Analysis models for variables associated with breastfeeding duration.

OBJECTIVE To analyze the factors associated with breastfeeding duration by two statistical models. METHODS A population-based cohort study was conducted with 86 mothers and newborns from two areas primary covered by the National Health System, with high rates of infant mortality in Vitória, Espírito Santo, Brazil. During 30 months, 67 (78%) children and mothers were visited seven times at home by trained interviewers, who filled out survey forms. Data on food and sucking habits, socioeconomic and maternal characteristics were collected. Variables were analyzed by Cox regression models, considering duration of breastfeeding as the dependent variable, and logistic regression (dependent variables, was the presence of a breastfeeding child in different post-natal ages). RESULTS In the logistic regression model, the pacifier sucking (adjusted Odds Ratio: 3.4; 95%CI 1.2-9.55) and bottle feeding (adjusted Odds Ratio: 4.4; 95%CI 1.6-12.1) increased the chance of weaning a child before one year of age. Variables associated to breastfeeding duration in the Cox regression model were: pacifier sucking (adjusted Hazard Ratio 2.0; 95%CI 1.2-3.3) and bottle feeding (adjusted Hazard Ratio 2.0; 95%CI 1.2-3.5). However, protective factors (maternal age and family income) differed between both models. CONCLUSIONS Risk and protective factors associated with cessation of breastfeeding may be analyzed by different models of statistical regression. Cox Regression Models are adequate to analyze such factors in longitudinal studies.

Introduction

When breastfeeding duration had its importance recognized by the World Health Organization (WHO)( ), studies on the factors associated with exclusive and total breastfeeding duration has intensified in the last years. The knowledge on the effects of breastfeeding on childrenâ€(tm)s health is based on scientific evidence showing its capacity to prevent morbidity and mortality from diarrhea in the first two years of life( ). Additionally, throughout life, breastfeeding duration is associated with lower levels of cholesterol and cardiovascular diseases in adulthood( ). Given the relevance of breastfeeding for the conjunctural development of the human being, both in childhood and in adulthood, empirical studies have been conducted to explain the possible risk and protective factors related to the duration of this practice( ).

Studies with cohort( ) and sectional( - ) epidemiological design, as well as clinical trials( ), have been useful to measure what determines early weaning. In these investigations, statistical analyses have varied between and within the types of study, since the techniques include univariate, bivariate and multivariate analyses( ). Logistic regression models (LRMs) and Cox regression models (CRMs) have been used to adjust results for variables and consider the influence of confounding factors.

LRMs may be used considering total breastfeeding duration in a fragmented way, i.e., by dividing breastfeeding duration into fixed intervals of childrenâ€(tm)s life( , ). In this sense, these models are limited, because they analyze longitudinal information as if it was cross-sectional, although being useful when one analyzes the fact that a child is breastfed at a specific lifetime. In CMRs, breastfeeding duration is related to childâ€(tm)s survival until weaning( , ), considered month to month. These models are appropriate for the analysis of longitudinal data and should not be used in the analysis of cross-sectional data, when the history of the outcome (weaning) is not known. However, it is suggested that these analysis techniques may differ among themselves with regard to the determination of risk and protective factors of breastfeeding.

Therefore, the aim of this study was to analyze the factors associated with breastfeeding duration by two regression models in a cohort of mothers and children that were followed for 30 months in Vitória, Espírito Santo, Brazil.

Method

A cohort study was conducted with 86 mothers and newborns, during 30 months, including all babies from zero to three months living in two areas covered by the Health Family Program from the National Health System in Vitória. These two areas were chosen because, in 2001, they had indicators of infant mortality higher than those of the city rate (13.49/1,000 live births)( ).

Sample size was calculated in order to estimate a proportion, considering the data provided by the Brazilian Ministry of Health regarding the size of the population of live births in 2001 in the city of Vitória (4,521 children) and the prevalence of total breastfeeding in the age from 151 to 180 days for the year 1999 (74.8%), considering an accuracy of 10% and a significance level of 5%. Sample size was estimated in 73 children. Therefore, the sample of 86 children allowed for a 18% safety margin to account for possible losses to follow-up.

The period for inclusion in the study group lasted from November 2003 to May 2004, moment when health community workers referred the babies at the predetermined age to four dental students at Universidade Federal do Espírito Santo who, after being trained and divided into pairs, observed the follow-up home visits made to the study subjects. All mothers and children approached were included in the study, since there were no impeditive factors justifying their exclusion. This same team followed the participants for 18 months; then, another team of similar composition completed the follow-up, after specific training.

