91% (n=77) were found in other tissue sites and 9% (n=8) were classified as having an unknown primary site.
A total of 15 cases (6 thyroid carcinomas, 5 malignant melanomas, 1 hepatocellular carcinoma, 2 nasopharyngeal carcinomas, and 1 gonadal carcinoma) had missing data on maternal race.

Adjusted for state (CA, WA, MN, NY, TX), birth year category (1970–85, 1986–89, 1990–93, 1994–2004), maternal race (white, other), sex (male, female), maternal age as a continuous variable, gestational age (<37, 37–42, >42 weeks), plurality (multiple, single), birth weight (<2500, 2500–4000, >4000 g), and birth order (1, 2, 3, ⩾4).
4238 controls and 48 total carcinoma cases (n=18 thyroid carcinoma and 12 malignant melanoma cases) were excluded due to missing data on one or more covariates.
Adjusted for state (CA, WA, MN, NY, TX), birth year category (1970–85, 1986–89, 1990–93, 1994–2004), maternal race (white, other), sex (male, female), gestational age (37–42, <37, >42 weeks), plurality (multiple, single), birth weight (<2500, 2500–4000, >4000 g), and birth order (1, 2, 3, ⩾4).
9966 controls and 95 total carcinoma cases (29 thyroid carcinoma and 19 malignant melanoma cases) were excluded due to missing data on one or more covariates.
