‡Major depressive disorder defined as Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM-IV) major depression in contrast to clinically significant minor depression (defined as four DSM-IV symptom groups, HDRS score ≥10, and four week duration); range of scores for MMSE is 0 to 30 (inclusion criteria limited range to 18-30), with high scores indicating less cognitive impairment; HDRS range is 0 to 76, with high scores indicating greater depressive symptoms; SSI range is 0 to 38, with high scores indicating greater suicidal ideation.