Mothers were invited to participate in the research and received information on the nature of the study and the frequency of the consecutive home visits. Without exception, all visited mothers spontaneously accepted to participate, signed a free and informed consent, and were interviewed to fill out a form.

Visits took place according to a schedule (initially every three months, later every six months) and served as observational control of the first visit. At each visit, a new form was filled out based on observation and on a structured interview applied to mothers.

Data collection by home visits finished in June 2006, amounting to seven home visits. The study period covered the breastfeeding duration recommended by the WHO, since the children were, on average, 29.4±2.5 months old at the end of the study.

Breastfeeding was considered according to the definitions of breastfeeding categories recommended by OMS( , ). For the accurate definition of this variable, at each visit mothers were asked whether they breastfed their child or used feeding bottles or other artificial nipples for feeding (such as chuquinha).

The variables on nutritive and non-nutritive sucking habits were measured by the duration of finger sucking, duration of pacifier sucking, and duration of bottle feeding. These variables were defined by asking at each visit whether the child had the habit. If yes, records were made of when it started and, in case the habit had been discontinued, when it had stopped. Then, duration was calculated.

At each visit, the mother was asked about the introduction of any food of any nature in the childâ€(tm)s diet: smashed fruits, vegetables, or even industrialized foods. If yes, records were made of the month in which the child had started this eating habit and literally what food had been used during the previous 24 hours.

Socioeconomic variables were collected at the first home visit. The following data were recorded: maternal education level, family income in minimum wages, paternal occupation, maternal occupation, maternal age, marital status, marital stability, number of people living in the same household, finishing of the house, number of rooms, number of children below five years of age, number of live births of the mother, number of siblings of the child (considering the same mother), and interval between pregnancies.

Statistical analyses were performed after reviewing and adapting the database applied in the Statistical Package for the Social Sciences (SPSS) for Windows software, version 12.0, selecting the variables of interest. The significance level was set at 5% for all the tests.

LRMs were built from the analyses, considering all study variables and the dichotomized breastfeeding variables, and calculating the association with the Yates chi-square test. In LRM (1), exclusive breastfeeding until three months was used as dependent variable; in LRM (2), breastfeeding until six months; in LRM (3), breastfeeding until 12 months. Independent variables were those with a p-value lower than or equal to 0.10 at the initial analysis. For independent variables with strong association between themselves (e.g., pacifier use until the first month, until the third month, and until the first year), only one of them was used in the model, i.e., that with the earlier onset of the risk factor and higher unadjusted Odds Ratio with regard to the dependent variable. The unconditional Stepwise forward logistic regression model was applied for the automatic selection of variables, which tests the entry of variables with statistical significance by a score and remove them using the likelihood ratio, based on maximum partial likelihood estimates. Significance level was set at 0.10 for the entry and 0.20 for the exit of variables in the model.

CRMs were built from the bivariate analysis of survival, whose variable of interest was breastfeeding duration (in months) and the independent variables were the remaining study variables. Kaplan-Meier survival curve was calculated from the maximum duration of breastfeeding, and the log-rank test was used to compare the curves according to the subgroups. Then, CRMs were obtained by including variables with a p-value lower than or equal to 0.10 in the log-rank test. For independent variables with strong association between themselves (e.g., pacifier use until the first month, until the third month, and until the first year), only one of them was used in the model. In CRM (1), bottle feeding and pacifier sucking occurred earlier; in CRM (2), it was possible to find variables with higher association strength in the log-rank test; in CRM (3), bottle feeding and pacifier use occurred later.

The research protocol was submitted to the Research Ethics Committee of the Biomedical Center at Universidade Federal do Espírito Santo and approved in the 43rd ordinary meeting of 25th June, 2003, under registration no. 20/2003.

Results

At the initial visit, 86 children (100%) were analyzed, with a mean age ranging from 1.58±1.05 months. At the second visit, 81 (94.1%) were approached, with a mean age of 4.89±1.55 months. At the third and forth visits, participants amounted to 79 (91.8%) children, with means ages ranging from 7.39±1.56 and 11.61±2.16 months, respectively. At the fifth visit, 76 (88.4%) children were found, and the group had a mean age of 16.39±2.88 months, while, at the sixth visit, 66 (76.7%) children participated, and mean age was 21.92±3.03 months. At the seventh visit, 67 (77.9%) children participated, since one child returned to household, yielding a mean age of 29.42±2.49 months. Losses to follow-up were motivated by change of address and by the failure to locate the household after at least three attempts. Mean for total breastfeeding duration was 15 months (95%CI 13-18), and median was also 15 months (95%CI 10-20).

