Hospitals and health systems should consider four strategies for improving documentation: Develop a short list of the most commonly underdocumented or incorrectly documented clinical conditions at the facility. Develop definitions for each of the conditions on this list. Ask the medical director of each specialty area to educate the clinicians in their group on these definitions.
