Criterion for entry of demographic and clinical factors into the final model on multivariate analysis was p < 0.2 on univariate analysis. Demographic factors entered into the model included age, gender, marital status, education level, and social network. Clinical factors entered into the model included: visual impairment; hearing impairment; had a fall in the past 1 year; overweight; impairment in iADL; having medical comorbidities as quantified using the Charlson Comorbidity Index; history of ischemic heart disease; history of cerebrovascular disease; history of depression, and average systolic blood pressure. All area and individual level indicators of socioeconomic status were also entered into the model. We did not enter impairment in basic ADL (measured via Barthel Index) into the final model because of significant collinearity between measurements of basic ADL and measurements of iADL (|r| > 0.6). Collinear variables were omitted from the final models in order of decreasing p values.