LRM analyses, shown in Table 1, allowed to detect the risk factors associated with cessation of breastfeeding, adjusted for all statistically significant factors, evidencing the interaction of these factors. The onset of pacifier use until the first month represented a risk factor in all breastfeeding patterns analyzed. Bottle feeding until the first month of life also represented a risk factor for cessation of breastfeeding in LRMs (2) and (3).

Regarding bottle use, which is generally associated with bottle feeding, the choice for the use of only one of these variables allowed to represent nutritive sucking habits without being detrimental to the analysis model. The variable use of other artificial nipples before six months remained significantly associated only with cessation of breastfeeding, while teeth eruption before six months posed risk for cessation of breastfeeding only at the sixth month of life. The only protective factor was maternal age equal to or higher than 35 years.

In Table 2, regardless of the fact that the onset of bottle feeding or bottle use was early or late, all variables had significance values lower than 5%. On the other hand, variables related to pacifier sucking showed higher association strength as the habit was established later.

In Table 3, as for CRM (1), the onset of bottle feeding until the first month of life was the only variable that remained significant as a risk factor for cessation of breastfeeding; on the other hand, lower family income remained as a protective factor. In CRM (2), the onset of bottle feeding until the first year and the onset of pacifier sucking remained as risk factors associated with cessation of breastfeeding, whereas lower family income showed to be a protective factor. In CRM (3), the onset of pacifier use until the sixth month of life was the only variable that was consolidated as a risk factor for reduction in breastfeeding duration. Income lower than or equal to two minimum wages remained as a protective factor for breastfeeding maintenance.

Figure 1 shows protective and risk factors and confidence intervals associated with breastfeeding duration, considering adjusted Hazard Ratio and Odds Ratio values. Only protective factors differed between models. Age equal to or higher than 35 years appeared only in LRM (3), whereas monthly family income was found only in CRMs.

Discussion

In cohort studies, in which data collection took place at determined time points, logistic regression analyses are performed by dividing breastfeeding durations into intervals that are processed separately, as if each time point was an individual cross-sectional study. In CRMs, analyses considered total breastfeeding duration, with the answer being the time until the occurrence of the event (cessation of breastfeeding), adjusted for co-variables. In both models, the purpose is to investigate the combined effect of independent variables, to neutralize confounding effects, and to adjust the final results for Odds Ratio and Hazard Ratio.

The results for LRM (1) with regard to exclusive breastfeeding, at the first three months of life, reinforce that pacifier use and the introduction of other artificial nipples until the first month of life are crucial to increase the risk for cessation of breastfeeding. A study by Mascarenhas et al ), involving 940 mothers and babies from one to three months of life, showed that pacifier use poses risk for exclusive breastfeeding. On the other hand, a study by Santiago et al ), with 101 dyads, followed for four months, found that the non-introduction of pacifier in this period was a protective factor for exclusive breastfeeding. Regarding the use of other artificial nipples until the first month of life, Wayland( ) describes it as a common practice, especially for the introduction of water, tea and juice in childrenâ€(tm)s diet. However, this alone would not characterize non-exclusive breastfeeding( ). Certainly, mothers who reported to make use of these utensils use feed their babies with their own breast milk. However, in possession of these objects, mothers possibly tend to include in the diet foods other than breast milk.

Teeth eruption before the sixth month was associated with cessation of breastfeeding before six months of childrenâ€(tm)s life in LRM (2). It is assumed that the eruption of the first teeth, usually lower central incisors, may represent, for the mother, a risk of getting hurt in their breast nipple during breastfeeding, hampering its maintenance for a prolonged time.

In turn, maternal age higher than or equal to 35 years showed to be a protective factor for breastfeeding for 12 months or more in LRM (3). A study by Oliveira et al ), using the age 20-34 years as a reference, after adjustment to the analysis model, showed that age below 20 years was the only variable that showed higher risk for cessation of breastfeeding, while age higher than 34 years were not a risk neither a protective factor in babies below 24 months.

In both regression models used, pacifier use, regardless of the period of onset, showed to be an important predictor of breastfeeding duration, except for CRM (1), in which the onset of bottle feeding until the first month annulled the influence of pacifier sucking.

