Sir,The recent report on “vacuum-assisted closure (VAC) and moist wound dressing in diabetic foot is very interesting.[1]” Ravari et al. concluded that “VAC appears to be as safe as and more efficacious than moist dressing for the treatment of diabetic foot ulcers”.[1] It is no doubt that VAC can be useful for diabetic foot care, however, the other side of the coin must be mentioned. The complications of VAC have to be considered. “Retention of polyurethane foam fragments” is possible and it can be serious if it is under diagnosed.[2] Finally, it should be noted that although there are many reports mentioning the advantage of VAC, the meta-analysis also showed the weakness of those reports (inclusion criteria of subjects, operative definition for outcomes, etc.) and further studies are required for finalised conclusion.[3]
