Abstract
We report a patient with primary anti-phospholipid syndrome (APS) who developed pulmonary edema following sudden-onset pain in the left, lower back of the chest. Radiological examinations demonstrated fresh infarction of the left adrenal gland but no obvious thrombi in pulmonary arteries. The patient quickly recovered from pulmonary edema with anti-coagulation therapy alone. Primary APS may have caused adrenal infarction in the patient, leading to transient pulmonary edema via microthrombosis and/or excessive release of catecholamine.Entities:

Figure 1Computed tomography demonstrated multiple irregular-shaped ground-glass opacities in the left lung (A, arrows). On the next day ground-glass opacities had increased in size, and appeared also in the right lung (B, arrows). One day later these opacities had obviously reduced in size (C).
Figure 2Abdominal computed tomography demonstrated marked swelling of the left adrenal gland (A), which showed hyperintensity signals on diffusion-weighted image of magnetic resonance imaging (MRI) suggestive of infarction (B). T2* image of MRI revealed no hypointensity signals indicating hemorrhages (C).