In LRMs (2) and (3), the chances of cessation of breastfeeding were higher at the sixth and the 12th month. In line with this finding, Audi et al ) reported higher chances of abandoning breastfeeding in children who use pacifiers. In CRMs (2) and (3), children who sucked pacifiers showed lower, but significant, risk for cessation of breastfeeding. These findings were similar to some studies(17,18) and very lower than those observed by Minagawa et al ) and Silveira and Lamounier( ), showing that, at a higher or lower intensity, pacifier use may be an important factor for cessation of breastfeeding.

Bottle feeding showed to be a risk factor for cessation of breastfeeding in all models, except in CRM (3). It should be considered that the variables bottle feeding and bottle use are very associated; therefore, it was chosen to exclude one of these variables in regression models. Howard et al ) showed that there are differences between the use of glasses and bottles for bottle feeding in breastfeeding duration and that bottle use increases the risk for cessation of breastfeeding, regardless of the type of milk used - of animal origin or industrialized.

In CRMs, income lower to or equal than two minimum wages was an important protective factor for prolonged breastfeeding, which did not occur in LRMs. In LRMs presented by Oliveira et al ), populations classified with a low living condition index in Salvador, state of Bahia, Brazil, had higher risk for cessation of breastfeeding than those classified with a high index. Victora et al ) analyzed the behavior of this variable using survival tables de in three birth cohorts and showed that, from six to nine months, breastfeeding is more prevalent in families of higher income, but, after 12 months of life, the prevalence of breastfeeding is significantly higher in poorer families.

Based on these observations, it can be concluded that differences in the socioeconomic profile may directly interfere with breastfeeding duration. However, one should consider that the study population does not show great differences regarding social status, because study subjects live in socially-deprived areas. Therefore, socioeconomic variables were profile descriptors and control markers. Even though, the small differences in family income influenced breastfeeding duration in CRMs.

It should be highlighted that the follow-up method by period home visits could not establish a strict pattern of time between data collection for all children, because the migratory movement of the families was intense. This is also due to the fact that sample loss was higher than initially planned, although it was not significantly detrimental to the reliability of the results.

The study on risk factors associated with breastfeeding duration is indispensible for the planning and implementation of activities that eliminate their effects on breastfeeding practice. Additionally, the use of different statistical analysis METHODS may lead to more detailed results, with the visualization of peculiarities from different angles, revealing specific results that, although are not identical, are not contradictory either. This possibly occurs because LRMs analyzed dichotomously the fact that the child is being breastfed or not at the first, sixth or 12th month. At these time points, it is understood that contextual factors compel to childrenâ€(tm)s weaning. In turn, in CRMs, the factors that boosted cessation of breastfeeding are analyzed as if they were present all the time during childâ€(tm)s life until weaning.

This study also suggests that it is necessary to fight against the use of pacifiers and early bottle feeding, in order to favor the practice of breastfeeding for an appropriate period of time, enabling the comprehensive promotion of childrenâ€(tm)s health since their first years of life.

Introdução

Quando a duração do aleitamento materno teve sua importância reconhecida pela Organização Mundial de Saúde (OMS)( ), os estudos sobre fatores associados ao tempo de aleitamento exclusivo e total intensificaram-se. O conhecimento sobre os efeitos do aleitamento materno na saúde da criança é fundamentado em evidências científicas que demonstram sua propriedade de prevenir a morbidade e a mortalidade por diarreia nos primeiros dois anos de vida( ). Além disso, ao longo da vida, a duração do aleitamento materno está associada a menores níveis de colesterol e doenças cardiovasculares na fase adulta( ). Diante da relevância do aleitamento materno para o desenvolvimento do ser humano, tanto na infância quanto na fase adulta, estudos empíricos têm sido realizados para esclarecer os possíveis fatores de risco e de proteção relacionados à duração dessa prática( ).

Os desenhos de estudos epidemiológicos de coortes( ), seccionais( - ), além dos ensaios clínicos( ), têm sido úteis para mensurar o que determina o desmame precoce. Nas pesquisas, as análises estatísticas têm variado entre e intra os tipos de estudo, visto que as técnicas incluem análises univariadas, bivariadas e multivariadas( ). Os modelos de regressão logística (MRL) e de regressão de Cox (MRC) têm sido utilizados para ajustar resultados por variáveis e considerar a influência dos fatores de confusão.

