Abstract
Primary ovarian lymphoma is a rare malignancy whose symptoms or signs are usually nonspecific. In this article, we report a very rare case initially presenting as a rectal submucosal-tumor-like lesion with a defecation disturbance caused by primary ovarian lymphoma with bilateral involvement. A 42-year-old woman visited chungnam national university hospital complaining of persistent defecation disturbance for 6 months. Colonoscopy demonstrated compression of the rectum by an extrinsic mass mimicking a rectal submucosal tumor. Magnetic resonance imaging detected bilateral ovarian tumors, 9.3 cm and 5.4 cm each in diameter, compressing the rectum without enlarged lymph nodes. The diagnosis was established following a bilateral adnexectomy and histological studies of the excised tissue. The tumor was classified as a diffuse large B-cell lymphoma. The patient was prescribed six cycles of standard CHOP (cyclophosphamide, hydroxydaunorubicin, vincristine, prednisolone) regimen and is presently on treatment.Entities:

Fig. 1Colonoscopic findings. External compression of the rectum without mucosal involvement was observed.
Fig. 2Longitudinal view of the transabdominal ultrasound. Large ovarian masses, left (A) and right (B) were observed.
Fig. 3Magnetic resonance imaging findings. (A, B) Large heterogeneously enhancing right ovarian mass (about 9.2 × 8.2 × 9.3 cm in size) with a small cystic portion and elongated, slighty heterogeneously enhancing left ovarian mass (about 3.6 × 5.4 × 5.3 cm in size) were observed with minimal amounts of fluid collection in the pelvic cavity.
Fig. 4Macroscopic view of the surgical specimen. Left (A) and right (B) ovarian tumors, measuring 11 × 9 × 5 cm and 7 × 5 × 5.3 cm in diameter, respectively, were showed solid, whitish-tan, and homogenous cut surfaces. Focal necrotic foci are seen.
Fig. 5Histological findings of the ovarian mass. (A) The section shows diffuse proliferation of large lymphoid cells which show abundant eosinophilic cytoplasm and vesicular nuclei. The chromatins are dispersed along the nuclear membrane and 2-4 nucleoli are bound to the nuclear membrane or a single nucleolus defined in the center of the nucleus (H&E, × 400). (B) Tumor cells express strong positivity for CD20, the pan B-cell marker (Immunohistochemistry, × 400).
