Abstract
OBJECTIVE: Recent US work identified "metabolically healthy overweight" and "metabolically at risk normal weight" individuals. Less is known for modernizing countries with recent increased obesity. DESIGN AND METHODS: Fasting blood samples, anthropometry and blood pressure from 8,233 adults aged 18-98 in the 2009 nationwide China Health and Nutrition Survey, were used to determine prevalence of overweight (Asian cut point, BMI ≥ 23 kg/m(2) ) and five risk factors (prediabetes/diabetes (hemoglobin A1c ≥ 5.7%) inflammation (high-sensitivity C-reactive protein (hsCRP) ≥ 3 mg/l), prehypertension/hypertension (Systolic blood pressure/diastolic blood pressure ≥ 130/85 mm Hg), high triglycerides (≥ 150 mg/dl), low high-density lipoprotein cholesterol (<40 (men)/ <50 mg/dl (women)). Sex-stratified, logistic, and multinomial logistic regression models estimated concurrent obesity and cardiometabolic risk, with and without abdominal obesity, adjusting for age, smoking, alcohol consumption, physical activity, urbanicity, and income.Entities:

Relative risk of falling into each category of cardiometabolic risk and overweight for abdominal obesity, sex-stratified multinomial logistic regression model results, with metabolically healthy normal weight as the referent category, 2009 China Health and Nutrition Survey. Models adjust for age, current smoking, drinking 7+ alcoholic beverages per week, engaging in high levels of total physical activity (across leisure, commuting, and occupational activity), urbanicity, and income
Metabolically healthy defined as none of the following risk factors: pre-diabetes or diabetes (HbA1c≥5.7%) inflammation (hsCRP ≥3 mg/L), pre-hypertension or hypertension (SBP/DBP≥130/85 mmHg), high triglycerides (≥150 mg/dL), low high-density lipoprotein cholesterol (<40 mg/dL, men; <50 mg/dL, women).
Metabolically at risk defined as one or more of the above risk factors
Overweight is defined using the recommended Asian BMI cutpoint of ≥23 kg/m2, with non-overweight <23 kg/m2.(17); Abdominal obesity (waist circumference: men ≥90 cm; women ≥80 cm) (13)
NOTE: Model interpretation: Abdominal obesity (versus no abdominal obesity) predicts membership in “metabolically healthy overweight” category at RRR=16.45 (CI 9.21–29.37) relative to membership in the “metabolically healthy non-overweight category”

Odds ratio for each risk factor modeled separately (i.e., separate models for inflammation, pre-hypertension or hypertension, etc.). Set A: overweight with (and without) control for abdominal obesity, Set B: overweight interacted with abdominal obesity, 2009 China Health and Nutrition Survey. All models adjust for age, current smoking, drinking 7+ alcoholic beverages per week, engaging in high levels of total physical activity (across leisure, commuting, and occupational activity), urbanicity, and income

Overweight is defined using the recommended Asian BMI cutpoint of ≥23 kg/m2, with non-overweight <23 kg/m2.(17) Abdominal obesity (waist circumference: men ≥90 cm; women ≥80 cm) (13)
NOTE: Set Model A 1 and 2, interpretation: For Men, overweight alone was associated with an increased risk of inflammation of 55%, which was no longer statistically significant with the addition of abdominal ovesity into the model, whereas abdominal obesity adjuting for overweight was associated with an increased risk of 70% for inflammation
NOTE: Set Model B interpretation: For Men, the combination of overweight and abdominal obesity was associated with an increased risk of inflammation of 2.03 relative to non-overweight with no abdominal obesity