Os MRL podem ser utilizados considerando-se o tempo total de aleitamento materno de modo fragmentado, ou seja, dividindo a duração do aleitamento em intervalos fixos da vida da criança( , ). Nesse sentido, são limitados, porque analisam informações longitudinais como se fossem seccionais, embora sejam úteis quando se analisa o fato de a criança estar em aleitamento materno em um momento específico da vida. Nos MRC, a duração do aleitamento materno está vinculada à sobrevivência da criança até o desmame( , ), considerada mês a mês. Esses modelos são apropriados para a análise de dados longitudinais e não devem ser utilizados em análise de dados seccionais, quando não se sabe o histórico do desfecho (desmame). Contudo, sugere-se que tais técnicas de análise possam diferir entre si quanto à determinação dos fatores de risco e proteção do aleitamento materno.

Portanto, o objetivo deste estudo foi analisar os fatores associados à duração do aleitamento materno por dois modelos de regressão em coorte de mães e crianças acompanhadas por 30 meses em Vitória, Espírito Santo, Brasil.

Método

Realizou-se um estudo de coorte com 86 mães e crianças recém-nascidas, por 30 meses, incluindo-se todos os bebês de zero a três meses, residentes em duas áreas com cobertura da Estratégia em Saúde da Família pelo Sistema Único de Saúde no município de Vitória. Essas duas áreas foram escolhidas por apresentarem indicadores de mortalidade infantil, no ano de 2001, superiores ao coeficiente municipal (13,49/1.000 nascidos vivos)( ).

O tamanho da amostra foi calculado para estimar uma proporção, considerando-se os dados fornecidos pelo Ministério da Saúde do Brasil sobre o tamanho da população de nascidos vivos em 2001, em Vitória (4.521 crianças), e a prevalência de amamentação total na idade de 151 a 180 dias, para o ano de 1999 (74,8%). A precisão desejada foi de 10% e o nível de significância, de 5%. O tamanho da amostra estimado foi de 73 crianças. Portanto, a quantidade de 86 crianças indica uma margem de segurança de 18% para se considerar as possíveis perdas de seguimento.

O período de inclusão no grupo de estudo foi de novembro de 2003 a maio de 2004, momento em que os Agentes Comunitários de Saúde referenciavam os bebês na idade predeterminada a quatro acadêmicos do Curso de Odontologia da Universidade Federal do Espírito Santo que, treinados e divididos em duplas, acompanhavam a visita domiciliar nas residências dos sujeitos de interesse. Todas as mães e crianças abordadas foram incluídas no estudo, pois não houve impeditivos que justificassem a exclusão. Essa mesma equipe acompanhou os participantes por 18 meses; em seguida, outra equipe de constituição semelhante finalizou o acompanhamento, após treinamento específico.

As mães foram convidadas a participar da pesquisa e esclarecidas sobre natureza do estudo e a periodicidade das visitas domiciliares consecutivas. Sem exceção, todas as mães visitadas aceitaram participar espontaneamente, assinaram o termo de consentimento livre e esclarecido e passaram por uma entrevista para preenchimento de um formulário.

As visitas ocorreram em periodicidade programada (inicialmente, de três em três meses; depois, de seis em seis meses) e serviram de controle observacional da primeira visita. A cada visita, fazia-se o preenchimento de um novo formulário pela observação e entrevista estruturada aplicada às mães.

A coleta domiciliar de dados encerrou-se em junho de 2006, totalizando sete visitas domiciliares. O período contemplou o tempo de amamentação preconizado pela OMS, pois as crianças tinham idade média de 29,4±2,5 meses ao final do estudo.

A amamentação foi considerada de acordo com as definições das categorias de aleitamento materno recomendadas pela OMS( , ). Para a definição precisa dessa variável, a cada visita perguntou-se às mães se amamentavam os filhos ou utilizavam mamadeira ou outros bicos artificiais para a alimentação.

As variáveis de hábitos de sucção nutritivos e não nutritivos foram mensuradas pelo tempo de sucção de dedo,tempo de sucção de chupeta e tempo de uso de mamadeira.Definiram-se essas variáveis perguntando-se, em cada visita, se a criança manifestava tal hábito. Em caso afirmativo, registrava-se o período quando a criança iniciou e, caso tivesse parado, quando isso ocorreu. Em seguida, calculou-se o tempo.

A cada visita, a mãe foi questionada se havia introduzido à dieta da criança algum alimento, de qualquer natureza: frutas, legumes ou verduras amassadas ou alimentos industrializados. Em caso afirmativo, anotava-se o mês em que a criança iniciou tal hábito alimentar e quais os alimentos utilizados durante as últimas 24 horas.

