, during the workshop judges suggested including personal activities, without specifying what type.

Other researchers( have achieved different results. At a chemotherapy center, 35 interventions and 48 activities were identified. Personal activities were among them, described as meal or restroom breaks. Interventions were organized into five domains and unlike our study they did not include the family domain. Other researchers( from a diagnostic imaging center identified 32 interventions and 92 activities.

During the workshop, participants realized the importance of balancing knowledge from the NIC and practical experience. This was also noted by other authors,( who identified the advantages of this method, since it provides a critical analysis for improving the proposed instrument.

The 25 interventions and 1 activity from the workshop validation created an instrument to measure workload. Nursing interventions are listed alphabetically with their respective NIC codes. Work days were divided into columns in 10-minute intervals. We included fields for up to six observers. This same formatting has been used( to propose an instrument to measure the working hours of Nursing professionals at a diagnostic imaging center. However, there were no fields in the instrument for six simultaneous observers, only space reserved for two of them.

CONCLUSION

It is clear that using essential interventions for Nursing specialties in Pediatric Oncology and Oncology Nursing as a reference optimized the time spent on research. However, other specialties should also be assessed and possibly included in the instrument, such as Outpatient Care Nursing, Palliative Care Nursing, Occupational Health Nursing and Emergency Nursing.

The instrument we developed was validated in a workshop, during which the instrument was considered appropriate to measure the workload of the Nursing team in an Oncology and Hematology Outpatient Service. However, it has some limitations. It needs to be applied in nursing care practice so that its reliability can be assessed and so that its degree of precision can be established. In order words, we still need to find out to what extent the proposed instrument reflects data identified in it.

INTRODUÇÃO

A identificação da carga de trabalho é a chave para a determinação do quadro de profissionais de Enfermagem, segundo a Canadian Nurses Association (CNA).( Em ambulatórios, prontos-socorros e Unidades Básica de Saúde, por exemplo, o cálculo da carga de trabalho pode ser obtido pelo produto do tempo médio despendido na realização das intervenções e nas atividades de Enfermagem pela quantidade de pacientes assistidos naquela intervenção/atividade.( Em uma unidade ambulatorial, a carga de trabalho de Enfermagem é influenciada por diversos fatores, como: características do paciente (sistemas de classificação de pacientes), características do papel da Enfermagem (competências, habilidades e atitudes) e número de pacientes que requerem cuidados. Devido às frequentes alterações de programação e de fluxo de pacientes, a carga de trabalho da Enfermagem ambulatorial é, muitas vezes, menos previsível.(

Para conhecer a carga de trabalho em unidades de internação, é necessário classificar os pacientes de acordo com o grau de dependência da equipe de Enfermagem, devendo-se adotar um Sistema de Classificação do Paciente (

Por várias décadas, gestores de Enfermagem desenvolveram uma grande variedade de ferramentas de medição de carga de trabalho em unidades de internação. Entretanto, tais ferramentas não são facilmente adaptáveis às unidades ambulatoriais. Essa escassez tem levado as instituições a utilizarem métricas, como número de médicos, quadro clínico, número de pacientes agendados e volumes de procedimentos, para determinar o número adequado de pessoal. Porém, essas métricas não refletem o número de pessoal de Enfermagem necessário para apoiar a prática assistencial.(

Para identificar as intervenções de cuidado desenvolvidas pela Enfermagem, com o objetivo de conhecer a carga de trabalho, pesquisadores brasileiros( têm utilizado como referencial a taxonomia proposta pelo Center for Nursing Classification and Clinical Effectiveness (CNC), denominada Classificação das Intervenções de Enfermagem (NIC - Nursing Interventions Classification).(

A NIC organizou as intervenções essenciais para 45 áreas de especialidades, ou seja, aquelas que se referem a um conjunto limitado de intervenções centrais e que definem a natureza de uma especialidade. São estas as intervenções de maior frequência, predominantes, ou as essenciais para o papel do enfermeiro especialista.(

