Abstract
PURPOSE OF REVIEW: This paper will discuss the concepts of pre-emptive and preventive analgesia in acute and persistent postsurgical pain, based on the most recent experimental and clinical literature, with a special focus on injury-induced central sensitization and the development from acute to chronic pain. RECENT FINDINGS: The nature of central sensitization during acute and chronic postsurgical pain share common features, and there may be interactions between acute and persistent postoperative pain. The term 'pre-emptive analgesia' should be abandoned and replaced by the term 'preventive analgesia'. Recent studies of preventive analgesia for persistent postoperative pain are promising. However, clinicians must be aware of the demands for improved design of their clinical studies in order to get more conclusive answers regarding the different avenues for intervention. SUMMARY: The concept of preventive analgesia is still an attractive working hypothesis but with inconclusive results. A plea for better design of clinical studies is forwarded.