As variáveis socioeconômicas foram coletadas na primeira visita domiciliar. Registraram-se dados sobre: escolaridade materna, renda familiar em salários mínimos, ocupação do pai, ocupação da mãe, idade materna, situação conjugal, estabilidade conjugal, número de pessoas que habitavam a mesma residência, acabamento da casa, número de cômodos, número de crianças com menos de cinco anos, número de filhos nascidos vivos da mulher, número de irmãos da criança (considerando-se a mesma mãe) e intervalo interpartal.

As análises estatísticas foram realizadas após a revisão e adequação do banco de dados aplicado no programa Statistical Package for the Social Sciences (SPSS) para Windows, versão 12.0, selecionando-se as variáveis de interesse. O nível de significância adotado para todos os testes realizados foi de 5%.

Os MRL foram construídos a partir das análises, considerando-se todas as variáveis do estudo e as variáveis de amamentação dicotomizadas, calculando-se a associação pelo teste do qui-quadrado de Yates. No MRL (1), utilizou-se como variável dependente o aleitamento materno exclusivo até três meses; no MRL (2), o aleitamento materno até seis meses; no MRL (3), o aleitamento materno até 12 meses. Consideraram-se como variáveis independentes aquelas que apresentaram o valor p menor ou igual a 0,10 na análise inicial. Para as variáveis independentes com forte associação entre si (por exemplo, uso de chupeta até o primeiro mês, uso de chupeta até o terceiro mês e uso de chupeta até um ano), utilizou-se no modelo apenas uma delas, ou seja, aquela com início do fator de risco mais precoce e maior Odds Ratio não ajustada em relação à variável dependente. Aplicou-se o método Stepwise forward LR não condicional para seleção automática de variáveis, o qual testa a entrada das variáveis com significância estatística por um escore e as remove utilizando a probabilidade da função de razão de verossimilhança, baseada na estimativa de máxima verossimilhança parcial. Adotou-se o nível de significância de 0,10 para entrada e 0,20 para a saída das variáveis no modelo.

Os MRC foram construídos a partir da análise de sobrevivência bivariada, cuja variável de interesse foi o tempo de aleitamento materno (em meses) e as variáveis independentes foram todas as outras do estudo. Calculou-se a curva de sobrevivência de Kaplan-Meier, a partir do tempo máximo de aleitamento materno, e utilizou-se o teste log-rank para comparar as curvas segundo os subgrupos. Em seguida, os MRC foram obtidos incluindo-se as variáveis que apresentaram o valor p menor ou igual a 0,10 no teste log-rank. Para as variáveis independentes com forte associação entre si (por exemplo, uso de chupeta até o primeiro mês, até o terceiro mês e até um ano), utilizou-se no modelo apenas uma delas. No MRC (1), o aleitamento artificial e a sucção de chupeta ocorreram mais precocemente; no MRC (2), encontraram-se variáveis com maior força de associação no teste log-rank; no MRC (3), o aleitamento artificial e a sucção de chupeta ocorreram mais tardiamente.

O protocolo de pesquisa foi aprovado pelo Comitê de Ética em Pesquisa do Centro Biomédico da Universidade Federal do Espírito Santo.

Resultados

Na visita inicial, 86 crianças (100%) foram analisadas com variação média etária de 1,58±1,05 meses. Na segunda visita, 81 (94,1%) foram abordadas em idade média de 4,89±1,55 meses. Na terceira e na quarta visitas, os participantes totalizaram 79 (91,8%) crianças, com variações etárias de 7,39±1,56 e 11,61±2,16 meses, respectivamente. Na quinta visita, 76 (88,4%) crianças foram encontradas e a idade média do grupo foi de 16,39±2,88 meses, enquanto, na sexta visita, 66 (76,7%) crianças participaram e a idade média foi de 21,92±3,03 meses. Na sétima visita, 67 (77,9%) crianças participaram, visto que uma criança retornou ao local domiciliar, totalizando uma média etária de 29,42±2,49 meses. As perdas de seguimento ocorreram devido à mudança de endereço ou à não localização no domicílio após, pelo menos, três tentativas. A média total do tempo de aleitamento materno foi de 15 meses (IC95% 13-18), assim como a mediana também foi de 15 meses (IC95% 10-20).

As análises dos MRL, apresentadas na Tabela 1, permitiram detectar os fatores de risco associados aos momentos de interrupção do aleitamento materno, ajustados por todos os fatores estatisticamente significantes, evidenciando a interação desses fatores. O início do uso de chupeta até o primeiro mês configurou-se como fator de risco em todos os padrões de amamentação considerados. O aleitamento artificial, até o primeiro mês de vida, também se configurou como fator de risco para interrupção do aleitamento materno nos MRL (2) e (3).