Dimensionar a equipe de Enfermagem de forma adequada às necessidades dos pacientes assistidos em ambulatórios é fundamental para a prestação de cuidados seguros, para um bom custo-benefício e para a qualidade do ambiente. A avaliação contínua do ambiente de trabalho e o estabelecimento de medidas consistentes, confiáveis e válidas são fundamentais para prever e justificar as necessidades de pessoal.(

A carga de trabalho é sempre o indicador mais importante na realização do dimensionamento de pessoal. Ela é considerada um poderoso instrumento de gestão, por ser capaz de demonstrar a importância da adequação quantitativa e qualitativa do quadro de pessoal na prestação de cuidados aos usuários dos serviços de saúde. A carga de trabalho, como indicador, subsidia as decisões administrativas e as políticas dos enfermeiros, além de contribuir efetivamente para a negociação do quadro de profissionais com os administradores das organizações de saúde.( Para identificá-la, é necessário um instrumento que tenha sido anteriormente validado.

Este estudo partiu do questionamento: quais as atividades que interferem na carga de trabalho da equipe de Enfermagem de uma unidade ambulatorial? Com base nesse questionamento, foi formulada a seguinte hipótese: as intervenções de Enfermagem essenciais para a área de Oncologia apresentadas na NIC podem ser o referencial para construção de um instrumento que possa identificar a carga de trabalho em um ambiente ambulatorial de Oncologia e Hematologia.

OBJETIVO

Construir e validar um instrumento para medida de tempo despendido pela equipe de enfermagem nas intervenções/atividades do Ambulatório de Oncologia e Hematologia, com base nas intervenções da Classificação das Intervenções de Enfermagem (NIC - Nursing Interventions Classification), para áreas essenciais de Oncologia Pediátrica e Enfermagem Oncológica.

MÉTODOS

Trata-se de um estudo transversal ocorrido nas dependências da Escola de Enfermagem da Universidade de São Paulo, após a aprovação pelo Comitê de Ética em Pesquisa, sob número 170.263/12. Os dados da pesquisa foram organizados e processados em duas etapas.

Primeira etapa: construção do instrumento

Foram selecionadas as intervenções propostas pela NIC para áreas essenciais de Enfermagem, a saber Oncologia Pediátrica e Enfermagem Oncológica, consideradas representativas da prática de Enfermagem ambulatorial de Oncologia e Hematologia.

As intervenções selecionadas foram listadas em ordem crescente, por domínios, com a identificação do código NIC. Essa distribuição teve o objetivo de otimizar a identificação das intervenções durante próxima etapa, que consistiu na realização de uma oficina.

Foi criado um instrumento constituído por 32 intervenções, distribuídas em 6 domínios dos 7 apresentados pela NIC: Fisiológico complexo, Comportamental, Fisiológico básico, Sistema de saúde, Segurança e Família. Não houve intervenção referente ao domínio Comunidade.

No instrumento, a pesquisadora solicitou a avaliação de cada atividade quanto à representatividade das práticas de cuidado assistenciais e gerenciais desenvolvidas pelos profissionais de Enfermagem no Ambulatório de Oncologia e Hematologia. Além disso, testou-se a pertinência da intervenção, segundo a definição da NIC. Verificou-se também a necessidade de inclusão ou exclusão de outras intervenções. Ao final do instrumento, foram disponibilizadas oito linhas para possível inclusão de qualquer intervenção ou atividade.

O detalhamento da pesquisa foi apresentado aos juízes em PowerPoint, 10 minutos antes da avaliação do instrumento. Nesse momento, houve um alinhamento no objetivo da oficina que foi analisar e validar as intervenções de enfermagem selecionadas segundo a NIC, além de esclarecimentos de dúvidas.

Durante a oficina, ocorreram duas etapas. A primeira foi a releitura e a manifestação dos participantes quanto à sua opinião do instrumento. A segunda etapa foi constituída pelo consenso das respostas.

Cinco dias antes da oficina, o instrumento foi enviado (Quadro 1) por e-mail aos juízes, junto de carta de orientação.

