SF-36 results are standardized for age and gender according to the score of the general Norwegian population (2002), such that a value of 50 is the mean of the general Norwegian population and 10 is the standard deviation, as described in Gandek .
P value for mean difference compared with the general Norwegian population found from one-sample t-test with a test value of 50. Significant difference if P value is <0·05.
P value for change from baseline to follow-up (n = 41) found from paired samples t-tests. Significant change if P value is <0·05.
Effect sizes (the size of changes) are judged against standard criteria: trivial (<0·2), small (0·2 to <0·5), moderate (0·5 to <0·8) and large (≥0·8) (Cohen 1978).