Considerando-se o uso de mamadeira, geralmente associado ao aleitamento artificial, a opção pela utilização de somente uma dessas variáveis permitiu a representação da sucção nutritiva sem prejudicar o modelo de análise. A variável uso de outros bicos artificiais antes dos seis meses só se manteve significativamente associada à interrupção do aleitamento materno, enquanto a erupção dentária antes dos seis meses ofereceu risco de interrupção do aleitamento materno apenas ao sexto mês de vida. O único fator de proteção foi o fato de a mãe ter idade igual ou superior a 35 anos.

Na Tabela 2, independentemente de o início da prática do aleitamento artificial ou do uso de mamadeira ser precoce ou tardio, todas as variáveis apresentaram valores de significância inferiores a 5%. Já as variáveis relacionadas à sucção de chupeta mostraram força de associação cada vez maior, à medida que o hábito se instalava mais tardiamente.

Na Tabela 3, quanto ao MRC (1), permaneceu significante somente a variável início do aleitamento artificial até o primeiro mês de vida, como um fator de risco para a interrupção do aleitamento materno; já a menor renda familiar permaneceu como um fator de proteção. No MRC (2), o início do aleitamento artificial até o primeiro mês e o início da sucção de chupeta permaneceram como fatores de risco associados à interrupção do aleitamento materno, enquanto a menor renda familiar manifestou-se como um fator de proteção. No MRC (3), apenas o início do uso de chupeta até o sexto mês de vida consolidou-se como um fator de risco para a redução do tempo de aleitamento materno. A renda menor ou igual a dois salários mínimos continuou como um fator de proteção para a manutenção do aleitamento materno.

A Figura 1 apresenta os fatores de proteção, de risco e intervalos de confiança associados ao tempo de aleitamento materno, considerando-se os valores de Hazard Ratio ajustados e de Odds Ratio ajustados. Apenas os fatores de proteção diferiram entre os modelos. A idade igual ou superior a 35 anos aparece apenas no MRL (3), enquanto a renda familiar mensal manifestou-se somente nos MRC.

Discussão

Nos estudos de coortes, em que a coleta de dados ocorre em tempos determinados, as análises por regressão logística são realizadas por meio da divisão do tempo de aleitamento materno em intervalos que são processados separadamente, como se cada momento fosse um estudo seccional em particular. Nos MRC, as análises consideram o tempo total de aleitamento materno, sendo a resposta o tempo até a ocorrência do evento (cessação do aleitamento materno), ajustado por covariáveis. Em ambos os modelos, pretende-se investigar o efeito conjunto de variáveis independentes, neutralizar os efeitos de confundimento e ajustar os resultados finais das Odds Ratio e Hazard Ratio.

Os resultados do modelo de MRL (1) sobre o aleitamento materno exclusivo, nos primeiros três meses de vida, reforçam que o uso da chupeta e a introdução de outros bicos artificiais até o primeiro mês de vida são decisivos para aumentar o risco de interrupção do aleitamento materno. O estudo de Mascarenhas et al ), envolvendo 940 mães e bebês de um a três meses de vida, mostrou que o uso de chupeta oferece risco para o aleitamento materno exclusivo. Já o estudo de Santiago et al ), com 101 díades acompanhadas por quatro meses, encontrou a não introdução de chupeta nesse período como fator de proteção ao aleitamento materno exclusivo. Quanto à utilização de outros bicos artificiais até o primeiro mês de vida, Wayland( ) a descreve como uma prática comum, principalmente para a introdução de água, chás e sucos na alimentação infantil. Entretanto, isso por si só caracterizaria o aleitamento materno não exclusivo( ). Certamente, as mães que relataram lançar mão desses artifícios utilizavam o próprio leite materno para alimentar seus bebês. Contudo, em posse desses objetos, possivelmente as mães tendem a incluir na alimentação outros alimentos que não o leite materno.

A erupção dentária antes do sexto mês esteve associada à interrupção do aleitamento antes dos seis meses de vida da criança no MRL (2). Supõe-se que o surgimento dos primeiros dentes, geralmente os incisivos centrais inferiores, pode representar para a mãe uma ameaça de ferimento do bico do seio durante a amamentação, dificultando sua manutenção por um tempo mais prolongado.