Segunda etapa: validação do instrumento

A avaliação da validade é a identificação do grau em que o instrumento se mostra apropriado para mensurar o que supostamente deve medir.( Nesta pesquisa, foi utilizada a validade de conteúdo, na qual há o envolvimento do julgamento por expertises com vasta experiência profissional em relação aos elementos contidos no instrumento e críticos quanto representatividade do que se pretende medir.(

Para análise dos dados foi aplicada a técnica Delphi, que consiste em solicitar, coletar, tabular e analisar os dados referente a um fenômeno, resultante da opinião de especialistas na área do estudo proposto. Tal técnica prevê a aplicação de questionário interativo, que é apresentado por diversas vezes ao grupo, que o responde, em busca de opiniões similares sobre o resultado.(

A primeira etapa da técnica Delphi foi a seleção dos juízes. Os critérios utilizados foram: ser enfermeiro ou técnico de Enfermagem com experiência mínima de 5 anos em Ambulatório de Oncologia e/ou com experiência mínima de 5 anos na utilização da NIC, e consentir em participar da oficina. Todos consentiram em participar dessa etapa.

Datas, horários previstos e local que melhor atendesse o agrupamento dos juízes foram propostos. Após a seleção, foram enviadas as informações para o encontro na oficina, com previsão de duração de 4 horas.

As intervenções foram consideradas validadas quando os juízes apresentaram um índice de concordância ≥70%.(

Os dados foram armazenados em um banco de dados elaborado para a pesquisa em Microsoft Excel 2013.

As intervenções validadas foram agrupadas em um instrumento contendo as intervenções que contribuíam para a carga de trabalho da equipe de Enfermagem do Ambulatório de Oncologia e Hematologia.

RESULTADOS

Foram selecionadas da NIC as intervenções essenciais para as áreas de especialidades: Oncologia Pediátrica e Enfermagem Oncológica. Essa seleção resultou em 32 intervenções, com seus respectivos códigos e definições, conforme demonstrado no quadro 2.

As intervenções estão distribuídas em seis dos sete domínios apresentados pela NIC (Figura 1).

A Oficina ocorreu em quatro horas, nas dependências da Escola de Enfermagem da USP. Durante a oficina, cada intervenção foi avaliada em relação à representatividade das intervenções desenvolvidas em Ambulatório de Oncologia e Hematologia, e à pertinência da intervenção, segundo a definição da NIC. Além disso, foi verificada a necessidade de incluir ou excluir intervenções.

A oficina foi coordenada pela pesquisadora, composta por seis juízes, caracterizados na tabela 1.

NIC: Classificação das Intervenções de Enfermagem.

Das 32 intervenções de Enfermagem selecionadas, houve concordância de 100% dos juízes sobre excluir 13, inserir 6, e agrupar e manter as demais intervenções, além de incluir a atividade pessoal, resultando em 25 intervenções e 1 atividade pessoal.

Entre as 25 intervenções validadas, 84% faziam parte das intervenções essenciais para as áreas de especialidades em Oncologia Pediátrica e Enfermagem Oncológica.

Com o resultado das intervenções e atividades validadas, foi desenvolvido um instrumento para coleta dos tempos despendidos pela equipe de Enfermagem do Ambulatório de Oncologia e Hematologia, no desempenho das atividades/intervenções de Enfermagem. Para otimizar a coleta, as intervenções foram listadas em ordem alfabética, com identificação do código NIC, e numeradas em ordem crescente. A jornada de trabalho foi dividida em colunas, em intervalos de 10 minutos, para o registro da intervenção/atividade realizada (Figura 2).

DISCUSSÃO

A seleção a partir das intervenções essenciais para Enfermagem em Oncologia Pediátrica e Enfermagem Oncológica da NIC, como ponto de partida na construção do instrumento, resultou em 84% das intervenções validadas na oficina, o que confirmou a hipótese de que as intervenções essenciais para especialidade constituem um referencial para pesquisa, além de facilitarem a identificação das intervenções que impactam na carga de trabalho da equipe de Enfermagem e na redução do tempo do pesquisador na seleção das intervenções.