Já a idade materna maior ou igual a 35 anos apresentou-se como fator de proteção para o aleitamento materno por 12 meses ou mais no MRL (3). O estudo de Oliveira et al ), tendo como padrão de referência a idade de 20-34 anos, mostrou que, depois do ajuste ao modelo de análise, somente a idade menor que 20 anos apresentou maior risco para a interrupção do aleitamento materno, enquanto a idade superior a 34 anos não funcionou nem como fator de risco nem como de proteção em menores de 24 meses.

Em ambos os modelos de regressão utilizados, o uso da chupeta, independentemente do período de início, mostrou-se como um importante preditor do tempo de aleitamento materno, exceto no MRC (1), em que o início do aleitamento artificial até o primeiro mês anulou a influência da sucção de chupeta.

Nos MRL (2) e (3), as chances de interrupção do aleitamento materno foram altas no sexto e no 12º mês. Em concordância, Audi et al ) relatam maiores chances de abandono do aleitamento materno em crianças que usavam chupeta.Nos MRC (2) e (3), as crianças que usavam chupetas apresentaram riscos de cessação do aleitamento menores, mas significantes. Esses achados foram semelhantes a alguns estudos(17,18) e muito inferiores aos encontrados por Minagawa et al ) e Silveira e Lamounier( ), mostrando que, em menor ou maior intensidade, o uso da chupeta pode ser fator preponderante para a interrupção do aleitamento materno.

O aleitamento artificial apresentou-se como fator de risco para interrupção do aleitamento materno em todos os modelos, exceto no MRC (3). Deve-se considerar que as variáveis aleitamento artificial e uso de mamadeira são muito associadas, por isso, optou-se pela exclusão de uma dessas variáveis nos modelos de regressão. Howard et al ) mostram que existem diferenças entre a utilização de copos e mamadeiras para aleitamento artificial no tempo de amamentação e que o uso de mamadeira agrava o risco de interrupção do aleitamento materno, independentemente do tipo de leite utilizado - de origem animal ou industrializado.

Nos MRC, a renda menor ou igual a dois salários mínimos foi um importante fator de proteção para o prolongamento do aleitamento materno, o que não ocorreu nos MRL. Em MRL apresentados por Oliveira et al ), as populações classificadas com baixo índice de condição de vida em Salvador, Bahia, apresentaram os maiores riscos de interrupção do aleitamento materno em relação à classificada com índice elevado. Victora et al ) analisaram o comportamento dessa variável por meio de tabelas de sobrevivência em três coortes de nascimentos e demonstraram que, de seis a nove meses, o aleitamento materno é mais prevalente em famílias com renda mais alta, mas, após os 12 meses, a prevalência da amamentação em famílias mais pobres é significativamente superior.

A partir dessas observações, conclui-se que as diferenças de perfil socioeconômico podem interferir diretamente no tempo de aleitamento materno. No entanto, deve-se considerar que a população estudada não apresenta grandes diferenças quanto à classe social, pois reside em áreas carentes de recursos sociais. Por isso, as variáveis socioeconômicas foram descritoras de perfil e marcadoras de controle. Mesmo assim, as pequenas diferenças na renda familiar influenciaram a duração do aleitamento materno nos MRC.

Deve-se ressaltar que o método de acompanhamento por meio de visita domiciliar periódica não conseguiu estabelecer um padrão rígido de tempo entre as coletas de dados em todas as crianças, pois o movimento migratório realizado pelas famílias foi intenso. Isso se deve também ao fato de a perda amostral ter sido superior ao planejado inicialmente, embora sem prejuízos significativos na confiabilidade dos resultados.

O estudo sobre fatores de risco associados ao tempo de aleitamento materno é imprescindível para o planejamento e execução de atividades que eliminem seus efeitos sobre a prática do aleitamento materno. Além disso, a utilização de diferentes métodos de análise estatística pode levar a resultados mais detalhados, com a visualização de peculiaridades sob diferentes ângulos, desvelando resultados específicos que, embora não sejam idênticos, também não são contraditórios. Isso ocorre possivelmente porque, nos MRL, analisou-se de modo dicotômico o fato de a criança estar ou não aleitando no terceiro, no sexto ou no 12º mês. Nesses momentos, compreende-se que fatores contextuais acelerem o desmame da criança. Já nos MRC, os fatores que impulsionam a cessação do aleitamento materno são analisados como se estivessem presentes o tempo todo durante a trajetória da vida da criança até o desmame.

Este estudo sugere ainda que o combate ao uso de chupeta e ao aleitamento artificial precoce é necessário, a fim de propiciar a prática do aleitamento materno por tempo adequado, possibilitando a promoção integral da saúde da criança desde os seus primeiros anos de vida.