A técnica de oficina também foi utilizada em dois momentos:( para validar as atividades dos enfermeiros em um centro de quimioterapia e para seu mapeamento em intervenções de Enfermagem, segundo a NIC.

O uso das intervenções da NIC como referencial em trabalhos de Ambulatório de Oncologia foi adotado por vários autores( e em estudos realizados em outras áreas, com o objetivo de mensurar a carga de trabalho da Enfermagem.(

Em ambulatório, tempo é fator crucial para o desempenho da assistência prestada com garantia de segurança. O estudo das intervenções resulta em ganho, por retratar todas as atividades. Dessa forma, a escolha por intervenções essenciais, para as áreas de especialidades de Enfermagem em Oncologia Pediátrica e Enfermagem Oncológica, da Classificação das Intervenções de Enfermagem (NIC), e não de atividades, é justificada para ganho de tempo em pesquisa.

De Souza et al.( iniciaram sua pesquisa a partir das atividades realizadas por enfermeiros em Ambulatório de Quimioterapia de um hospital especializado em Oncologia. Utilizaram como métodos: a entrevista semiestruturada, a análise de documentos e o questionário. Na sequência, foram mapeadas as atividades em intervenções, segundo a NIC.

As intervenções mapeadas neste estudo resultaram em 32 intervenções distribuídas e 6 domínios: Fisiológico complexo, Fisiológico básico, Comportamental, Sistema de saúde, Família e Segurança. A não utilização das atividades pessoais deveu-se ao fato de se considerar que as mesmas podem dificultar a comparação com outros estudos, já que podem variar entre instituições; entretanto, na oficina, foi sugerida a inserção da atividade pessoal, sem descrever o tipo.

Outros pesquisadores( diferem em seus resultados. Em centro de quimioterapia, foram identificadas 35 intervenções e 48 atividades; dentre elas, estava a atividade pessoal, descrita como pausas para alimentação e atendimento às necessidades fisiológicas. As intervenções foram organizadas em cinco domínios e, ao contrário deste estudo, não foi identificado o domínio família. Já pesquisadores( em centro de diagnóstico por imagem identificaram 32 intervenções e 92 atividades.

Durante a oficina, foi percebida a importância do equilíbrio da associação do conhecimento da NIC com a vivência prática. Esse fato também foi observado por outras autoras,( que também identificaram a vantagem dessa técnica ao proporcionar uma análise crítica para o aperfeiçoamento do instrumento proposto.

As 25 intervenções e 1 atividade resultantes da validação da oficina compuseram um instrumento de medida de carga de trabalho, sendo as intervenções de Enfermagem listadas em ordem alfabética com seus respectivos códigos NIC. A jornada de trabalho foi dividida em colunas e intervalos de 10 minutos. Foram inseridos campos para colocação de até seis observadores. Essa mesma formatação já foi utilizada( para propor um instrumento de medida da distribuição de tempo de trabalho dos profissionais de Enfermagem em centro de diagnóstico por imagem, porém não foi disponibilizado campo no instrumento para seis observadores simultâneos, reservando espaço para apenas dois deles.

CONCLUSÃO

Ficou evidenciado que a utilização do referencial, a partir das intervenções essenciais para as áreas de especialidades de Enfermagem em Oncologia Pediátrica e Enfermagem Oncológica, otimizou o tempo da pesquisa. Entretanto, outras áreas de especialidades devem ser avaliadas como possibilidades de inserção na construção do instrumento, tais como Enfermagem Ambulatorial, Enfermagem de Cuidados Paliativos, Enfermagem em Saúde Ocupacional e Enfermagem na Emergência.

O instrumento desenvolvido foi validado em uma oficina, na qual demonstrou-se que ele era apropriado para mensurar a carga de trabalho da equipe de Enfermagem de Ambulatório de Oncologia e Hematologia. Entretanto, ele apresentou limitação, pois necessita ser aplicado na prática assistencial para avaliação de sua confiabilidade e, assim, definição do grau de precisão, ou seja, do quanto consegue refletir os dados identificados no instrumento proposto.
